Causes of
hemoptysis
in children have not been well documented in the paediatric otolaryngology literature. The aim of this retrospective review is to determine the commonest causes of
hemoptysis
in the paediatric age group presenting to an otolaryngologist. We reviewed the charts of patients presenting to an otolaryngologist at The Hospital for Sick Children, Toronto, Ontario, over a 10-year period. A total of 37 inpatients beyond the neonatal period were referred for further assessment of
hemoptysis
. Thirty-two patients (86.5%) underwent bronchoscopy to determine the cause, the
hemoptysis
resolving spontaneously in the remaining five patients without a diagnosis. Four patients who had a bronchoscopy also had no identifiable pathology. Tracheobronchitis was the commonest diagnosis (19%), followed by tracheotomy-related problems (15.5%) Other causes included bronchiectasis, aspiration of blood, pulmonary hemorrhage, foreign-body aspiration, cystic fibrosis, A-V malformation, tracheobronchial
hemangioma
, hereditary telangiectasia, laceration of a vocal cord, and pneumonia. Otolaryngologists need to be aware of the etiology of
hemoptysis
in children. The commonest causes are infection and trauma, and not vascular anomalies or neoplasms as often perceived.

We reported a case of racemose
hemangioma
of the bronchial artery and intercostal to pulmonary arterial anastomosis. A 67-year-old woman was admitted because of repeated
hemoptysis
. Bronchoscopic examination revealed a torous lesion of the right B7 bronchus. Intercostal angiography demonstrated communications between right dilated, meandered intercostal arteries and right pulmonary artery. Bronchial angiography showed dilatation and convolution of the right bronchial artery. Angiographic embolization of the right bronchial artery and the right intercostal artery was underwent. There was no recurrence of
hemoptysis
one year after the embolization procedure. We think that angiographic embolization is an effective method of treatment of

hemangioma
of the bronchial artery and intercostal to pulmonary arterial anastomosis.

. A chest X-ray film disclosed an infiltrative shadow. A bronchoscopic examination revealed a small, pulsatile, papillary protrusion in the orifice of the middle lobe bronchus, and massive bleeding from the protrusion was observed during the examination. Bronchial arteriography showed convolution and mild hypervascularization of the right bronchial artery. A middle lobectomy was performed, and the protrusion was histologically shown to be a primary racemose

Two cases with capillary
hemangioma
of the trachea and the left upper lobe bronchus are presented. The adult patients were referred to the hospital because of
hemoptysis
and cough. The chest radiographs were normal in both cases. The bronchoscopic examination revealed circumscribed lesions with a capillarized surface protruding into the lumen of the trachea and the left upper lobe bronchus, respectively. The lesions were excised in toto with flexible bronchoscopic forceps. The specimens contained typical capillary hemangiomas without any signs of malignancy. Capillary hemangioma in the bronchial tree is an extremely rare benign lesion in adults. Nevertheless, it should be considered as a possible cause of
hemoptysis
and cough.

We used thoracoscopy for the successful ligation and transection of a racemose
hemangioma
of bronchial artery. The patient was a 61-year-old woman who had been admitted to our hospital because of
hemoptysis
. Bronchoscopic examination revealed bulging lesions covered with normal bronchial mucosa in the right B5 and B8, and bronchial arteriography revealed a shunt between the right bronchial arteries and pulmonary arteries and veins. Ligation and transection of the right bronchial artery under thoracoscopy was performed.

has not recurred 9 months after the operation. Thoracoscopic ligation and transection of bronchial artery may be an effective and less invasive procedure for the treatment of racemose

A 60-year-old woman was admitted to our department for evaluation of a hepatic mass. The mass was diagnosed as a

of the liver by abdominal angiography because of typical cotton wool appearance and stretched arterial vessels and no peripheral staining. However, one month later, the mass was surgically removed because of extravasation. Histological findings of a specimen of the mass revealed that it entirely contained abundant necrotic tissue, and a small residual part after transcatheter arterial embolization was consistent with

due to a small tumor of the proximal trachea. By endoscopic removal of the tumor by flexible bronchoscopy, the diagnosis of LCH was made, and during the following year there was no recurrent

. To our knowledge, this is the first case of histologically proven LCH of the trachea.
