Tracheal diverticulum is a rarely encountered entity, usually discovered incidentally as an outpouching at the right side on radiography or computed tomography (CT). We present the case of a male patient with a tracheal diverticulum detected during a work-up for hemoptysis. His thoracic CT scan showed a sac formation on the right dorsolateral wall of the trachea. Flexible bronchoscopy showed a collapsed entrance to the diverticulum, which could be opened by flexible forceps. The mucosal lining of the diverticulum seemed normal, without retained secretions or blood. Although with limited clinical consequences, tracheal diverticulum must be kept in mind as a reservoir for secretions that may cause infection, and also hemoptysis if infected.

Tracheal diverticulum is a rare entity with only 79 symptomatic cases reported till date. We herein describe the case of a 38 year old male who presented with recurrent dry cough and hemoptysis and was diagnosed to have tracheal diverticulum on High Resolution Computed Tomography (HRCT) of chest. This was further confirmed on fibre optic bronchoscopy.

Tracheal diverticulum (DV) is a type of paratracheal air cyst (PTAC) that is often asymptomatic and usually detected incidentally by imaging methods. Tracheal DV are divided into two subgroups: congenital and acquired. Dysphagia, odynophagia, neck pain, hoarseness, hemoptysis, choking, and recurrent episodes of hiccups and burping can also be seen in symptomatic patients. Thin-section multidetector computed tomography (MDCT) is useful for diagnosis of tracheal diverticulum. The relationship between DV and tracheal lumen can be demonstrated by axial, coronal, and sagittal reformat multiplanar images. Bronchoscopy can also be used in diagnosis for tracheal DV. However, the connection between DV and tracheal lumen can not be shown easily with bronchoscopy. Conservative treatment is the preferred treatment in asymptomatic patients. Surgical or conservative treatment can be performed for symptomatic patients, depending on patient age and physical condition.

Tracheal diverticulum is defined as an air cyst located on the lateral wall, congenital or acquired. Most of them are asymptomatic, incidentally found on CT. The common symptoms are chronic cough, stridor, or recurrent respiratory infections. Asymptomatic diverticulum requires no treatment and managed conservatively while surgical excision is indicated in cases of local complications or symptom permanence. We report a case of tracheal diverticulum presented with haemoptysis, a rare symptom. The diagnosis was made with a CT that shows a 15 mm air image on the right lateral trachea wall. Due to symptoms' persistence, we decide to perform surgery with a good outcome.
