The authors report a case of type A viral hepatitis in an 8 year old boy in whom the clinical course was marked by prolonged cholestasis associated with porta hepatis lymphadenopathy. The positivity of anti-HVA IgM, the negativity of the other serodiagnostic tests, the parallel course of the hepatitis and the porta hepatis lymphadenopathy argus strongly in favour of the responsibility of hepatitis A in the development of this porta hepatis lymphadenopathy.

The aim of the present study was to define and characterize the occurrence of a hepatitis outbreak. A hepatitis outbreak developed from November 1987 and June 1988 in a peripheral district of the city of Granada. Sixty cases were diagnosed (59 confirmed and 1 probable). An increase in the incidence was observed during October, November and December. There were no sex differences in the specific attack rates and the mean age of cases was 7.14 years; the specific rates in the different age groups were higher in the 0-5 years and 6-10 years groups. The most common symptoms and signs were choluria, abdominal pain and jaundice. Anti-HVA IgM was positive in 83.3% of cases. Only 5 children required hospital admission, and the outcome of 83.3% of cases was known and favorable. The strategies adopted against the outbreak were to recommend hygienic measures in the households and school where cases had developed and to give nonspecific human immunoglobulins before or after exposure in the closed contacts of cases. We do not feel that these strategies have had a sizeable effect on the course of the outbreak.

The results of study of the clinical picture and outcomes of type A viral hepatitis (HA) in 80 male adolescents aged 15-18 years are presented. The diagnosis was confirmed serologically. A greater incidence of the protracted forms of HA has been found in adolescents which in most cases had a favourable outcome. Protracted hepatitis was marked by long-term persistence of anti-HVA IgM. The chronic form of viral hepatitis had a course of a mixed A and B viral infection.

The presence of IgM antibodies for the HVA and HBc antigens in 90 children of 15 or less years with acute hepatitis were investigated, finding that 62% (58) were positive for HVA, 8.9% (8) for HBc and 27.8% (25) did not show antibodies for HVA or HBc (non-A non-B hepatitis); comments are made on some clinical and epidemiological findings such as a higher frequency of blood transfusions in HNANB, more males in HB and HNANB, and more hepatomegaly and liver sensitivity in HNANB.

Hepatitis A virus can manifest with severe and unusual neurological attack. We report the case of a 59-year old man with balance disorder associated with lower limb paresthesias and sphincter disorders. Spinal MRI was in favor of acute myelitis. Etiologic assessment showed viral hepatitis A (HVA) -specific IgM virus as the sole abnormality. Despite high-dose corticosteroid therapy, only partial recovery was seen. A systematic etiologic search for infection caused by hepatitis A virus (HAV) should be performed in every patient with acute myelitis even in the absence of clinical or biological signs suggestive of hepatitis, especially in endemic countries where vaccine prophylaxis is lacking.
