Three patients were admitted to Hat Yai Hospital, Songkhla Thailand with hemoptysis. They were previously healthy children aged 6, 13, and 14 years old who had attended schools in which outbreaks of influenza had occurred. They all had a history of fever, rhinorrhea, and severe cough accompanied by hemoptysis. Two developed hemoptysis on Day 3 and the third on Day 6 of illness, with one of them displaying massive hemoptysis. Chest radiographs were compatible with viral pneumonia in two cases and the third case was unremarkable. Coagulation profiles in the severe case were carried out and were normal. All the patients responded very well to treatment with oseltamivir and did not require intubation. Their subsequent nasopharyngeal swabs were positive for human pandemic influenza A H1N1 by real-time reverse transcription-polymerase chain reaction (RT-PCR), and their sputum for acid-fast bacilli and tuberculin skin tests were negative.

Leptospirosis, a spirochetal zoonosis, is frequently unrecognized due to its manifestation as an undifferentiated fever. It is an emerging infectious disease that has changed from an occupational disease of veterinarians, farmers, butchers, and other animal handlers to a cause of epidemics in poor and decayed urban communities in developing countries. Humans are infected when mucous membranes or abraded skin come into direct contact with the urine of infected animals, especially rats and dogs. Mortality from severe leptospirosis is high, even when optimal treatment is provided. The diagnosis of leptospirosis is based on clinical findings, history of direct or indirect exposure to infected animals in endemic areas, and positive serological tests. It should be considered in the differential diagnosis of patients with febrile illnesses associated with pneumonitis and respiratory failure, especially when hemoptysis is present. Severe pulmonary involvement in leptospirosis consists primarily of hemorrhagic pneumonitis. In advanced cases, adult respiratory distress syndrome and massive pulmonary hemorrhage may occur. Chest radiographs show bilateral alveolar infiltrates and/or resemble viral pneumonia, bronchopneumonia, tuberculosis, adult respiratory distress syndrome, and other causes of pulmonary hemorrhage such as Goodpasture syndrome. High-resolution computed tomography scans may show nodular infiltrates, areas of consolidation, ground-glass attenuation, and crazy-paving patterns. Bronchoalveolar lavage and autopsy studies have suggested that ground-glass opacities and air-space consolidations are secondary to pulmonary hemorrhage. Although not specific, the presence of these computed tomography findings in a febrile patient with an appropriate history should suggest a diagnosis of leptospirosis.

A 51 year-old Aboriginal woman was diagnosed with H1N1 influenza virus and methicillin-resistant Staphylococcus aureus. Contrasted CT of the pulmonary and bronchial vasculature undertaken for investigation for of haemoptysis revealed an 11 mm left upper lobe pulmonary artery pseudoaneurysm (PAP). The PAP was successfully treated with angioembolisation of the parent artery with two platinum coils. Pulmonary artery pseudoaneurysms are uncommon but potentially life-threatening and have not previously been reported in association with H1N1 influenza viral pneumonia, which has been prominent in recent times in both medical literature and popular press.

Diffuse alveolar hemorrhage (DAH) is a serious disease whose main clinical manifestations are hemoptysis and dyspnea. In some cases, invasive mechanical ventilation is ineffective and patients can die quickly. Extracorporeal membrane oxygenation (ECMO) is a supportive therapy that can provide oxygenation support to patients when mechanical ventilation fails. This article reports successful early initiation of veno-venous extracorporeal membrane oxygenation (V-V ECMO) in an emergency department to rescue an adult patient with diffuse alveolar hemorrhage caused by viral pneumonia.
