Physicians treating adolescents should take a complete sexual history, including sexual orientation and practices, to determine whether their patients are homosexually active. Lesbians are at very low risk for sexually transmitted diseases, but they do have other health concerns. Four general groups of conditions may be encountered in homosexually active men: classical sexually transmitted diseases (gonorrhea, infections with Chlamydia trachomatis, syphilis, herpes simplex infections, genital warts, pubic lice, scabies); enteric diseases (infections with Shigella species, Campylobacter jejuni, Entamoeba histolytica, Giardia lamblia, hepatitis A, hepatitis B, hepatitis non-A, non-B, and cytomegalovirus); trauma (fecal incontinence, hemorrhoids, anal fissure, foreign bodies, rectosigmoid tears, allergic proctitis, penile edema, chemical sinusitis, inhaled nitrite burns, and sexual assault of the male patient); and the acquired immunodeficiency syndrome (AIDS). Clinicians can assist homosexual teenagers by understanding their special health needs, by counseling them about safe sexual practices, and by accepting their relationships nonjudgmentally.

The incidence of sexual assault continues to escalate, and it is under-reported. Victim care requires special attention in the emergency department because the history and physical examination are unique and frequently time consuming. The victim should receive counseling and treatment for potential STDs, HIV, and hepatitis exposure, while stressing the importance of follow-up care. The emergency department physician must fulfill obligations not only to the patient but also to law enforcement. To be done well, documentation of findings and the completion of the evidentiary portion of the exam require a familiarity with the process.

Adherence to local guidelines on the use of HIV post exposure prophylaxis (PEP) and hepatitis B vaccine following sexual assault was evaluated by means of audit. Forensic Medical Examiners (FMEs) were asked to complete an audit form after conducting sexual offence examinations at Gloucester Sexual Assault Referral Centre (SARC). Only one HIV PEP pack was prescribed during the six and a half month audit period. Examination of the SARC records of the allegations made by complainants did not reveal any high-risk cases involving a failure to offer HIV post-exposure prophylaxis following sexual exposure (PEPSE). The majority of the examinations performed at the SARC were carried out by trained sexual offence examiners (SOEs). The audit indicates that these SOEs were considering the appropriate use of HIV PEPSE and hepatitis B vaccine when they performed examinations. Some examinations were performed by general forensic medical examiners who completed the audit forms infrequently. It was not possible to determine whether these examiners were considering the appropriate use of HIV PEPSE and hepatitis treatments.

Sexual exposure to STDs including HIV and hepatitis is common. Sexual assault is also prevalent and should be screened for in a patient presenting for medical care after potential sexual exposure to STDs. Primary care providers should be familiar with current recommendations for STD prophylaxis and treatment after sexual exposure to STDs, and be aware that HIV postexposure prophylaxis is effective and available if indicated after sexual exposure to HIV. Providers should also be aware of the need for prompt referal for evaluation and medical care of the adult patient after a sexual assault.
