A 58-year-old female, with a previous history of radical resection of malignant schwannoma arising in the maxillary sinus, presented with a metastasis to the dura mater following lung metastases 15 years after the initial diagnosis. A large mass at the left temporal region was totally removed immediately after an intratumoral hemorrhage. She subsequently died of massive hemoptysis. Although the prognosis is poor for patients with malignant schwannoma, aggressive treatment including radical surgery combined with radiotherapy and chemotherapy is recommended.

A 62-year-old man who presented with the chief complaint of hemoptysis is reported. A chest radiograph obtained on admission showed a huge cystic mass located at the posterior mediastinum. Prior to surgery, transarterial embolization was done because of continuous massive hemoptysis. An uneventful removal of the tumor was performed, and the pathological diagnosis was schwannoma. The hemoptysis was thought to have been caused by changes in the cystic formation in combination with inflammation which extended to the lung.

Neurogenic tumor of the lung is very uncommon. To the best of our knowledge, endobronchial schwannoma complicated by massive hemoptysis in a patient with neurofibromatosis 2 has not been reported previously. We report a case of endobronchial schwannoma complicated by massive hemoptysis in an 18-year-old man with neurofibromatosis 2. The diagnosis of neurofibromatosis 2 was established by demonstration of bilateral vestibular schwannomas on magnetic resonance imaging of the brain and pathologic examination of the resected brain tumors. Massive hemoptysis developed after surgical removal of the brain tumors in this patient. Bronchoscopy was done immediately after the first episode of hemoptysis and a tumor protruding from the orifice of the right lower lobe bronchus was found. Despite 2 additional bronchoscopies to stop bleeding and ameliorate airway obstruction in the consecutive 2 days, hemoptysis recurred rapidly and caused profound oxygen desaturation. The patient was subjected to right lower lobectomy and endobronchial schwannoma was evidenced pathologically.

Endotracheal and endobronchial schwannomas are extremely rare tumors of neurogenic origin. These tumors often present late. Common symptoms of hemoptysis and dyspnea result from the size and location of the tumors. Two cases of tracheobronchial schwannoma with proximal airway obstruction are reported. Various diagnostic modalities employed in the evaluation of these tumors are reviewed. Because malignant transformation is exceedingly rare, operative resection of tracheobronchial schwannomas based on the principle of conservation of lung parenchyma is emphasized.

A 56-year-old man was admitted to our hospital with a complaint of massive hemoptysis. Bronchoscopy revealed a tumor obstructed the orifice of the right lower lobe bronchus. The diagnosis of endobronchial schwannoma was made by broncho-fibroscopic biopsy. Schwannomas are benign tumors which originate from schwann cells. They rarely occur in the trachea or bronchus. On the other hand symptoms in pulmonary schwannoma are usually mild. Massive hemoptysis is extremely rare. We report a case of endobronchial schwannoma complicated by massive hemoptysis.

Primary tracheal tumours are histologically similar but 100 times less common than main stem tumours. We report a case of primary tracheal schwannoma in a middle aged man. He had chronic cough for 2 years, and developed hemoptysis and stridor in the days preceding to presentation. He was treated by resection of the tumour and primary anatomosis of the trachea. The patient recovered well after surgery and had a near normal PEFR after one and a half year of follow-up and bronchoscopic examination excluded any recurrence.

Schwannomas are benign nerve-sheath tumors that arise from Schwann cells. They can present as endobronchial lesions with symptoms of obstruction and hemoptysis. We describe the first case of multiple endobronchial schwannomas at 3 different sites in a single patient. The autofluorescence ratio was <0.5 for 2 of 3 lesions. The optimal follow-up and treatment plan for these lesions remain unknown, especially in asymptomatic patients.

Schwannomas are rare neurogenic tumor originating from Schwann cells of the nerve sheath, most frequently encountered type of posterior mediastinal tumors. In most cases, schwannomas are benign, malignant and multiple schwannomas are rare. Histopathologically, the tumor is composed of fascicles of spindle cells, which are strongly positive for S-100 proteins. Surgical resection is a treatment of choice, and prognosis is excellent. Here, we report a case of posterior mediastinal schwannoma in a 20- years old male patient who complained of right-sided back pain and two episodes of massive hemoptysis of recent onset. Contrast enhanced computed tomography (CECT) and magnetic resonance imaging of the chest showed a well circumscribed, heterogeneous mass in the posterior mediastinum, compressing the right lower lobe with widening of intervertebral foramens. CT-guided trucut biopsy revealed spindle cell neoplasm. On immunohistochemistry, tumor cells expressed strong positivity for S-100 protein. Final diagnosis was schwannoma, probably originating from the right vagus nerve. Surgical resection of the encapsulated tumor resulted in the successful recovery, without any recurrence over next one year follow up.
