We report the case of a 25-year-old woman undergoing a laparoscopic cholecystectomy, who suffered, one min after the beginning of intraperitoneal insufflation of CO2 (2.5 L at a pressure of 10 mmHg), a sudden decrease to 0.8 L.min-1 of the aortic blood flow (ABF), monitored in the descending aorta by an oesophageal echo-Doppler probe, associated with a decrease of PetCO2 to 15 mmHg and of SpO2 readings to 88%. Despite the lack of simultaneous changes in heart rate and arterial pressure, pulmonary gas embolism (GE) was suspected. The pneumoperitoneum was exsufflated and CPR was started because of circulatory inefficiency. Ten min later, efficient spontaneous cardiac activity restarted, whereas PetCO2 and ABF returned rapidly to normal values. At this time, a typical gas noise was clearly obtained through the oesophageal Doppler transducer. The patient remained in deep coma (GCS:6) with a left sided hemiplegia. However, she fully recovered after four sessions of hyperbaric oxygenation. Simultaneous continuous monitoring of ABF and PetCO2 allows an undelayed recognition of major circulatory disturbances, before significant changes in heart rate and arterial pressure occur.

In order to observe the effects of acupuncture combined with point-injection therapy on post-traumatic coma, 30 such cases were randomly divided into the following two groups. The patients in the control group were simply treated with the basic neural medical treatment; while patients in the treatment group were treated by acupuncture and point-injection therapy in addition to the above treatment. Comparisons were made between the two groups in the therapeutic effects by GCS evaluations as well as in the changes of main symptoms. The results showed that the GCS value in the treatment group was higher than that of the control group, but with no statistical significance (P>0.05). However, the main symptoms of the patients in treatment group, such as aphasia, hemiplegia, and injuries of cranial nerves (including injuries of the facial, oculomotor and abducent nerves) were obviously improved, showing significant differences as compared with the control group (P<0.05). Conclusion can be made that acupuncture combined with point-injection has the consciousness-inducing effect for post-traumatic coma, and shows good effects for the cranial nerve injuries and aphasia.

Acute hemorrhagic leukoencephalitis (AHL) is a rare demyelinating disease mainly affecting children, characterized by acute onset, progressive course and high mortality. A 62-year-old man was admitted to our Unit for diplopia and ataxia ensuing 2 weeks after the onset of pneumonia. MRI T2-weighted images showed signal hyperintensities in the brainstem. Antibodies against Mycoplasma Pneumoniae and cold agglutinins were found. Two weeks later the patient had a worsening of his conditions: he developed left hemiplegia with motor focal seizures and the day after he was deeply comatose (GCS = 4). A second MRI scan showed extensive hyperintensities involving the whole right hemisphere white matter with a small parietal hemorrhagic area. The clinical and neuroimaging features suggested the diagnosis of AHL, Aciclovir in association with steroid therapy were administered and then plasmapheresis was started. After 30 days of coma, the patient gradually reacquired consciousness and motor functions; anyway a left hemiplegia persisted.
