characterized by fever, lymphadenopathy, arthralgias, and hemolytic anemia. A 34-year-old woman, who had previously undergone surgical removal of her spleen, was hospitalized because of presumed
hepatitis
. The many retinal nerve fiber layer infarcts and serologic abnormalities suggested collagen disease. The diagnosis of babesiosis was established by the demonstration of intraerythrocytic parasites and a greatly elevated titer to Babesia microti. The infection was treated with chloroquine, orally administered quinine, and pyramethamine, and her symptoms resolved within one month. The retinopathy was probably the result of focal vasculitis secondary to immune complex disease caused by chronic infection.

A workshop for collaborative study on circulating Schistosoma japonicum antigen detection was held at the Hunan Institute of
Parasitic Diseases
, Yueyang, Hunan Province in May, 1993. Eight laboratories from schistosomiasis-endemic provinces (municipalities) were joined in this study. Scientists from these laboratories performed serological assays in blind using their own brought test systems that developed during recent years. The serum samples tested covered 100 non-endemic controls, 190 from patients with active schistosomiasis, 110 from cured cases of schistosome-infection, and 170 from individuals harbouring other parasitic infections or
hepatitis
. Results of the study revealed an assay specificity, in terms of false positive rate, of 2% in Series B versus 14-46% in the other 7 series. With regard to assay sensitivity, all test series showed 95-100% for acute cases. While for chronic cases, test series A and F demonstrated sensitivities at 73.3% and 72.7% respectively. Other series were all below 70%. Negative reversion rates for sera from cured cases within 1 year post-treatment were below 33.3%, those within 2 years were 60% for Series E, while below 50% for all others except the Series H which was excluded due to an extremely low sensitivity obtained. The low values of these test series for chemotherapy assessment may be attributed to the poor specificity of the detecting systems used and/or the sample donors being not totally cured. It is strongly indicated that all the 8 test systems involved in the present study should further be improved in sensitivity, specificity as well as in capacity for pre-and post-treatment evaluations.

This study was based on the immunohistochemical characterization of hepatic lesions caused by Elaeophora elaphi in 15 red deer. In eight animals filariae were found in branches of the portal vein. "Web-like" endophlebitis (15 cases), lymph nodule-like thrombi associated with filarial debris (12 cases) and fibrinous thrombi (three cases) were also observed in the portal vein. hepatitis
(six cases) and lymph nodule-like tissue (three cases) were found in the liver parenchyma. The distribution of CD3 antigen, IgG-lambda light chains, and S-100 protein in lymphoid tissue in the portal vein thrombi and hepatic parenchyma was similar to that found in the cortex of the lymph nodes. In these lesions, lymphoid follicles with large germinal centres (mainly composed of IgG+ B lymphocytes, with occasional S-100+ follicular dendritic cells) predominated over interfollicular lymphoid tissue (mainly composed of CD3+ T lymphocytes and S-100+ interdigitating cells). This suggested that the local humoral immune response was more important than the cellular response in this
parasitic disease
. The presence of interdigitating and follicular dendritic cells confirmed the high degree of organization of the abnormal lymphoid tissue and suggested that antigen presentation to T and B lymphocytes and the subsequent proliferation of the latter was particularly efficient in such tissue. The latter may therefore be superior to diffuse granulomatous infiltrate in terms of the production of a local immune response.

With the goal of reducing false-positive results in enzyme-linked immunosorbent assay (ELISA) serodiagnosis of hepatitis C virus in clinical practice, a study was undertaken to establish better cutoff values. We examined 277 serum samples from patients with
hepatitis
(non-A, non-B; B; autoimmune); subjects with antinuclear antibodies or rheumatoid factor, anticytomegalovirus or Epstein-Barr virus IgG or IgM antibodies, or
parasitic disease
(Chagas disease, leishmaniasis); and healthy volunteers. Concordant positive results in 2 different immunoblot assays in 250 samples were taken as indicative of true-positive, and when negative, of absence of infection. Reactivity in 3 ELISA tests were evaluated for the manufacturer recommended cutoff (CO) and for 2CO, 3CO, and 4CO; and corresponding sensitivity and specificity were calculated for single or combined pairs of ELISA tests. Although CO is adequate for blood bank screening, because it provides maximal sensitivity, the frequently observed false-positive results could be significantly reduced by increasing the cutoff value to 2CO, with no significant loss in sensitivity either in relation to pairs of immunoenzymatic tests or to a single ELISA.

Coastal dolphins are regarded as indicators of changes in coastal marine ecosystem health that could impact humans utilizing the marine environment for food or recreation. Necropsy and histology examinations were performed on 35 Indian Ocean bottlenose dolphins (Tursiops aduncus) and five Indo-Pacific humpback dolphins (Sousa plumbea) incidentally caught in shark nets off the KwaZulu-Natal coast, South Africa, between 2010 and 2012. Parasitic lesions included pneumonia (85%), abdominal and thoracic serositis (75%), gastroenteritis (70%),
hepatitis
(62%), and endometritis (42%). Parasitic species identified were Halocercus sp. (lung), Crassicauda sp. (skeletal muscle) and Xenobalanus globicipitis (skin). Additional findings included bronchiolar epithelial mineralisation (83%), splenic filamentous tags (45%), non-suppurative meningoencephalitis (39%), and myocardial fibrosis (26%). No immunohistochemically positive reaction was present in lesions suggestive of dolphin morbillivirus, Toxoplasma gondii and Brucella spp. The first confirmed cases of lobomycosis and sarcocystosis in South African dolphins were documented. Most lesions were mild, and all animals were considered to be in good nutritional condition, based on blubber thickness and muscle mass. Apparent temporal changes in
parasitic disease
prevalence may indicate a change in the host/parasite interface. This study provided valuable baseline information on conditions affecting coastal dolphin populations in South Africa and, to our knowledge, constitutes the first reported systematic health assessment in incidentally caught dolphins in the Southern Hemisphere. Further research on temporal disease trends as well as disease pathophysiology and anthropogenic factors affecting these populations is needed.
