The examination of 5-cholestasis indicating enzymes was carried out in patients with viral B
hepatitis
. The changes in activity of such enzymes: alkaline phosphatase, its liver isoenzyme, gamma glutamyltranspeptidase, 5'nucleotidase, leucine aminopeptidase and
alanine aminopeptidase
were estimated as for being useful in discovering the states of cholestasis. Already in the first week of observation all examined enzymes showed the highest level of activity, only in the case of FZ and FW, however the difference between the patients with and without cholestasis were markedly static. In the following weeks the differences in average activities between group with and without cholestasis were present in the case of 5'N and LAP. The activity of GGTP and AAP were high in both groups of patients. And so alkaline phosphatase is the enzyme which discovers the states of cholestasis in viral B
hepatitis
quickly and markedly.

The concentrations of triiodothyronine (T3), thyroxine (T4), T3/T4 ratio, free thyroxine index, and thyroxine-binding globulin were investigated in 114 viral hepatic disease patients and 36 controls. The T3/T4 ratio in healthy hepatitis B virus carriers was significantly greater than those in the controls and fulminant, acute, or chronic active hepatitis patients. The T3/T4 ratio in the fulminant
hepatitis
patients was significantly less than those in the controls and other liver disease patients. The correlation coefficient between the T3/T4 ratio and microsomal
arylamidase
activity in liver tissue from 30 patients was 0.78 (p less than 0.001). The correlation coefficient between the T3/T4 ratio and the content of cholinesterase was 0.64 (p less than 0.001). These results suggest that the T3/T4 ratio represents a marker of microsomal function and is useful for estimation of prognosis of fulminant
hepatitis
or differentiation of various viral hepatic diseases.

In a survey the present possibilities are outlined to get knowledge about diseases of inner organs with the help of enzyme determinations in the urine. Here it is remarkable that changes of the enzyme excretion appear not only in renal disease with acute renal failure, pyelonephritis, glomerulonephritis, renal infarction and nephroptosis but are also to be observed in primarily extrarenal diseases such as diabetes mellitus, hyperthyroidism, thesaurismoses, myocardial infarction, hypertension, acute pancreatitis, epidemic
hepatitis
, liver cirrhosis, obstructive jaundice and rheumatoid arthritis. The causes of the changes of enzyme excretions are various. Since enzymes of different origin and localisation behave themselves variably, the simultaneous determination of a brush border marker (e.g.
alanine aminopeptidase
), a lysosomal enzyme (e.g. beta-glucuronidase or N-acetyl glucosaminidase) and a low molecular enzyme (e.g. lysozyme) is of use for the recognition of renal alterations. By the control of activities of urinary enzymes it is possible to get without risk informations about pathobiochemical processes in the kidney which are not to be gained by means of other methods.
