The prevalence of hepatitis B virus infections was examined in four groups from Liberia using radioimmunoassays for HBsAg, anti-HBs and anti-HBc. At the age of 15-19 years 90% have been infected, at the age of 40 years 100%. In a rubber plantation 30% had HBsAg, in the remaining 3 groups only 13% were HBsAg-positive. In the first group onchocerciasis was also a more frequent finding. However the parasitic infection was not the cause of the HBs-antigenemia. The HBsAg-concentration is mostly lower in the positive Liberians than in German HBsAg-carriers. Apparently the HBV caused frequently latent infections with low production of viral antigens in Liberia.

Liberian sera collected in 1973 from 592 residents of agricultural and iron ore mining companies were tested for LAV (HTLV-III), now known as HIV, by ELISA and western blotting, and no positives were found. The ELISA tests were kits from ELAVIA, Institute Pasteur, France, and VIRAMED, Electro Nucleonics, USA, and the immunoblot method was that of Towbin et al. The subjects included 430 men and non-pregnant women who had no positive findings, and 162 pregnant women at parturition, of whom 7 had positive ELISAs but negative confirmatory western blots. This population had high rates of onchocerciasis, hepatitis B, nematodes, Schistosomas, Marburg virus and Ebola virus.

The health profile of two groups of Ethiopian immigrants in Israel was evaluated. Trichophytosis and active trachoma were observed in 5%-8.7% of prepubertal children. Among adults 13% had corneal opacities and 10.4% had cataracts. Goiter, spastic bronchitis, valvular heart disease, leprosy and onchocerciasis were prevalent. Malnutrition was common, with average body weights ranging from 72.4% to 85.2% of normal. Anemia was observed in 6.3% of young children and 70% of all immigrants. Fifty percent of children over 12 years and 98% of adults over 40 years of age had been exposed to hepatitis B virus; 9.8%-11.8% were HBsAg carriers. Tuberculin tests were positive in 9.8%-13%; and intestinal parasites were identified in 86%-98%. Hospitalization was required for 15.7% of immigrants arriving during the period 1978-90, and for 3.7% of those arriving in mid-1991 (Operation Solomon).

A comprehensive medical evaluation to identify persistent and untreated tropical infections among members of the Sudanese group "Lost Boys of Sudan" living in Atlanta, GA, was initiated. Medical examinations and laboratory testing including blood cell counts, liver function tests, stool studies for parasites, hepatitis B serologies, and serologic testing for Schistosoma spp., Strongyloides, and filariae were performed. Preliminary results showed a high prevalence of untreated active schistosomiasis and strongyloidiasis infections in this group, 5 years after their resettlement in the United States. In addition, we found that many of them were infected with onchocerciasis and hepatitis B. We suggest that based on these preliminary results, pre-departure presumptive treatment and/or testing algorithms need to address some of these persistent tropical infections.
