The diagnosis of mislaying forms of pulmonary embolism, where even angiography was not pathognomonic, induced the use of a phlebography on lower limbs in patients where this diagnosis was suspected. The revelation of a distal thrombo-phlebitis and even more so an iliocaval one, becomes a major value when the clinical and paraclinical data remained arguable. The comparative study of diagnostic usual clinical and paraclinical elements (radiographies, E.C.G., gasometries and scintigraphies) confirmed, in 22 patients, the value of phlebography. On a pneumological background, it is useful to perform this examination, most of the time : a) on patients over 55, when a cardiorespiratory decompensation remained unexplained in a patient with chronic respiratory insufficiency, or even an unproved
hemoptysis
; b) but mostly before 55, in case of painful pneumopathy above all if it is bilateral and recurring, or in case of paroxysmal bronchospasm without atopic ground, when a belated
asthma
could be thought of. Finally vascular tests induced the fitting in situ of a clamp or "umbrella" in the vena cava in 7 patients out of 22.

The patient was a 60-year-old white male who, for 18 months, had complained of a substernal wheeze on exertion, exertional dyspnoea and cough, and attacks of acute respiratory distress. There was no
haemoptysis
or dyshpagia and he was treated for bronchial
asthma
until bronchoscopy revealed the tumour which had not been recognized in plain chest films. He showed no evidence of a neurofibromatosis and apart from reduction in pulmonary function tests on a PO2 of 74, his laboratory tests were negative. There was no family history of neurofibromatosis. He underwent thoracotomy and a smooth rounded pedunculated tumour, 2.5 cm in diameter, arising from the posterior wall of the trachea, 3 cm above the carina was excised. He has had no tumour recurrence.

Three instances of intense laryngospasm and bronchospasm occurred as a result of fiberoptic bronchoscopic examination in three patients with quiescent bronchial
asthma
. The indications for the procedure were
hemoptysis
in one patient and lobar collapse in two. It is likely that vagally mediated reflex laryngospasm and bronchoconstriction occur when irritant receptors are mechanically stimulated by the bronchoscope. Therefore, in the asthmatic population with its increased airway reactivity, indications for fiberoptic bronchoscopy should be absolute, and the procedure should be performed under optimal conditions. A rationale for minimizing the risk of this procedure in patients with bronchial

Upper respiratory and pulmonary complications of cocaine addiction have been increasingly reported in recent years, with most of the patients being intravenous addicts, users of freebase, or smokers of "crack." The toxicity of cocaine is complex and is exerted via multiple central and peripheral pathways. Recurrent snorting of cocaine may result in ischemia, necrosis, and infections of the nasal mucosa, sinuses, and adjacent structures. Pulmonary complications of cocaine toxicity include pulmonary edema, pulmonary hemorrhages, pulmonary barotrauma, foreign body granulomas, cocaine related pulmonary infection, obliterative bronchiolitis,

A survey on pulmonary acariasis was carried out in the grain store and in the Chinese medicinal herb plant. Of 363 persons examined, 92(25.3%) were mites-positive in their sputum. 65 of them had symptoms and signs attributed to pulmonary acariasis, the incidence being 17.9%. The main clinical manifestations were productive cough,
hemoptysis
, chest pain, dyspnea
asthma
and marked eosinophilia. Roentgenogram of these cases revealed widening hilum shadow, increased and disordered lung markings, multiple cloudy shadow and nodular opacities ranging from 1-5mm in diameter scattered throughout the lower field of lungs. All the patients were treated with three courses of metronidazole. In each course a daily dose of 0.6g (0.2g tid) or 0.8g (0.4g bid) was given orally for seven days with an interval of 7-10 days between two courses. After three courses, the clinical manifestations and radiographic findings were much improved in most cases, eosinophilia dropped to normal limit, mites disappeared from sputum in 94.4% of patients. All these showed that metronidazole is rather effective in treating pulmonary acariasis.

We describe a case of fibrous histiocytoma of the trachea diagnosed in a 17-year-old female who presented with symptoms of '

'. Management included rigid bronchoscopy with biopsy and debulking of this obstructing tumor, later excised with partial tracheal resection. Although tracheal tumors are quite rare in children, the majority (6/9) of reported cases of fibrous histiocytoma of the trachea have been described in the pediatric age group. The possibility of a tracheal neoplasm as a cause of wheezing, stridor or
hemoptysis
in children should be recognized. Control of the airway without tracheotomy may facilitate surgical cure via tracheal resection in such cases.

924 subjects, 542 males and 382 females working in grainstoring, or herb-storing, from different parts of Anhui Province were examined for the possible existence of pulmonary acariasis. 49 cases(5.3%), 31 males and 18 females, were found positive for mites in their sputum. 83.7% of the sufferers aged 16-45. 22 out of the 22 out of the 49 sufferers developed a marked eosinophilia ranging from 4% to 48% and a count of 320-5,050/mm3, whereas X-ray films revealed varied degrees of widening lung hilum shadow with increased and disordered lung markings. In some cases, the chest-film showed a lot of scattered nodular shadows 1-5 mm in diameter in the lung lobes. The authors analysed 116 hospitalized patients who, in their routine cytologic examination of the sputum, had also a cytomorphologic finding of lymphocytes. The greatest majority of these patients, 63 of them or 54.3% suffered from malignant neoplasm. Out of these 63 patients, 53 of them or 45.7% suffered from primary bronchial carcinoma, whereas 10 patients or 8.6% had non-Hodgkin's lymphoma, metastatic lung cancer of extrathoracic primary localization, Hodgkin's lymphoma, while two patients were supposed to have lung neoplasm. Our study also revealed that 14 patients (out of 116 hospitalized patients) or 12.0% suffered from broncho-pleuropneumonia, 13 or 11.2% from an active pulmonary tuberculosis, 7 or 6.0% from a chronic obstructive bronchitis, 5.1% from sarcoidosis, 3.4% from post tuberculosis pulmonary changes while 2.5% of the patients were found to have a pleural empyema. and a pulmonary infarction were found as well. We report a 15-year-old male with bronchial

since five years old, unresponsive to treatment; at age 12 a diagnosis of pulmonary tuberculosis was made and treated with several drugs. He was referred to our hospital because of
hemoptysis
. A diagnosis of aspergillosis in the aspergilloma form was made; after, a left upper lobectomy showed the invasive form; later he presented recurrent obstructive respiratory problems, secondary to the allergic form. Serum IgE was elevated, lowering after treatment with corticoids; simultaneously he had clinical improvement. Treatment was discontinued when clinically asymptomatic and serum IgE was normal. Diagnostic route and treatment are discussed.
