Intrapulmonary teratoma
(
IPT
) is exceedingly rare, hence preoperative diagnose is usually inaccurate. A 21-year-old man was presented with a 5-year history of recurrent
hemoptysis
and left upper lung mass with heterogeneous opacity on chest x-ray. Diagnostic fibrobronchoscopy detected tuft of brownish hairs in the left upper anterior segmental bronchus, achieving the diagnosis of
IPT
preoperatively, which was further confirmed by lobectomy. The case report is noteworthy with fibrobronchoscopic finding of endobronchial hairs in a heterogeneous lesion, which underscores the important role of fibrobronchoscopy in the differential diagnoses of chronic cavitary lesions such as
IPT
without trichoptysis.

Benign cystic teratoma of lung is an extremely rare tumour, which was first described in literature by Mohr in 1839.
Intrapulmonary teratoma
is thought to be a derivative of the third endo-dermal pharyngeal pouch, which is an anlage of the thymus. The authors present a rare case of mature cystic teratoma in a young male involving the right upper lobe of the lung. Diagnosis is often missed and patients are treated for various infectious conditions. Treatment is complete resection for both benign and malignant teratomas and carries excellent prognosis. Benign cystic teratoma, if not excised may cause grave complications like life-threatening
haemoptysis
or malignant transformation with metastatic disease.

Intrapulmonary teratoma
is a rare and little-known disease. The present case was that of an adolescent with recurrent cough and
haemoptysis
. Chest radiography revealed a mass on the left lung base, and computed tomography (CT) indicated a heterogeneous tumour with a fluid component, soft parts, fat, and calcifications in the left upper lobe. Upper left lobectomy was performed; histopathological findings confirmed a mature pulmonary teratoma.

Intrapulmonary teratoma
(
IPT
) is a rare mediastinal teratoma.
IPT
arise because of abnormal migration of pluripotent stem cells along the developing lung bud. Patients present with chest pain,
hemoptysis
, and trichoptysis. These tumors have the potential to rupture into adjacent pleura, pericardium, or tracheobronchial tree. The differential diagnosis includes hydatid cyst, fungal ball, lung abscess, and lung parenchymal tumor. We report a case of
IPT
, which was initially diagnosed as hydatid cyst of the lung due to positive hydatid serology and correlating radiological features. The role of cross-reaction of hydatid antigens with sera of cancer patients needs to be evaluated. Surgical resection of the tumor is advised to avoid complications.
