A Caucasian male developed florid
dermatomyositis
documented by serum enzyme elevation, electromyography, and histology of skin and muscle. Serum enzymes, including creatine phosphokinase (CPK), aldolase, glutamic oxaloacetic transaminase (SGOT), and lactic dehydrogenase (LDH), decreased initially during high dose systemic corticosteroid therapy, although profound muscle weakness persisted. Subsequent elevation of serum LDH and SGOT levels during treatment provided a clue to underlying neoplasia. Primary
hepatoma
with widespread metastases was found at necropsy.

A 56 year old female developed combined hepatocellular cholangiocarcinoma associated with
dermatomyositis
. Serum tumour markers except for carbohydrate antigen (CA 19-9; 6400 ng/ml) were within normal range. Despite extensive chemotherapy, no clinical response was obtained and the patient's course deteriorated after 4 months. Macroscopically, the liver was mainly occupied by
hepatocellular carcinoma

The association of rheumatic syndromes and malignancy is highlighted in this review. The prevalence of malignancy in a series of patients with unclassified rheumatic syndromes is reported. Using the differential display polymerase chain reaction (DDRT-PCR) a 169 bp cDNA product, which is 88.8% homologous to the human Mi-2beta autoantigen, was identified in H4IIE rat
hepatoma
cells. At protein level 100% homology was found. The Mi-2 mRNA was downregulated after hypoosmotic exposure and upregulated after hyperosmotic exposure in H4IIE cells and rat hepatocytes. The human Mi-2 is an autoantigen in
dermatomyositis
and is a member of the SNF/RAD 54 helicase family. Accordingly, Mi-2 may not only be a target of osmosignalling but could also be involved in the osmosignalling pathway towards gene expression in H4IIE and liver parenchymal cells.

dermatomyositis
is very rare. In this report, we present a patient with dematomyositis with abnormal liver function test results and elevated alfa-fetoprotein level. After excluding multiple viral infections known to cause myositis, the case was proven to be chronic hepatitis C by positive HCV-RNA in the serum. Abdominal computed tomography showed a liver tumor on the right lobe and needle biopsy proved it to be
hepatocellular carcinoma
. Chronic hepatitis C or
hepatocellular carcinoma
might cause
dermatomyositis
by inducing the formation of autoantibodies. Chronic hepatitis C or
hepatocellular carcinoma
should be considered in patients of
dermatomyositis
if no other cause is found.

A 79-year-old female patient with hepatitis C virus-related liver cirrhosis was diagnosed as having
hepatocellular carcinoma

HCC
) with a diameter of 2.0 cm. She refused therapy for
HCC
. Nine months after the diagnosis, she developed
dermatomyositis
when the
HCC
enlarged to a diameter of 6.0 cm. She underwent therapy for
dermatomyositis
, and then transcatheter arterial chemoembolization for
HCC
. Although the manifestations of
dermatomyositis
improved and entire tumor necrosis was achieved, she died of pneumonia 2 mo after the treatment of
HCC
.
HCC
and/or chronic hepatitis C virus infection might be involved in the pathogenesis of
dermatomyositis

It has been proposed that DM may be caused by autoimmune responses to viral infections, and previous studies have also shown that an association between DM and malignancy. However, chronic hepatitis B virus (HBV) infection associated with DM and
hepatocellular carcinoma
(
HCC
) is rarely encountered. The authors report a case of DM and
HCC
in a patient with a HBV infection. A 58-year-old man presented erythematous skin rashes on a sun-exposed area of 2 year's duration, and recent proximal muscle weakness. His medical history revealed that he had a chronic HBV infection. A diagnosis of DM relies on proximal muscle weakness, elevated muscle enzymes, myopathic changes (demonstrated by electromyography), muscle biopsy evidence of myositis, and its characteristic cutaneous findings. A Liver mass in the left lobe visualized by abdominal computed tomography was confirmed histologically as
HCC
. This case suggests that DM associated with
HCC
might be caused by a HBV infection.

A man in his late fifties was treated with acitretin for psoriasis. The treatment was discontinued when liver enzymes started to increase. He subsequently developed erythrodermia, weakness of proximal muscle groups, heliotrope rash, Gottron's papules and elevation of creatine kinase to more than 9000 U/L. However, histological examination of a muscle biopsy displayed neuropathic atrophy and only a few scattered necrotic fibers. Alpha 1 foetoprotein was substantially elevated (1600 kU/L) and computed tomography showed two hepatic tumors, lytic lesions in the spine and in the ninth rib. Although no chronic liver disease could be documented, the patient was found to have a
hepatocellular carcinoma
which presented as
dermatomyositis
and erythrodermia.

coinciding with HBV-associated
HCC
. A 55-year-old male was confirmed to have
HCC
and
dermatomyositis
based on proximal muscle weakness, typical skin manifestations, elevated muscle enzyme levels, and muscle biopsy findings. This case suggests that
HCC
and/or a chronic HBV infection may be factors in the pathogenesis of
dermatomyositis
through a paraneoplastic mechanism.
