Travelers to developing countries participated in a follow-up study of the health risks associated with short (less than three months) visits to these nations. Travelers to the Greek or Canary Islands served as a control cohort. Participants completed a questionnaire to elicit information regarding pretravel vaccinations, malaria prophylaxis, and health problems during and after their journey. Relevant infections were confirmed by the respondent's personal physician. The questionnaire was completed by 10,524 travelers; the answer rate was 73.8%. After a visit to developing countries, 15% of the travelers reported health problems, 8% consulted a doctor, and 3% were unable to work for an average of 15 days. The incidence of infection per month abroad was as follows: giardiasis, 7/1,000; amebiasis, 4/1,000; hepatitis, 4/1,000; gonorrhea, 3/1,000; and malaria, helminthiases, or syphilis, less than 1/1,000. There were no cases of typhoid fever or cholera.

The case of a 25-year-old patient is reported who suffered from a syndrome similar to immune complex disease following cholera revaccination. The clinical picture included fever, muscle, joint and abdominal pain, vomiting, serositis, hepatitis, suspected myocarditis, anaemia and thrombocytopenia. Clinical symptoms subsided spontaneously within two weeks. This case illustrates a hazard of cholera vaccination so far not reported in the literature.

Epidemic emergencies have shown increasing trend in India and most parts of the country appear to be vulnerable to these emergencies. In this paper we present a profile of epidemic emergencies attended by the National Institute of Communicable Diseases in the last five years, to delineate aspects that will promote better preparedness and management. Water borne and water related disease epidemics constituted more than 70% of the epidemic emergencies in India. Non 01 cholera epidemics constituted one fourth of total cholera epidemics during 1991-95. Most of the hepatitis outbreaks were attributed to Non A Non B. The source of infection in majority of the cholera and jaundice epidemics was contaminated water. Dengue and resistant typhoid fever were among other emergencies reported during last five years. Some of these epidemic were reported to local health authorities as mysterious diseases due to lack of public health laboratory facilities. Encephalitis and encephalitis like epidemics in the form of Liquor poisoning and chronic Heat syndrome encephalopathy were also observed. The re-emerging disease epidemics like plague in Beed, Pneumonic plague in Surat and malaria in Rajasthan were also investigated during 1994. These observations indicate the weakness in the epidemiological and laboratory surveillance besides inadequacy in water management practices and other socio environmental reasons.

For over a decade we have maintained within a district of 5 million people, a system of prompt reporting of cases of childhood vaccine-preventable diseases, encephalitis, meningitis, hepatitis, and rabies; together with a sentinel laboratory surveillance of cholera, typhoid fever, malaria, HIV infection and antimicrobial-resistance patterns of selected pathogens. The system combined government and private sectors, with every hospital enrolled and participating. Reports were scanned daily on a computer for any clustering of cases. Interventions included investigations, immunisation, antimicrobial treatment, health education, and physical rehabilitation of children with paralysis. All vaccine-preventable diseases have declined markedly, whilst malaria and HIV infections have increased steadily. Annual expense was less than one US cent per head. The reasons for the success and sustainability of this model include simplicity or reporting procedure, low budget, private-sector participation, personal rapport with people in the network, regular feedback of information through a monthly bulletin, and the visible interventions consequent upon reporting. This district-level disease surveillance model is replicable in developing countries for evaluating polio eradication efforts, monitoring immunisation programmes, detecting outbreaks of old or new diseases, and for evaluating control measures.

This study was designed to review all grossly detectable abnormalities and conditions (GDACs) encountered in poultry in Canadian abattoirs to determine which have potential to cause adverse health effects for the consumer. Review of the literature and consultation with scientists in the field of microbiology, epidemiology, poultry pathology, chemistry, and meat inspection served to generate an inventory of GDACs, and a decision tree containing algorithms was developed to identify GDACs potentially representing a health hazard to consumers. Through the use of the decision tree, GDACs were classified into different categories with regard to the risk they represent to humans. A number of GDACs were identified as being of potential concern from a food safety perspective, namely Erysipelas, fowl cholera, Campylobacteriosis, clostridial diseases, hepatitis/enteritis associated with Helicobacter, Listeriosis, Salmonella infections (nontyphoid infections, Salmonella arizonae, pullorum disease, and fowl typhoid), Staphylococcosis, and Toxoplasmosis. Further characterization--i.e., hazard characterization, exposure assessment, and risk characterization--is required to quantify or better characterize the probability that products derived from affected carcasses may affect the consumer as well as the resulting consequences. Risk assessment is a dynamic process. Results presented in this paper are based on available information and expert opinion. As new information is obtained, the inventory of GDACs and their classification may be modified.

In 1987, the population estimate was 10.5 million, 60% of which live in rural areas; the annual growth rate was 7%. Infant mortality rate is 92/1000. Life expectancy stands at 54 years. In health related matters, visitors must have vaccination certificates against yellow fever and cholera, and vaccinations are recommended against tetanus, typhoid, paratyphoid, polio, and hepatitis.

The rural population in India is exposed to working and living conditions: drinking supply, sanitation, fuel wood shortages, maternal mortality, alcoholism among males, pesticide use, environmental degradation, migrant workers, sickness and injury compensation in natural resource based industry, and mechanization in the workplace. Good health is dependent on a supportive home environment which physically provides protection, has access to safe potable drinking water and sanitary facilities, and reinforces health habits and behavior. One of the greatest health hazards is the lack of safe drinking water. The result is increases in water-related diseases such as dysentery, cholera, diarrhea, and hepatitis among men, women, children, and fetuses. Today only 30% of the total population has access to sufficient, safe drinking water. Personal hygiene is also affected by inadequate supplies. Another hazard is waste disposal, which if improperly managed, results in hookworm and ascarias infestations. Barefoot people are particularly affected. In 1982, 8790 villages were found to be without latrines, or with only bucket latrines. The firewood fuel shortages impact directly on women through food habit changes and excessive labor in acquiring adequate supplies. Women are also affected by high rates of anemia which are a by-product of environmental and social conditions. There are a number of psychosocial conditions that impact on the health of women. In Himachal Pradesh women complain that their husbands drink too much alcohol, which increases acts of domestic violence. Male migration for work places women in stressful work conditions managing the land and child care, and exposing women to sexually transmitted diseases. The workplace also had hazards. Agricultural workers have little bargaining power and few organizations representing their interests. A brief description is given of conditions among plantation workers in Assam and Darjeeling. There are hazards due to unskilled workers operating machinery, exposure to harmful pesticides, and the lack of benefits for sickness or injury. Mining hazards are characterized. Awareness of conditions will hopefully lead to efforts to establish preventive health.

Viral hepatitis is caused by a number of unrelated hepatotrophic viruses, known and unknown. Five hepatitis viruses namely HAV, HBV, HCV, HDV and HEV have been well characterized and the epidemiology and disease pattern of each agent has been defined. In the West, HAV, HBV and HCV are major causes of viral hepatitis. In the East, HEV is the most common cause of viral hepatitis. HAV is ubiquitous in childhood in such countries and accounts for less than 4% of disease in adults. Viral hepatitis becomes a problem to an international traveller when he envisages a journey from low endemic to high endemic area and is susceptible to the infection endemic at his destination. Millions of such potentially susceptible travellers from Europe, the USA, Canada, Japan, Australia, and New Zealand visit endemic areas every year for various reasons. Viral hepatitis is the most common reported immunization-preventable disease among travellers to developing countries. Imported viral hepatitis incapacitates the incumbents for an average of 4-10 weeks. Considering the magnitude of the travel, the number of cases of viral hepatitis and case fatality of around 2%, the disease causes significant morbidity and mortality in such communities. It has been estimated that viral hepatitis occurs 100 times more frequently than typhoid fever and 1,000 times more often than cholera in travellers to developing countries. Hepatitis A is the most common form of viral hepatitis in travellers and cumulative data have shown a risk of 3-6 cases/1,000 persons/month of stay whereas the risk of acquiring hepatitis B is 10 times lower.

Emerging water-borne pathogens constitute a major health hazard in both developed and developing nations. A new dimension to the global epidemiology of cholera-an ancient scourge-was provided by the emergence of Vibrio cholerae O139. Also, water-borne enterohaemorrhagic Escherichia coli ( E. coli O157:H7), although regarded as a problem of the industrialized west, has recently caused outbreaks in Africa. Outbreaks of chlorine-resistant Cryptosporidium have motivated water authorities to reassess the adequacy of current water-quality regulations. Of late, a host of other organisms, such as hepatitis viruses (including hepatitis E virus), Campylobacter jejuni, microsporidia, cyclospora, Yersinia enterocolitica, calciviruses and environmental bacteria like Mycobacterium spp, aeromonads, Legionella pneumophila and multidrug-resistant Pseudomonas aeruginosa have been associated with water-borne illnesses. This review critically examines the potential of these as emerging water-borne pathogens. It also examines the possible reasons, such as an increase in the number of immunocompromised individuals, urbanization and horizontal gene transfer, that may underlie their emergence. Further, measures required to face the challenge posed by these pathogens are also discussed.

Cholera, hepatitis and typhoid are well-recognized water-borne illnesses that take the lives of many every year in areas of uncontrollable flood, but far less attention is afforded to the allegedly safe potable water in affluent nations and the presumed healthful quality of water in communities and hospitals. Recent literature, however, points to increasing awareness of serious clinical sequelae particularly experienced by immunocompromised patients at high risk for disease and death from exposure to water-borne microbes in hospitals. This review reflects the literature indicting hospital water as an important source for nosocomial infections, examines patient populations at greatest risk, uncovers examples of failures in remedial water treatment methods and the reasons for them, and introduces point-of-use water filtration as a practical alternative or complementary component of an infection control strategy that may reduce the risk of nosocomial infections.
