Serum alpha-fetoprotein levels were measured by radioimmunoassay in 473 patients with biopsy-proved noneoplastic hepatic disorders; 22% had values greater than 40 ng/ml, whereas only 1 of 350 patients with nonhepatic benign diseases had a value greater than this. Levels exceeded 40 ng/ml in more than 30% of patients with various types of
hepatitis
, and in 0% to 15% with inactive postnecrotic cirrhosis, primary biliary cirrhosis, biliary tract obstruction, and alcoholic liver disease. Values greater than 500 mg/ml were observed solely in viral subacute hepatic necrois. Only one patient had a level exceeding 3,000 ng/ml, the concentration at which alpha-fetoprotein is detectable by agar-gel diffusion. Of 75 patients with
hepatoma
, serum alpha-fetoprotein levels exceeded 40 ng/ml in 69%, and exceeded 3,000 ng/ml in 48%. These studies indicate that serum alpha-fetoprotein levels are elevated in several nonneoplastic hepatic disorders when a sensitive assay is used; this phenomenon may reflect hepatic regeneration.

The sera from 89 patients from the Eastern Higlands of Papua New Guinea, all with histologically diagnosed liver disease, were tested for Hepatitis B Antigen (HB Ag) and Hepatitis B antibody (HB Ab) and alpha1 fetoprotein (AFP) by a variety of techniques which included radioimmunoassay. , cirrhosis and non specific liver disease, elevated levels of AFP were again frequently present, but at generally lower levels. It is conlcuded that HB Ag and AFP frequency and levels in liver disease are similar to those reported from other tropical countries. Further study is required to elicit the cellular immunological changes in liver disease.

There are two well-characterized antigen-antibody systems which relate specificially to viral hepatitis B. Tests for HBsAg and anti-HBs are readily available and of great benefit to the diagnosis, prevention and understanding of hepatitis B. Tests for HBcAg and anti-HBc are still research techniques which requires further development before they can be used at the level of everyday medical practice. HBsAg in an individual indicates that he harbors the virus of hepatitis B; it may be present in the absence of liver disease or be found in association with both acute and chronic type B
hepatitis
. The presence of HBsAg also suggests that HBV may be causally related to some cases of periarteritis nodosa, chronic glomerulonephritis, and
hepatoma
. Although HBV is readily transmitted in blood, the major portion of post-transfusion
hepatitis
now appears to be serologically unrelated to either the hepatitis B virus ("serum") or the hepatitis A virus ("infectious"); the etiology of these cases is currently undetermined. There is increasing evidence that HBV may be transmitted by modes other than blood, but the exact mechanisms of such transmission is not established. The combined transmission of HBV by blood and other routes has resulted in a large number of persistent carriers of HBsAg in the world. There is no current method to alter this carrier state. The
hepatitis
risk of such persistent carriers to their personal and professional contacts is under investigation.

, proliferation of pseudotubules, and hyperplastic nodules. Serum alpha-fetoprotein levels, monitored in one of the monkeys by radioimmunoassay, paralleled tumor growth and recurrence of the
hepatocellular carcinoma
. Normal serum alpha-fetoprotein levels were noted for a monkey with hemangioendothelial sarcoma. Our results implicate AFB1 as a liver carcinogen in monkeys and add additional support to the hypothesis that humans exposed to this substance may be at risk of developing liver cancer.

, 66 other malignancies, 74 cases of cirrhosis of the liver, 60 of chronic aggressive
hepatitis
, 12 of chronic persistent hepatitis, 16 of subacute
hepatitis
, 36 of acute viral hepatitis, and 13 healthy hepatitis B-surface antigen (HBsAg) carriers. Double immunodiffusion and radioimmunoassay (RIA) were used in all cases. AFP greater than 10 ng-ml appeared in 90% of the cases, and was above 400 ng/ml in 69%. In 80% of those above 400 ng/ml, AFP could also be demonstrated by immunodiffusion. The AFP level in
hepatocellular carcinoma
was discovered to decline as the age increased. It also appeared to be related to the tumor cell type; the relatively immature cell type was more frequently associated with a higher AFP level. The presence of HBsAg did not influence the AFP level. Although the AFP in other malignancies and liver diseases ranged abnormally from 14 to 69%, the level did not exceed 400 ng/ml as in our cases of
hepatocellular carcinoma
(except in one case). Thus, this figure provides a diagnostic serum level of AFP for the identification of

Alpha1-fetoprotein (AFP) is an alpha1-glycoprotein which can be found in high concentration during fetal development in many mammals, birds, sharks and, also, man. The alpha-fetoproteins of various species have similar physico-chemical properties and often common antigenic determinants. Differences of microheterogeneity depend on a different content of sialin-acid. During human fetal development the serum AFP concentration falls with increasing gestational age. 4-5 weeks after birth AFP can be detected usually in low serum concentrations. Using more sensitive immunulogic techniques e.g. radioimmunoassay there was shown that AFP is present in sera of normal adults in concentrations of 10-20 ng/ml. AFP serum concentrations rise physiologically during pregnancy up to 500-550 ng/ml. During fetal development liver, yolk sac and gastrointestinal tract are the major sites of synthesis. In primary
liver cell carcinoma
, hepatoblastoma and in teratoblastoma containing yolk sac tissue AFP synthesis rises in tumor cells; the AFP serum concentration increases above 2 microgram/ml. In patients with benign liver diseases e.g. virus
