The true etiology of severe aplastic anemia is unknown; however, autoimmune activation of T-lymphocytes is one of the potential causes. Stem cell transplantation is regarded as a first line therapy if a fully matched sibling is available. Immunosuppressive therapy is reserved for those who have no matched sibling available for transplant, or for those individuals who fall outside the age range eligible for stem cell transplantation. This case describes a child with a
hepatitis
-associated severe aplastic anemia for whom a fully matched sibling was available but a transplant was postponed due to active
hepatitis
. While awaiting bone marrow transplantation, the child acquired a life-threatening aspergillosis infection treated with amphotericin B, granulocyte infusions, and surgical resection of the involved lung. A decision was made to proceed with immunosuppressive therapy while waiting for successful treatment of the fungal infection. Following administration of equine anti-thymocyte globulin (ATG), high dose granulocyte colony stimulating factor (G-CSF), cyclosporine, and steroids, the child had partial hematopoietic reconstitution and is now followed in our clinic. This case demonstrates the extraordinary multidisciplinary care required during the early phases of treating severe aplastic anemia. With such care, recovery is a possibility.

For this retrospective study, infectious bursal disease virus (IBDV) was detected in 134 bursal samples that originated from flocks with conditions such as airsacculitis, tracheitis, pneumonia, septicaemia, inclusion body
hepatitis
, coccidiosis, and/or a history of production problems without overt clinical symptoms. Samples were from seven Canadian provinces: Ontario, Quebec, Manitoba, British Columbia, Nova Scotia, Alberta, and Newfoundland and Labrador. Viral RNA was identified in bursae with moderate to severe and acute to chronic bursal damage. The ages of the flocks from which samples were collected ranged from 3 to 63 days. Following reverse transcriptase-polymerase chain reaction the nucleotide sequence of the VP2 hypervariable region was determined and compared with sequences available in GenBank. The most common Canadian IBDV field strains were North-American variant viruses. Forty-four viruses were highly related (97.5% to 100.0%) to the US IBDV strain NC171. Moreover, 16 field viruses whose VP2 sequences were 99.2% to 100% identical to the South African 05SA8 IBDV strain appeared closely related to the NC171 group. Delaware E-related field viruses, 98.3% to 100.0% identical to the prototype virus, were identified in 33 samples. Thirty-four Canadian IBDVs showed the highest identity, 94.2% to 98.3%, to US IBDV strain 586. Five samples contained vaccine-related viruses, while two field strains showed the best match to

Herbal products are widely used by American consumers. Herbal remedies are not regulated by the Food and Drug Administration, but they are not immune from serious medication side-effects. We report the case of a 50-year-old woman who presented with fatigue and right upper quadrant pain. The patient had begun the popular postmenopausal herbal remedy black cohosh two weeks prior to presentation. Laboratory results revealed acute hepatitis. After other causes of liver failure were ruled out, the patient was diagnosed with black cohosh-induced
hepatitis
. She recovered uneventfully following withdrawal of the herb. There are five prior reports of
hepatitis
or hepatic failure likely caused by the herbal remedy black cohosh in the English literature. This case illustrates the importance of a broad differential diagnosis for abdominal pain and highlights the importance of a complete medication list, including herbs.

polymorphism in patients with chronic Hepatitis B virus (HBV) infection in a case-control study. Genomic DNA was extracted from 263 patients with chronic HBV
hepatitis
and 246 control subjects and was examined for the HLA-G 14-bp Ins/
Del
polymorphism by PCR. The polymorphic variants were genotyped in chronic HBV seropositive cases stratified according to HBV DNA levels, fibrosis stages and in a control population. There was no statistical significant association between the 14-bp Ins/
Del
polymorphism and increased susceptibility to HBV infection neither for alleles (P = 0.09) nor for genotypes (P = 0.18). The stratification of HBV patients based on HBV DNA levels revealed an association between the 14-bp Ins/
Del
polymorphism and an enhanced HBV activity with high HBV DNA levels. In particular, the Ins allele was significantly associated with high HBV DNA levels (P = 0.0024, OR = 1.71, 95% CI 1.2-2.4).
