A case of fatal radiation
hepatitis
occurring after treatment with 2200 rad given by the moving strip technique is presented. This fatality occurred in a 28-year-old woman with persistent
ovarian carcinoma
despite prolonged chemotherapy. Scans of the liver using computerized tomography, 99mTc sulfur colloid, and 99mTc diisopropyl iminodiacetic acid showed abnormalities only in sulfur colloid uptake and these resolved late in the course. Death was caused by progressive hepatic and renal failure. Postmortem examination showed changes typical of radiation
hepatitis
. Only 32 cases of radiation
hepatitis
have been previously reported and they are reviewed. The possible role of prolonged prior chemotherapy in enhancing sensitivity to radiotherapy is considered.

This study was designed to investigate the usefulness of serum ferritin determinations for the diagnosis of cervical squamous cell carcinoma. The origin of ferritin in the circulation of these patients was also studied by an in vitro incubation system. Ferritin levels were determined by a radioimmunoassay kit (SPAC kit, Daiichi Radioisotope Lab.). Pretreatment serum ferritin levels were significantly higher (p less than 0.05) in patients with cervical squamous cell carcinoma,
ovarian carcinoma
,
hepatitis
and anemia than in normal women. All cases with endometrian cancer showed normal ferritin levels. Among patients with cervical squamous cell carcinoma, stage IV and recurrence groups showed higher ferritin levels than other stages. In vitro incubation studies revealed that squamous cell carcinoma could release significantly larger amount of ferritin than normal squamous epithelium. In addition, circulating and tissue ferritin of squamous cell carcinoma had the same immunological behavior in a ferritin radioimmunoassay, and also showed the identical localization on isoelectrofocusing gels. These results indicated that (1) circulating ferritin in patients with squamous cell carcinoma would, at least in part, be derived from the tumor tissue, and (2) serum ferritin determinations would be useful for the management of patients with cervical squamous cell carcinoma.

The diagnosis of hepatocellular carcinoma (HCC) is based on imaging studies particularly in high-risk patients without histologic confirmation. This study evaluated the prevalence and characteristics of false-positively diagnosed HCC in a liver resection cohort for HCC. A retrospective review was performed of 837 liver resection cases for clinically diagnosed HCC between 2005 and 2010 at our institute. High-risk patients with tumors > 1 cm with one or two image findings consistent with HCC and tumors < 1 cm with two or more image findings consistent with HCC with persistently increased serum alpha-fetoprotein (AFP) levels above the normal range with underlying inhibited
hepatitis
activity underwent liver resection. The false-positive rate was 2.2% (n = 18). Of the 18 patients, 7 patients (0.8%) were diagnosed with benign conditions (one each of hemangioma, inflammation, cortical adenoma, dysplastic nodule, angiomyolipoma, bile duct adenoma, and non-neoplastic liver parenchyme) and 11 patients (1.3%) were diagnosed with malignancies (cholangiocarcinoma [n = 6], hepatoblastoma [n = 2], and one each of lymphoepithelioma-like
carcinoma, ovarian
cystadenocarcinoma, and nasopharynx carcinoma metastasis). The clinical characteristics of pathologically diagnosed HCC patients were similar (P > 0.05) compared to non-HCC patients except for higher rate of history of alcoholism (P < 0.05) observed in non-HCC patients. Four of 18 non-HCC patients (22.2%) showed diagnostic discordance on the dynamic imaging study. Despite the recent progression in diagnostic imaging techniques, 2.2% of cases were false-positively diagnosed as HCC in a liver resection patient cohort; and the final diagnosis was benign disease in 0.8% of liver resection patients clinically diagnosed with HCC.

Checkpoint inhibitor immunotherapy, including ipilimumab and nivolumab, is associated with numerous immune-related adverse events including dermatitis, pruritus,
hepatitis
, diarrhea, and hypophysitis. As the number of patients undergoing immunotherapy treatment increases, however, rare and unusual immune-related adverse events are observed. Many of these resemble known autoimmune phenomenon, such as subacute lupus erythematosus and myositis. Herein, we report a patient with metastatic serous
ovarian carcinoma
undergoing treatment with combination ipilimumab and nivolumab who developed subacute cutaneous lupus erythematosus (SCLE). Recent case reports have documented SCLE as a novel immune-related adverse event. In our case, she was able to successfully restart immunotherapy after a course of oral corticosteroids and maintenance oral hydroxychloroquine and topical corticosteroid therapy.
