Anti-liver-specific membrane lipoprotein (anti-LP-1) and anti-Tamm-Horsfall glycoprotein (anti-THGP) rabbit antibodies were found to bind to Chang liver cells, a cultured human hepatocyte cell line, and PLC/PRF/5, a hepatoma cell line. The antibodies bound were determined by an immunofluorescence staining and a semiquantitative 125I-protein A binding assay. The 125I-protein A binding assay was successfully adapted to determine anti-hepatocyte plasma membrane antibodies in sera of patients with lupoid hepatitis and chronic active hepatitis. The percentage of 125I-protein A bound in 10 normal subjects were 1.5 +/- 0.4 (mean +/- standard deviation) for PLC/PRF/5 and 1.6 +/- 0.6 for Chang liver cell, while those in 2 patients with lupoid hepatitis were 7.2 +/- 0.3, 5.9 +/- 0.1, and those in 8 patients with chronic active hepatitis 3.9 +/- 1.3, 3.2 +/- 1.5, respectively. Furthermore, a blocking study revealed that LP-1 and THGP were partially involved in antigen sites recognized with anti-hepatocyte plasma membrane antibodies in sera of a patient with lupoid hepatitis. The retaining ability of antibody binding to the hepatocytes after the absorption with non-hepatocyte cells suggested the presence of antibodies specific for the hepatocyte plasma membrane in the patient's serum.

The levels of antibodies of the IgG, IgA and IgM class against the Tamm-Horsfall glycoprotein (anti-THP) were determined in 108 patients with the following diagnoses: acute hepatitis A, acute hepatitis B, acute hepatitis non-A non-B, chronic hepatitis B, chronic hepatitis non-A non-B and chronic autoimmune hepatitis. The mean levels of antibodies of the IgM class were higher in all types of hepatitis than in healthy controls, while the mean levels of anti-THP of the IgA and IgG class were increased in acute hepatitis B only. 13 of 20 patients (65%) with acute hepatitis B exhibited increased levels of anti-THP of the IgA class compared to 6-18% of the patients in the other categories. The increased levels of anti-THP of the IgA class could not be attributed to elevated serum IgA but instead were correlated with the levels of serum aminotransferase. The possible impact of this particular association of anti-THP with acute hepatitis B is discussed.

To study autoantibodies against liver cell surface membrane clinically, anti-LP-1 and anti-Tamm-Horsfall glycoprotein (THGP) were determined in the sera of patients with various liver diseases. They were detected by ADCC assay using antigen-coated cells as the target. A high incidence of anti-LP-1 was seen in chronic hepatitis (CH), liver cirrhosis (LC), primary hepatic cancer with cirrhosis (PHC), and primary biliary cirrhosis. The incidence of anti-THGP was also high in CH, LC, and PHC. Both anti-LP-1 and anti-THGP were detected in 2 of 3 patients with lupoid hepatitis. The patients studied here had no obvious evidence of renal tubular acidosis or pyelonephritis. Serum alanine transaminase activity, serum gamma-globulin content, and the presence of rheumatoid factors were not associated significantly with the presence of anti-LP-1 or anti-THGP in chronic liver disease. In 7 cases of CH tested serially during their clinical course, anti-LP-1 and/or anti-THGP tended to appear during acute exacerbations. The demonstration of anti-LP-1 and anti-THGP suggested that their appearance was related to the development of chronic liver disease.
