A 65-year-old man complaining of a left medial ocular angle mass and
hemoptysis
was admitted to our hospital. Chest radiography revealed a right hilar mass and bronchoscopy revealed widespread cancerous invasion of the carina, both main bronchi, right upper lobe bronchus, truncus intermedius, and middle lobe bronchus. Histological examination of a biopsied specimen revealed small cell carcinoma. Brain CT scan, abdominal CT scan, and bone scintigram showed solitary brain, multiple liver, and multiple bone metastases. Partial response was obtained with three courses of combined chemotherapy with carboplatin and etoposide, and the ocular tumor until it was almost invisible to the unaided eye. However, complete response could not be obtained with additional two courses of combined chemotherapy, and the patient was discharged. Seven months after the first admission, he was readmitted with dyspnea and an enlarged (6 mm in diameter) left ocular mass. Histological examination of the resected mass revealed a small cell carcinoma that had metastasized to the conjunctival substantia propria. He died of respiratory failure one month after readmission. is uncommon and patients such as this are extremely rare. Although some cases of uveal or orbital metastasis from
lung cancer
have been reported, we can find no other report of conjunctival metastasis from
lung cancer

Hamartoma is one of the most common benign tumors in the lung, and most of the lung hamartomas are located in the parenchyma. This report reviews 24 cases with this kind of tumor. The male-female ratio was 1.4:1 and the mean age 46.4 years. Sixteen patients of the 24 were asymptomatic, while in the remainder cases chest pain or distress, cough, short breath or
hemoptysis
were presented. Lung hamartoma is usually composed of cartilage, fat, smooth muscle and respiratory epithelium. X-ray findings of the parenchymal type in these cases were characterized by a round or oval sharply outlined mass, but only one with a mass in the shape of a dumbbell. 33% cases showed calcification. In 19 cases the mass was < 3.0 cm diameter (79%) and in 17 it was located in the superficial edge of the lung. The diagnosis was confirmed in only 6 cases preoperatively. The other cases were misdiagnosed as
lung cancer
, tuberculoma, metastatic tumors of the lung, etc. Enucleation or wedge resection is desirable for this kind of tumor.

The EORTC Study Group on Quality of Life has developed a modular system for assessing the quality of life of cancer patients in clinical trials composed of two basic elements: (1) a core quality of life questionnaire, the EORTC QLQ-C30, covering general aspects of health-related quality of life, and (2) additional disease- or treatment-specific questionnaire modules. Two international field studies were carried out to evaluate the practicality, reliability and validity of the core questionnaire, supplemented by a 13-item
lung cancer
-specific questionnaire module, the EORTC QLQ-LC13. In this paper, the results of an evaluation of the QLQ-LC13 are reported. The

questionnaire module comprises both multi-item and single-item measures of
lung cancer
-associated symptoms (i.e. coughing,
haemoptysis
, dyspnoea and pain) and side-effects from conventional chemo- and radiotherapy (i.e. hair loss, neuropathy, sore mouth and dysphagia). It was administered to patients with non-resectable
lung cancer
recruited from 17 countries. In total, 883 and 735 patients, respectively, completed the questionnaire prior to and once during treatment. The symptom measures discriminated clearly between patients differing in performance status. All item scores changed significantly in the expected direction (i.e.
lung cancer
symptoms decreased and treatment toxicities increased) during treatment. With one exception (problems with a sore mouth), the change of toxicity measures over time was related specifically to either chemo- or radiotherapy. However, the single item on neuropathy did not measure adequately the full range of symptoms. The hypothesised scale structure of the questionnaire was partially supported by the data. The multi-item dyspnoea scale met the minimal standards for reliability (Cronbach alpha coefficient > 0.70), while the pain items did not form a scale with reliability estimates acceptable for group comparisons. In conclusion, the results form international field testing lend support to the EORTC QLQ-LC13 as a clinically valid and useful tool for assessing disease- and treatment-specific symptoms in
lung cancer
patients participating in clinical trials, when combined with the EORTC core quality of life questionnaire. In a few areas, however, the questionnaire module could benefit from further refinements. In addition, its performance over a longer period of time still needs to be investigated.

The precise roles of fiberoptic bronchoscopy (FOB) and computed tomography (CT) of the chest in the evaluation of patients presenting with
hemoptysis
have not been clearly defined. On the assumption that both procedures would likely provide unique and complementary information, a prospective study with blinded interpreters using a modified high-resolution CT technique (HRCT) and FOB was designed to evaluate 57 consecutive patients admitted to Bellevue Hospital with
hemoptysis
. Etiologies included bronchiectasis (25 percent), tuberculosis (16 percent),
lung cancer
(12 percent), aspergilloma (12 percent), and bronchitis (5 percent): in an additional 5 percent of cases,
hemoptysis
proved to be due miscellaneous causes, while in 19 percent
hemoptysis
proved to be cryptogenic. Patients with
lung cancer
all were at least 50 years old, smoked an average of 78 pack-years, and had less severe
hemoptysis
but of longer duration. All had conditions diagnosed both by HRCT and FOB. High-resolution CT proved of particular value in diagnosing bronchiectasis and aspergillomas, while FOB was diagnostic of bronchitis and mucosal lesions such as Kaposi's sarcoma. Fiberoptic bronchoscopy localized bleeding in only 51 percent of cases. The high sensitivity of CT in identifying both the intraluminal and extraluminal extent of central lung cancers in conjunction with its value in diagnosing bronchiectasis suggest that CT should be obtained prior to bronchoscopy in all patients presenting with
hemoptysis

Still today bronchial artery embolization (BAE) is an important procedure in the management of
hemoptysis
, as an alternative to, or in association with, surgical or medical therapy. From 1974 to 1990, BAE was performed in 209 patients who suffered from
hemoptysis
with different etiopathogeneses and severity. Bronchiectasis were the most frequent indication (46%) followed by tuberculosis (31%), cystic fibrosis (16%),
lung cancer
(4%), and aspergillosis (3%). BAEs were performed by means of polyvinyl alcohol particles and absorbable gelatin sponge. In a few cases complete occlusion of the main truncus of particularly large bronchial arteries was obtained by use of Gianturco steel coin (5 mm o) technique. In the last seven years, together with the traditional angiographic techniques, selective digital angiography (SDA) was performed, above all in preliminary control phases, to evaluate occlusion during embolization. SDA reduced catheterization time and the mean quantity of contrast administered, decreasing side effects; 98% of
hemoptysis
was controlled in the first twenty-four hours, the other 2% in the following forty eight hours; 16% relapses occurred within the first year; 27 patients needed reembolization (15 patients twice, 11 patients three times, 1 patient five times). The goals of diagnostic testing in patients with suspected

are to establish the diagnosis and to determine the stage of the disease so that appropriate therapy can be initiated. Unless a patient has

, fever, or a change in cough as an initial manifestation, resectable

will seldom be diagnosed on the basis of the history. Screening tests--particularly chest roentgenography--have usually identified the abnormality. The managing physician should then select diagnostic procedures that are associated with low risk and that will provide further diagnostic and staging information. A biopsy will almost always be necessary before definitive therapy can be planned. In many cases, a single procedure--for example, a needle biopsy of a hepatic lesion or biopsy of a supraclavicular lymph node--will provide a definitive diagnosis and establish the stage of the disease.
