and pulmonary infiltrates.
Paragonimiasis
can masquerade as pulmonary tuberculosis, especially in patients from areas that are endemic for both the parasite and the tubercle bacillus. The ease and safety with which this infection can be treated, in contrast to tuberculosis, reiterates the importance of diagnosing this lung fluke when it is present.

Thirty-nine cases of pulmonary
paragonimiasis
due to Paragonimus westermani were identified in Manipur, India. Recurrent
haemoptysis
, often increased on exertion, was the most common symptom. The clinical features and radiological appearances simulated pulmonary tuberculosis and 23 cases were treated for tuberculosis for varying periods before the final diagnosis. Young males aged 11 to 30 years comprised two-thirds of the cases. The disease was acquired by eating raw or improperly cooked crabs (Potamon deham). Intradermal tests (IDT) employing a saline extract of adult Paragonimus were a simple and reliable method of detecting present or past infection. Erythema of large size even without wheal and pseudopodia may indicate a positive IDT. Charcot-Leyden crystals (CLC) were detected in all sputum samples positive for Paragonimus ova. In patients (four) presenting with pleural effusion, Paragonimus ova were not present in the sputum, stool or pleural fluid. Bithionol (2,2'-thiobis [4,6-dichlorophenol]) orally, 40 mg/kg body-weight/day, 10 to 25 doses, cured all cases and, except in one, side effects were minimal. Relapse in one patient after six months' treatment was apparently cured by a second course of Bithionol.

One hundred cases of
paragonimiasis
from Saraburi province, Thailand were studied with respect to epidemiology, clinical manifestations, radiography and treatment. Highly endemic areas were Cha-om and Sum-pugpaew villages in Kang-koy district. The ratio of male to female was 48:52. Uncooked crabs and shrimps are the second intermediate hosts of Paragonimus.
Hemoptysis
and chronic cough were the main symptoms; with crepitations and rhonchi in both lungs. There was no specific changes in chest X-rays of the lungs, except in cases of

where changes in pulmonary vessels were observed. Tomograms of the lungs provided 100% accuracy and confirmation of diagnosis of pulmonary

A patient from Southeast Asia with cough and
hemoptysis
was documented to have pulmonary
paragonimiasis
. Initial treatment with bithionol failed. The patient was then treated with praziquantel 75 mg/kg body weight a day for 2 days under an investigational protocol. Subsequent follow-up studies showed clinical improvement and indicated parasitologic cure. A concurrent asymptomatic Clonorchis infection was also cured following praziquantel treatment. Side effects were minor.

Three Laotian refugee children with chronic pulmonary complaints and findings were found to have pulmonary
paragonimiasis
during a one-year period in Chicago. These patients ranged from 8 to 11 years of age and the diagnosis was delayed five to six months in two children because of the unfamiliarity of American physicians with signs and symptoms of this disorder. Clinical manifestations included chronic cough for up to two years, apparent
hemoptysis
in two patients, lack of fever or sweats, and family history negative for tuberculosis. Physical findings included rales and dullness to percussion, clubbing (one patient), and lack of fever or respiratory distress. All three patients showed interstitial infiltrates on chest roentgenogram whereas two had multiple small cystic areas. Moderate eosinophilia was present. Paragonimus westermani ova were found in stools of two patients, in sputum of two patients, and in bronchoscopic specimens in one patient. All patients demonstrated striking clinical and radiologic improvement following treatment with bithionol (50 mg/kg every other day for 15 doses), which was well tolerated.
