Hemangiopericytoma growth in syngeneic male mice F1 (CBA X C57BL/bj) led to regular hemopoietic changes: an increase in the spleen weight, spleen cell count, in the number of colony-forming units (CFU). It also induced augmentation of myelopoiesis in the spleen and leukocytosis with a sharp increase of segmented granulocytes in the circulating blood characterized as the "leukemoid reaction" syndrome. The latter occurred even when the tumour cells were transplanted into the splenectomized host. Although less marked, this leukemoid reaction was also noted in advanced hepatoma transplanted to syngeneic male mice F1 (CBA X C57BL/6j). No leukemoid reaction was observed in these mice after grafting syngeneic strain of urinary bladder carcinoma.

We have studied the induction of a granulocyte-associated leukocytosis (leukemoid reaction) in C3HA, C57B1/6, and DBA/2 mice by a number of transplantable tumors of different origin. Leukemia L1210, Hepatoma 22, a transplantable mammary carcinoma of spontaneous C3HA origin, and a L929 culture fibroblasts-derived rhabdomyosarcoma, all induced a leukemoid reaction in their specific mouse strain. Melanoma B16 and Lewis lung carcinoma gave no reaction; Adenocarcinoma 755 and Harding-Passey melanoma evoked a leukocytosis but not due to an increase in neutrophils. Some extratumoral factors can influence the hematological response; the intensity of final leukemoid reaction was higher in female mice than in males bearing the same tumor. On the other hand, Ehrlich ascites tumor transplanted in all three inbred mouse strains rendered different levels of leukemoid reaction; response was higher in DBA/2, intermediate in C3HA and lower in C57B1/6.

Leukaemoid reaction is a rare, growth factor-driven, paraneoplastic manifestation of hepatocellular carcinoma. It may masquerade as the neutrophilic chronic myeloid leukaemia or as chronic neutrophilic leukaemia. A 52-year-old male presented with hepatosplenomegaly and severe leucocytosis. He had progressive leucocytosis, neutrophil alkaline phosphatase score elevated, liver function tests altered. FNAC from the mass in the liver revealed features of moderately differentiated hepatocellular carcinoma. The patient deteriorated within two weeks and died thereafter.

A leukemoid reaction is defined as reactive leukocytosis exceeding 50,000/mm(3), with a significant increase in early neutrophil precursors, and can be a paraneoplastic manifestation of various malignant tumors. A 71-year-old male patient complained of decreased appetite, fatigue, and abdominal fullness. He had a palpable, firm liver, and laboratory investigations suggested leukemoid reaction. Liver dynamic computed tomography revealed a hypervascular mass, and an ultrasound-guided fine-needle aspiration of the mass confirmed hepatocellular carcinoma (HCC) with a sarcomatoid component. The leukocyte count of the patient had increased to 147,800/mm(3), and he died 10 days after admission. This is a rare case of leukemoid reaction in a patient with sarcomatous HCC.
