The sera from 89 patients from the Eastern Higlands of Papua New Guinea, all with histologically diagnosed liver disease, were tested for Hepatitis B Antigen (HB Ag) and Hepatitis B antibody (HB Ab) and alpha1 fetoprotein (AFP) by a variety of techniques which included radioimmunoassay. In the three main forms of liver disease,
viral hepatitis
, cirrhosis and
hepatoma
, HB Ag was found with a higher frequency than in patients with non specific liver disease. The frequency of HB Ab was decreased in cirrhosis and
hepatoma
. AFP was detected in all
hepatoma
patients by radioimmunoassay, levels being very high in most subjects. In hepatitis, cirrhosis and non specific liver disease, elevated levels of AFP were again frequently present, but at generally lower levels. It is conlcuded that HB Ag and AFP frequency and levels in liver disease are similar to those reported from other tropical countries. Further study is required to elicit the cellular immunological changes in liver disease.

There are two well-characterized antigen-antibody systems which relate specificially to
viral hepatitis
B. Tests for HBsAg and anti-HBs are readily available and of great benefit to the diagnosis, prevention and understanding of hepatitis B. Tests for HBcAg and anti-HBc are still research techniques which requires further development before they can be used at the level of everyday medical practice. HBsAg in an individual indicates that he harbors the virus of hepatitis B; it may be present in the absence of liver disease or be found in association with both acute and chronic type B hepatitis. The presence of HBsAg also suggests that HBV may be causally related to some cases of periarteritis nodosa, chronic glomerulonephritis, and
hepatoma
. Although HBV is readily transmitted in blood, the major portion of post-transfusion hepatitis now appears to be serologically unrelated to either the hepatitis B virus ("serum") or the hepatitis A virus ("infectious"); the etiology of these cases is currently undetermined. There is increasing evidence that HBV may be transmitted by modes other than blood, but the exact mechanisms of such transmission is not established. The combined transmission of HBV by blood and other routes has resulted in a large number of persistent carriers of HBsAg in the world. There is no current method to alter this carrier state. The hepatitis risk of such persistent carriers to their personal and professional contacts is under investigation.

A radioimmunoassay method was used for the detection of alpha fetoprotein (AFP) in the sera of 112 Papua New Guinean patients who had undergone liver biopsy. Sera from 69 normal subjects and 20 hospital patients were also tested. Alpha fetoprotein was found to be elevated above normal levels in many of these subjects, but particularly in those suffering from
viral hepatitis
, cirrhosis and primary
liver cell carcinoma
(PLC). The highest values were found in patients with PLC. It is concluded that because of the elevation of AFP values in all these different types of liver disease the RIA test is not of great value in Papua New Guinea, except in the follow-up of some patients with cirrhosis who are at risk of developing PLC and in those who have undergone treatment for PLC.

, 66 other malignancies, 74 cases of cirrhosis of the liver, 60 of chronic aggressive hepatitis, 12 of chronic persistent hepatitis, 16 of subacute hepatitis, 36 of acute
viral hepatitis
, and 13 healthy hepatitis B-surface antigen (HBsAg) carriers. Double immunodiffusion and radioimmunoassay (RIA) were used in all cases. AFP greater than 10 ng-ml appeared in 90% of the cases, and was above 400 ng/ml in 69%. In 80% of those above 400 ng/ml, AFP could also be demonstrated by immunodiffusion. The AFP level in
hepatocellular carcinoma
was discovered to decline as the age increased. It also appeared to be related to the tumor cell type; the relatively immature cell type was more frequently associated with a higher AFP level. The presence of HBsAg did not influence the AFP level. Although the AFP in other malignancies and liver diseases ranged abnormally from 14 to 69%, the level did not exceed 400 ng/ml as in our cases of
hepatocellular carcinoma
(except in one case). Thus, this figure provides a diagnostic serum level of AFP for the identification of

The hypothesis that hepatitis B infection is etiologically related to
hepatoma
has been investigated by studying the interrelationships between hepatitis B surface antigen (HBsAg, Australia antigen) and the fast-moving 5'-nucleotide phosphodiesterase Band V isoenzyme (5'-NPDase-V). Sera from 58 patients with
viral hepatitis
were tested for 5'-NPDase-V and HBsAg. The isoenzyme was found in 34 of 37 patients who were also positive for HBsAg but in only 4 of 21 hepatitis patients who were HBsAg negative. Five patients convalescing from hepatitis were negative for both HBsAg and the isoenzyme. Preparative gel electrophoresis showed that these 2 markers were different proteins. Of 34
hepatoma
patients, 29 were positive for 5'-NPDase-V. Only 1 isoenzyme-positive patient was positive for HBsAg by counterimmunoelectrophoresis. However, of 16 isoenzyme-positive
hepatoma
patients available for radioimmunoassay, 8 were NBsAg positive (50%). None of 21
hepatoma
samples tested for antibody to NBsAg was positive. Of 21 "normal" carriers of HBsAg and 10 carriers with Down's syndrome, 4 persons were detected with the isoenzyme. The etiologic relationship of parasitic liver disease to primary liver cancer has long been debated. For this reason, a review of 4611 necropsies was carried out to determine the frequency with which
hepatocellular carcinoma
occurred in association with schistosomiasis. Of 227 cases of
hepatocellular carcinoma
, 24 (10.6%) were associated with schistosomiasis japonica. This was significantly higher than the incidence of this carcinoma without schistosomiasis (2.78%). The majority of the 24 cases exhibited the features of a mixed macronodular and micronodular cirrhosis (Gall's posthepatitic cirrhosis); this was super-imposed upon and caused a masking of schistosomiasis fibrosis. By radioimmunoassay hepatitis B antigen was positive in 27% of these cases. A review of the literature indicated that chronic schistosomiasis, on its own, is unlikely to be the cause of primary
liver cell carcinoma
. Histologic features resembling post-hepatitic cirrhosis combined with a high frequency of hepatitis B antigen suggest that
