Four months before her death of right heart failure, a 38-year-old woman with a known inoperable echinococcosis of the liver developed fever, increasing
eosinophilia
and pleuropneumonia. After improvement of the clinical, radiological and laboratory parameters under steroid medication, two months before her death
hemoptysis
and multifocal perfusion defects in the lung scintigram as a sign of pulmonary embolism occurred, for which heparin therapy was introduced. At autopsy a recurrent embolization due to echinococcus cysts with extensive displacement of the arterial pulmonary blood circulation were found.

Following inhalation of cocaine two young men developed
haemoptysis
associated with dyspnoea. One of these patients had severe clinical symptoms. There was blood
eosinophilia
, and haemosiderin was found in the macrophages that were present in the fibroscopic alveolar lavage fluid. X-ray films of the chest showed bilateral micronodular opacities. The outcome was favourable after treatment with parenteral dexamethasone, oxygen therapy and mask-administered continuous positive pressure ventilation. The frequency of cocaine-induced alveolar haemorrhage is probably underestimated; the condition must be suspected in subjects who inhale cocaine and have
haemoptysis
, no matter how small.

A survey on pulmonary acariasis was carried out in the grain store and in the Chinese medicinal herb plant. Of 363 persons examined, 92(25.3%) were mites-positive in their sputum. 65 of them had symptoms and signs attributed to pulmonary acariasis, the incidence being 17.9%. The main clinical manifestations were productive cough,
hemoptysis
, chest pain, dyspnea asthma and marked
eosinophilia
. Roentgenogram of these cases revealed widening hilum shadow, increased and disordered lung markings, multiple cloudy shadow and nodular opacities ranging from 1-5mm in diameter scattered throughout the lower field of lungs. All the patients were treated with three courses of metronidazole. In each course a daily dose of 0.6g (0.2g tid) or 0.8g (0.4g bid) was given orally for seven days with an interval of 7-10 days between two courses. After three courses, the clinical manifestations and radiographic findings were much improved in most cases,
eosinophilia
dropped to normal limit, mites disappeared from sputum in 94.4% of patients. All these showed that metronidazole is rather effective in treating pulmonary acariasis.

924 subjects, 542 males and 382 females working in grainstoring, or herb-storing, from different parts of Anhui Province were examined for the possible existence of pulmonary acariasis. 49 cases(5.3%), 31 males and 18 females, were found positive for mites in their sputum. 83.7% of the sufferers aged 16-45. 22 out of the 22 out of the 49 sufferers developed a marked
eosinophilia
ranging from 4% to 48% and a count of 320-5,050/mm3, whereas X-ray films revealed varied degrees of widening lung hilum shadow with increased and disordered lung markings. In some cases, the chest-film showed a lot of scattered nodular shadows 1-5 mm in diameter in the lung lobes. The symptoms presented were cough, expectoration, depression in the chest, restlessness, low fever, asthma,
hemoptysis
, etc. 10 species of mites were found in the sputum of the 49 sufferers, i.e. Acarus siro, Tyrophagus putreseltiae, Aleuroglyphus ovatus, Caloglyphus berlesei, C. myoophagus, Dermatophagoides farinae, D. pteronyssinus, Euroglyphus maynei, Tarsonemus granarius, Cheyletus eruditus.

The clinical, radiologic, and pathologic features of seven patients with pulmonary dirofilariasis were studied. The findings were analyzed in conjunction with those of 76 cases previously reported from the United States. We found that, in most instances, the disease was acquired in states along the Atlantic and Gulf coasts and occurred predominantly in whites (94.7%) in their fifth or sixth decades of life, with a male to female ratio of 2:1. Symptoms, commonly chest pain, cough, or
hemoptysis
, were present in 37.6% of patients. Most patients (62.4%) were asymptomatic, and the disease was discovered incidentally on routine radiography or during the investigation of another problem. Peripheral
eosinophilia
was present in 20% of patients. The radiologic findings consisted of single (89.8%) or multiple (10.2%) pulmonary nodules that simulated primary or metastatic lung tumor. Dirofilariasis was not included in the clinical differential diagnosis in any of the patients. In one case, the diagnosis was accurately obtained by fine needle aspiration biopsy. All other patients required thoracotomy with excisional lung biopsy for diagnosis. Pathologically, the dirofilaria nodule consisted of a spherical subpleural infarct with a central thrombosed artery containing Dirofilaria immitis in various stages of disintegration.

We report a patient who developed asthma associated with exposure to cats. During his evolution he presented pulmonary infiltrates accompanied by
eosinophilia
in blood and sputum,
hemoptysis
and flu-like symptoms. The sera contained specific IgE and precipitating antibodies to cat epithelium. After a year without contact with cats, precipitating antibodies were negative, and a bronchial provocation test only demonstrated an immediate response.

In association with treatment for pharyngitis, a 47-year-old white man developed a systemic illness with fever, myalgia, episcleritis,
hemoptysis
, pleurisy,
eosinophilia
, and renal impairment. Renal biopsy revealed granulomatous interstitial nephritis, which resolved due to no specific treatment other than withdrawing all medication. Both the severe systemic manifestations and spontaneous recovery are unusual in association with drug-induced granulomatous nephritis. The drugs that may have caused the reaction included dihydrocodeine, phenylpropanolamine, erythromycin, and amoxycillin. Although the latter three drugs have been previously implicated in the development of interstitial nephritis, there have been no previous reports of granulomatous interstitial nephritis with any of these drugs.

A 47-year-old man with a history of mild asthma presented with
hemoptysis
attributed to a large multiloculated cavitary mycetoma. Peripheral blood
eosinophilia
of 43 percent led to the diagnosis of allergic bronchopulmonary aspergillosis (ABPA). Treatment of ABPA with prednisone led to resolution of an upper lobe infiltrate and a dramatic reduction in the total serum IgE level. Evaluation over a two-year period did not demonstrate enlargement of the cavity or disseminated aspergillosis.

The clinical and immunologic features of ten patients with allergic bronchopulmonary aspergillosis (ABPA), observed over periods varying from 12 months to 10 years, are reported. Acute attacks of ABPA were characterized by several, or all of: increased cough and sputum,
haemoptysis
, pleuritic pain, expectoration of sputum plugs, and increasing airways obstruction. Peripheral blood
eosinophilia
and acutely elevated serum IgE levels were seen in all patients during acute attacks, sputum
eosinophilia
and recovery of Aspergillus in sputum was less common. However, blood
eosinophilia
was not present in all attacks of ABPA and sputum
eosinophilia
varied similarly from one attack to another. Six patients with previously documented multiple precipitin lines have had no demonstrable precipitins to Aspergillus on several occasions between attacks, three of these patients have also been negative during attacks. Five of the six patients have again developed positive precipitin lines. The total number of episodes in these ten patients was fifty two, three patients have had more than nine acute attacks of ABPA. There is no seasonal variation in this group of patients. Since diagnosis, only three patients have had an attack-free interval longer than 12 months. Two patients are steroid-dependent because of severe asthma, and nine have symptoms of bronchiectasis.

A 26 year-old woman first complained at a sparse sputum stained with blood. 18 days later haemorrhagic petechias on the lower limbs skin appeared and rapidly disappeared. The gradually enhancing cough was followed by
haemoptysis
and bleeding. The death occurred at the 41st day of the disease from persisting lung bleeding. The cause of lung bleeding was Wegener's granulomatosis which was characterized by destructive angiitis, haemic and tissue
eosinophilia
, lymph node granulomatosis and diffuse glomerulonephritis.
