Hemoptysis
and small local bleeding are of no clinical significance. Tumor cell spread and air embolism may occur but extremely rarely. Indications, contraindications equipment, and biopsy technique are treated. A recent technical improvement in needle biopsy is described.

Results of transthoracic needle biopsy have been evaluated on the basis of fourteen years experience comprising 5300 procedures on 2726 patients. In 90.7% of the cases a diagnosis was established. Of these, 46.4% showed cytological evidence of primary or secondary malignancy. In 2.4% false positives and in 3% false negatives were noted. The most important complication was found to be
pneumothorax
, which was noted in 27.2%, but these cases did not as a rule call for treatment.
Hemoptysis
was observed in 2-5%. There was minor local bleeding around the lesion in 11% of the patients but this only required observation. In one case out of 1264 malignancies an implantation metastasis was found. There was a single case of air embolism with spontaneous recovery, but no mortality in this series. In our experience, needle biopsy represents a minor, inexpensive and safe procedure, which--with a simple technique--permits a direct approach to all kinds of localized lung lesion with a high degree of accuracy.

Pulmonary infarction is a very uncommon cause of
pneumothorax
. The authors report two patients with
pneumothorax
arising as a complication of pulmonary infarction. One was a 72-year-old man who had
hemoptysis
, pleural effusion, and alveolar condensation. Four days later he developed a hydropneumothorax and pulmonary cavitation. He died of heart failure. The pulmonary infarction was not septic in this case. The other patient was a 12-year-old boy who suffered a septic embolism with cavitation as a result of an infected wound. He later developed a tension pneumothorax and died in a state of shock. The authors have found only 16 cases of
pneumothorax
as a complication of pulmonary infarction in the literature. It is surprising that, even though all infarctions are in contact with the pleural surface, the incidence of
pneumothorax
is not higher. The infarctions may or may not be septic. Cavitation is not necessarily present, though infarctions are usually cavitated before
pneumothorax
develops. Tension pneumothorax occurs in some cases.

Fluoroscopically guided percutaneous needle aspiration of focal pulmonary lesions was performed in 108 presumed infectious episodes in 82 immunocompromised patients in whom prior diagnostic studies, including transtracheal aspiration, were negative to inconclusive. Two-thirds of the lesions were 4 cm or smaller. Single (61/79) or multiple (18/79) organisms were recovered, for a diagnostic yield of 73% (79/108). Complications were

(26%), half of which required tube drainage, and limited
hemoptysis
(3%). This technique can be quickly performed and frequently repeated with existing personnel and equipment in institutions currently employing it for suspected pulmonary neoplasm, and is a productive study in the evaluation of "opportunistic pneumonia".

Between 1976 and 1978 percutaneous needle aspiration biopsies of 120 pulmonary and pleural lesions were performed. Cytologic examination of malignant and benign lesions was correct in 64% of the cases, false negative results were obtained in 19%. Complications included:

Transbronchial lung biopsy using the fibreoptic bronchoscope was carried out in 85 patients. There were no serious complications; two patients had a 10%
pneumothorax
and 17 had slight
haemoptysis
lasting less than 24 hours. The problems of interpreting small biopsy specimens are considered. Satisfactory specimens were obtained without fluoroscopic guidance, particularly in diffuse and lobar lesions. A histological diagnosis was made in 62% of diffuse lesions and compatible histology was found in a further 22%. In a further case Pneumocytis carinii infection was diagnosed. Blind biopsy of discrete periheral lesions was less successful with only one positive diagnosis in 12 patients.

The authors report on 5 cases of diffuse pulmonary lymphangiomyomatosis. This disease occurs in women during the period of genital activity and usually presents with pulmonary signs (9/10) : dyspnoea,

. It is also found in association with renal angiomyolipomas and uterine fibromyolipomas. X-ray films of the lungs show : an interstitial syndrome, gas or fluid pleural effusions, and thoracic distention. None of these signs is constant, but the association of an interstitial syndrome with thoracic distention in a young woman appears to be specific for this affection.

The authors report their experience of 100 transbronchial pulmonary biopsies done during fibroscopy. The technique is easy; patients with respiratory insufficiency or bullous lesions are excluded. Biopsy is done blindly, without any brillance amplifier. A hundred biopsies were performed (34 in limited opacities, 66 in diffuse images). Pulmonary parenchyma was brought out in 77% of cases and a histological diagnosis was made in 52% of cases. Two

and 5 minor
hemoptysis
occured as sequellae of these biopsies. This method of study is interesting mainly for alveolar or interstitial diffuse pneumopathies. In such cases, it represents a real improvement over the bronchial biopsy, often avoiding the need for a surgical biopsy.
