OBJECTIVES: The objective of this article is to make an analysis of the dynamics of the imunization schedule and an updating of the practical aspects of the vaccination. METHODS: The authors, based on the official recommendations, in the imunization schedule of the Infectology Department of the Brazilian Society of Pediatrics and on their experience, present practical aspects to facilitate the understanding of the dynamics of application of the calendar. RESULTS: The current calendar of the Brazilian Society of Pediatrics (

) is presented with a practical analysis of the vaccines BCG,
hepatitis B
, poliomyelitis, Haemophilus influenzae type b (Hib), DPT and triple viral, which are also part of the Calendar of the National Program of Immunizations. Besides this, they analyze two other suitable vaccines for
SBP
, against varicella and hepatitis A. Finally they comment on the risk of urbanization of the yellow fever and the increasing indication of vaccination against this disease in Brazil. CONCLUSIONS: The imunization schedule should be dynamic, adapted to the epidemiologic characteristics of each country or place. The presented calendar is what is now recommended by the Infectology Department of the Brazilian Society of Pediatrics (

In a population-based sample, after excluding alcohol consumption, hepatotoxic drugs and
hepatitis B
and C infected, we investigated if alanine-aminotransferase (ALT) was associated with metabolic syndrome and insulin resistance, and if this association was caused by non-alcoholic fatty liver disease (NAFLD). The sample (432 female and 119 male) was divided into two ALT thresholds corresponding to the 50th and 75th percentiles (P) (female > or = 15 and > or = 19 U/L; male > or = 17 and > or = 23 U/I, respectively). Blood pressure, body mass index, waist circumference, cholesterol, HDL cholesterol (HDLc), triglyceride (TG), TG/HDLc ratio, glycemia and homeostasis model assessment of insulin resistance (HOMA-IR) were compared between those above and below each ALT threshold. Female placed above the 50th P of ALT had higher levels of TG/HDLc ratio (p=0.029), glycemia (p=0.028), and homeostasis model assessment of insulin resistance, (p=0.045), and above the 75th P had higher
SBP
(p=0.036), DBP (p=0.018), TG (p=0.024), TG/HDLc ratio (p=0.028), glycemia (p=0.004) and HOMA-IR (p=0.0014). Male placed above the 50th P of ALT had higher BMI (p=0.017) and TG/HDLc ratio (p=0.048), and above the 75th P had lower values of HDLc (p=0.042). Only 16.5% of women and 14.5% of men, above the 75th P of ALT, showed an increase in liver brightness in the echography. This work shows in woman an early association of ALT with TG/HDLc ratio and HOMA-IR. Since the last two are independent predictors of cardiovascular risk, attention should be drawn to ALT values near the upper limit of the normal range even in the absence of NAFLD and obesity.

) shows a specific binding ability to HBV surface antigen HBsAg. Previous work proved an ability of
SBP
to enhance the immune response of HBsAg vaccine. To investigate the function and mechanism of this protein, we constructed
SBP
-expression strains with Pichia pastoris expression system. We screened these strains and have got an expression strain with high protein expression quantity. Fermentation product was collected and purified to gain a large amount of purified protein. Identification of purified
SBP
with SDS-PAGE, High performance liquid chromatography, Western blotting and mass spectrometry suggested that the protein was highly purified and with a good integrity. ELISA test of purified
SBP
showed a significant binding ability to HBsAg, suggesting a good protein activity. This work offers a solid foundation to the research of
SBP
function and mechanism of immune enhancement.

A high titer of antibody to HBsAg (
Hepatitis B
virus surface antigen) (anti-HBs) is a requisite for the prevention of HB (
Hepatitis B
), and adjuvants generally play a great role in eliciting special anti-HBs to HB vaccine. However, adjuvants still need to be improved because of their shortages such as unremarkable efficacy, undesirable side effect or poor security. In this study, we used HBsAg separated from HB patient sera to screen a human liver cDNA expression library, and found a novel HBsAg-binding protein (
SBP
), which is located at the human chromosome 14q32.33 and is similar to human IgG heavy chain in structure. Western blot demonstrated that
SBP
existed in both healthy human sera and HB patient sera. Furthermore,
SBP
could bind to HBsAg by its N-terminal domain. Notably, we confirmed that
SBP
could promote dendritic cells (DC) to phagocytize HBsAg more effectively and enhance the immunogenicity of HB vaccine, when
SBP
was mixed proportionally with HBsAg and the resulting mixture was infused into mice. These results suggest that
SBP
could be developed into a safe and promising adjuvant of HB vaccine.

vaccine. The results indicate that
SBP
could enhance the HBsAg-specific immune response, thus increasing the protection provided by the
hepatitis B
vaccine. The safety data obtained here warrant further investigation of
SBP
as a vaccine adjuvant.
