To the best of our knowledge this is the reported first case of the successive occurrence in the same patient of chronic active hepatitis and acquired pure red cell aplasia, both probably of autoimmune origin. The diagnosis of autoimmune
hepatitis
was based on the presence of characteristic lesions at the examination of the liver biopsy specimen, high titer of anti-smooth muscle antibodies in the serum, and remission obtained by steroid therapy. Erythroid aplasia, which appeared during the course of this treatment, was revealed by a regenerative
anemia
(4.4 g Hb/100 ml) and proved by bone marrow aspiration and biopsy. In vitro bone marrow culture was normal, suggesting the in vivo presence of an inhibitor of erythroblastic differentiation. Red cell aplasia was cured by cyclophosphamide (100 mg/day during 56 days). No recurrence was noted until the death of the patient, which occurred one year later, due to hepatic cholangiocarcinoma. Action of cyclophosphamide on the pure red cell aplasia suggested the immune origin of this disease. The liver and the bone marrow erythroid lineage have probably been the successive targets of immunologic dyscrasia.

Hairy cell leukemia is a chronic lymphoproliferative disorder characterized clinically by splenomegaly and cytopenias. Spontaneous remissions are rare and splenectomy is often performed when the blood counts worsen and cause symptoms. Three of our patients with hairy cell leukemia developed recurrent pancytopenia and transfusion-dependent
anemia
after splenectomy. Each subsequently acquired transfusion
hepatitis
and in two patients marked hematologic improvement was noted within 2 months. Complete peripheral blood remission occurred within 17 months in all patients although bone marrow infiltration with hairy cells persisted. One patient remains in remission for 12 years; the other two succumbed to infectious illnesses but with normal blood counts. The mechanism by which
hepatitis
virus induces hematologic recovery in patients with hairy cell leukemia is unknown but may involve augmentation of the interferon system.

Sarcocystosis was studied in 37 sheep after oral inoculation with 10(4)-5 x 10(7) sporocysts of Sarcocystis tenella from canine feces. Two sheep inoculated with 2.5 x 10(7) and 5 x 10(7) sporocysts became moribund 16 and 19 days post-inoculation (DPI), respectively, due to occlusion of arteries of gut and mesentery by first generation meronts. Sheep inoculated with 10(7) sporocysts remained clinically normal until 21 DPI and those inoculated with 10(5)-10(6) became ill 24-28 DPI due to
anemia
coincident with maturation of second generation meronts. Inflammation,
hepatitis
and myocarditis were the main lesions of acute and subacute ovine sarcocystosis. Inflammation began to subside by the time (75 DPI) sarcocysts matured. Sarcocystis-induced encephalitis was distinguished from naturally occurring myelomalacia in sheep caused by an unidentified sporozoan.

Five calves inoculated orally with 10(5)-10(6) sporocysts of Sarcocystis hominis from human feces were necropsied 10, 18, 24, 111, and 222 days postinoculation (DPI). Calves became febrile (greater than 40-41 C) between 10 and 24 DPI and developed mild

(packed cell volumes were reduced by 40% of initial values) between 29 and 57 DPI but otherwise remained clinically normal. Focal
hepatitis
, mesenteric lymphadenitis, and myocarditis were seen in calves at 10, 18, and 24 DPI. No stages of the parasite were found at any of these times except for a few merozoites in macrophages associated with myocardial lesions in the calf necropsied 24 DPI. Mature sarcocysts at 111 and 222 DPI were up to 950 microm long and their walls were up to 6 microm thick. They were found only in skeletal muscles. One immature sarcocyst was seen in the myocardium of the calf at 222 DPI.

Fifty-nine children treated with acute lymphoblastic leukemia (ALL) were evaluated for abnormalities of liver function in order to monitor chemotherapy-induced hepatotoxicity. Twenty-one patients (36%) had elevations of alanine aminotransferase (ALT) in a pattern consistent with either drug-induced hepatocellular injury or non-A, non-B (NANB)
hepatitis
. These 21 patients (Group 1) were compared with the other 38 children (Group 2) with regard to a number of clinical and laboratory characteristics. Patients in Group 1 were older (P = 0.002) and had lower platelet counts (P less than 0.001) and hemoglobin values (P = 0.075) at diagnosis than Group 2 patients. The median number of units of blood products transfused was significantly greater in Group 1 patients (9.0 versus 1.0 units, P less than 0.001). The two groups were similar with regard to chemotherapy regimens. Children with ALL who present at an older age and who have more marked
anemia
and thrombocytopenia require more blood transfusions and are more likely to develop elevated ALT values in a pattern consistent with acute or chronic NANB

. These findings suggest a predominant role of NANB posttransfusion hepatitis--rather than or in addition to chemotherapy-induced hepatic injury--as a cause for elevated ALT values in children with ALL. In view of the potentially serious consequences of NANB

Aflatoxins are hepatotoxic in many species including dogs. In two separate outbreaks, the primary signalment was high morbidity and mortality in hunting dogs presenting with clinical signs of icterus, anorexia and listlessness. Preliminary laboratory examinations revealed toxic

. In the first case, a feed sample was not available and the diagnosis was established by confirming the presence of significant levels of aflatoxin B1 in tissues. Although 50% of Indochinese refugees are under 18 years of age, previous studies have emphasized the prevalence of parasites,
anemia
, tuberculosis, and
hepatitis
, with few addressing age-related health care needs. In this study the specific health care needs of 80 Indochinese refugee teenagers, evaluated during a 4-year period, were determined. The Centers for Disease Control's suggested screening measures were used, and it was found that 52% had positive purified protein derivative skin tests, 38% lacked immunizations, 35% had stool specimens positive for parasites (prevalence and number of parasites greatest among Cambodians), 14% had blood tests positive for hepatitis B surface antigen, and 10% were anemic. Additional evaluations showed that 19% had hemoglobinopathies, 14% were in or below the fifth percentile for height and weight, 12% had goiters, 12% had skin disorders, 8% had positive hepatitis B surface antigen, 5% had visual defects, 5% had hearing loss, 5% had psychosomatic illness, and 4% had idiopathic scoliosis. Although suggested Centers for Disease Control screening measures may be adequate for younger Indochinese children, these data suggest that additional studies are necessary for teenagers. For the sexually active adolescent, identification of and counseling for
hepatitis
antigenemia and hemoglobinopathies are crucial. In addition, early identification of emotional and physical problems during screening may enhance assimilation into a new society and facilitate completion of the psychosocial tasks of adolescence.
