In this paper, 20 cases of
teratoma
of the lung (3 cases were presented and 17 reviewed) were analyzed. There were 13 males and 7 females, the age ranging from 16-47 years. The primary symptoms were: cough (19 cases, 95%), blood sputum or
hemoptysis
(16 cases, 80%), chest pain (12 cases, 60%), fever (7 cases, 35%) and expectoration of hair (4 cases, 20%) etc. The histogenesis, diagnosis and treatment of the disease were discussed.

Hemoptysis
as a presenting symptom in benign intrathoracic
teratoma
is rare, but we have documented one such case and discuss the various possibilities of why such a problem should arise. The bleeding was secondary to systemic-pulmonary artery shunting, caused at least in part by extensive pleural adhesions. The exact cause of the adhesions was not determined but possibilities include previous infection or trauma. The tumor was successfully removed surgically.

A 32-year-old woman who complained of cough and
hemoptysis
was found to have a metastatic trophoblastic tumor in her lungs and in a supraclavicular lymph node. A regressed primary
teratoma
of the ovary was considered the most likely origin of the tumor. Sputum cytology showed malignant cells, many of which were in elongated fiberlike forms and some of which had orange-staining cytoplasm. These cytologic appearances of choriocarcinoma are described and compared with the brief descriptions available in the literature. The possibility of misdiagnosis as squamous carcinoma is emphasized. The correct diagnosis is of particular importance because choriocarcinoma of gestational origin, which is similar cytologically to that of teratomatous origin, responds well to chemotherapy.

A 24-year-old man admitted to our hospital because of
hemoptysis
and left precordial pain. Chest X-ray and chest CT scan showed a mass in the left anterosuperior mediastinum. Laboratory examination revealed elevated levels of AFP (550 ng/ml), which were highly indicative of malignant germ cell tumor. Imaging examination indicated that the mass was resectable, so surgery was performed prior to chemotherapy. Histological examination revealed an admixture of seminoma, immature

, embryonal carcinoma and yolk sac tumor. Because the resection performed was incomplete, the patient received three courses of combination chemotherapy consisting of CDDP, VP-16 and BLM and 40 Gy irradiation. The serum level of AFP was normal after one course of combination chemotherapy. The patient has shown no sign of recurrence for 3 years since surgery.

are described. A specific symptom of trichoptysis occurred in two patients. The other presented with only recurrent

. All were treated by lobectomy of the affected lung. Pathologic examination in each specimen showed that the tumor had a cystic portion which was connected to the bronchial system and contained sebaceous material and pieces of hair. The solid portion consisted of tissues representing all three germ cell layers. The pathologic findings correlated well with the clinical symptoms and radiologic findings in the patients.
