The aim of current study was to evaluate the prospects of adjuvants against DNA vaccination (pES/2SS) encoding
somatostatin
(SS) and
hepatitis B
surface antigen fusion gene. A total of 60 female Hu lambs were divided into 6 groups and vaccinated in the context of various adjuvants (and controls): pE-CpG, Escherichia coli DH5alpha DNA, crude liposomes or GM-CSF in combination with the pES/2SS plasmid. Controls included pES/2SS only vaccinated and physiological saline groups. The antibody against SS level in the E. coli DH5alpha DNA group was significantly increased compared to that in the pES/2SS vaccine alone. Vaccination with pES/2SS/pE-CpG or pES/2SS/E. coli DH5alpha resulted in elevated weight gains that were 33.0 and 31.6% higher, respectively, than in saline group and pES/2SS only vaccinated controls. The concentrations of GH and IGF-I in the DNA vaccine groups were remarkably higher than those in the saline group, and those with positive antibody higher than negative antibody. These results suggested that different adjuvant/pES/2SS combinations can enhance the immune effect and had significant positive effects on growth.

We describe the case of a 54 years old woman, with
hepatitis B
, in whom the diagnosis of a 6 cm hepatocellular carcinoma (HCC) in the left liver was made in 2001. Alpha-foeto-protein (AFP) was 63 ng/mL (NI < 10 ng/mL). After work-up including liver and tumor biopsy confirming HCC and only fibrosis in the nontumoral liver, left hepatectomy was performed. Final pathology showed a well differentiated HCC with tumoral portal vein thrombosis. Unfortunately, lung and mediastinal adenopathies were detected by CT scan 17 months later. Mediastinal nodes were punctured by endosonographic ultrasound, confirming HCC. The patient started treatment with Lanreotide 30 mg twice a month (Somatuline PR, Ipsen). Three months later, CT showed decrease in size of the mediastinal nodes and complete disappearance of the lung nodes. This objective response lasted for 42 months. The treatment was without any significant side effect. Retrospectively, immunohistochemistry was performed to detect somatostatine receptors (sstr) 2. Both the primary tumor and the node showed intense membranous and cytoplasmic staining for sstr2. In 2006, AFP rose and CT showed the appearance of a new mediastinal node. At that time, octreoscan was performed and showed uptake in the new node, although insufficient for metabolic radiotherapy. This case suggests that, although a number of randomized controlled trials did not show a benefit of
somatostatin
analogues in the treatment of advanced HCC, a subset of patients could benefit from treatment provided their tumor expresses sstr2, on which the existing drugs are efficient.

Liver resections for hepatocellular carcinoma (HCC) in cirrhotic livers are associated with early recurrence and poor survival.
Somatostatin
analogues (SSAs) have been reported to inhibit cell proliferation by interacting with specific
somatostatin
receptors (SSTRs) 2 and 5. The present study investigated whether SSTR expression in HCC was associated with the clinical outcome following octreotide long-acting release (LAR) treatment. Paired tumor and cirrhotic liver samples were obtained following a liver resection from 99 patients with stage I-II HCC and HBV-related cirrhosis. The expression of SSTR2 and 5 was assessed using quantitative (q)PCR and immunohistochemistry. The patients were classified into two groups, the high expression (n=47) and low expression (n=52) groups, based on the gene expression levels. The clinicopathological data and survival results of the two groups were compared. When compared with the surrounding cirrhotic tissue, the SSTR2 and 5 mRNA levels were significantly decreased in the HCC tissue. There were no significant differences between the groups with respect to the baseline characteristics. The tumor recurrence rate was significantly lower in the high expression group compared with that of the low expression group (63.83% vs. 82.69%; P=0.033). The 1-, 3- and 5-year disease-free and overall survival rates of the high expression group were 97, 89 and 71% and 98, 89 and 74%, respectively. The survival time of the members of the high expression group was longer compared with that of the low expression group. The multivariate analysis revealed that the TNM-7 stage and SSTR2 expression were independent prognostic factors for survival. In conclusion, SSTR mRNA expression correlated with survival in patients with early-stage
hepatitis B
virus (HBV)-related HCC who were treated with octreotide LAR following surgery. The inhibitory effects of SSAs on tumor growth may be mediated by SSTR expression.
