Acute
headaches
are in most cases significant symptoms or premonitary signs of a neurological condition. From a semiological point of view, they may be: (i) isolated, (ii) associated with neurological symptoms (ophtalmoplegia,
hemiplegia
, hemianesthesia...). From an etiological point of view, the haemorragic conditions are predominant (30%): encephalic vascular malformation with or without subarachnoidal haemorragia (21%), subarachnoidal haemorragia without malformation (6%) and subdural haematoma (3%). Two types of conditions are also frequently observed: ischemic attacks (22,3%) and inflammatory meningeal syndromes (12%). Rare cases with hypophyseal adenomas, ischemic attacks under oestro-progestative treatment, accidents of mono-amine-oxydase inhibitors and multiple sclerosis are observed. 23,8% of the cases remained without any precise diagnosis. One of the interesting points in the acute

issue is the possibility of discovering an encephalic vascular malformation without any important bleeding and, therefore, good conditions for surgery.

A 55-year-old man having
hemiplegia
after the sudden onset of a stroke was referred for rehabilitation. Cerebral angiography had demonstrated occlusion of the left middle cerebral artery and steroid therapy had been started. Attempted tapering of the steroid therapy on admission for rehabilitation resulted in the patient having severe
headaches
and confusion. Blink reflex evaluation, somatosensory cerebral evoked potential determinations and visual evoked responses were all consistent with a widespread process involving the parietal lobe of the patient's left cerebral hemisphere. Computerized axial tomography indicated an abnormality consistent with a space-occupying lesion. Craniotomy revealed the presence of a glioblastoma multiforme in the left cerebral hemisphere. Electrodiagnostic evaluation was entirely consistent with the operative finding of widespread involvement of the patient's left parietal lobe. Stroke patients whose conditions deteriorate over time must be serially evaluated in order to determine possible other causes of their symptoms.

There are patients who present with less ominous symptoms who have heretofore been though to have cerebral infarction or transient ischemic attacks who in reality have intracerebral hemorrhages. Computerized tomography has allowed us to identify these patients and to separate them from the large group of patients with ischemic infarcts either due to thromboses or emboli. Six patients were reported with unexpected brain hemorrhages and their good prognosis is emphasized.

Non-invasive investigations may yield false negative results, although in the case of radionucleide scanning and computerized axial tomography the reliability is approaching 90 per cent. The diagnosis will, however, remain an unexpected finding at angiography in a percentage of cases.

The incidence and different presentations of basilar migraine are described in a series of 132 children (80 boys, 52 girls) presenting with recurring
headache
considered to be migraine. Basilar migraine occurred in 29 children, and a further 18 had minor symptoms compatible with but not diagnostic of this form. The course was most often benign, with infrequent, fragmentary attacks and little disability. Rarely, attacks were both severe and frequent, but even then the clinical pattern and outcome usually followed a similar course. Only two children (with the youngest ages at onset) have serious disability, with mental slowing and (in one) permanent neurological sequelae. The relationship between basilar migraine and the syndrome of alternating
hemiplegia
of infancy is discussed. Because of the limited present knowledge, it is suggested that rigid definitions of migraine should be avoided.

Alternating hemiplegia in children is a rare form of "complicated" migraine. There are a number of similarities to seizure disorders and correct diagnosis may prove difficult. The clinical features of 6 patients with alternating
hemiplegia
are presented together with the results of electrophysiological, radiological, and biochemical studies. While there were a number of clinical similarities between the patients, extensive investigations failed to demonstrate significant abnormalities. Although a diagnosis of a seizure disorder was suggested at some time in all of the patients, in only 2 was it certain there was a fit.
Headaches
occurred in the eldest patient (although not always with a hemiplegic attach) while in the younger patients misery often accompanied their attacks. Intellectual status was impaired in 5 patients, although in 2 of these the cause was most likely to be perinatal difficulties. Response to various forms of treatment was generally not encouraging and concern is expressed that this alternating
hemiplegia
of childhood may carry an unfavourable prognosis.

and transient visual obscuration in the left eye. She showed evidence of embolism to the central retinal artery, and particulate matter could be seen within the retinal arterioles. Attention is drown to the fact that echocardiography now constitutes a simple, noninvasive, and highly reliable method of making this diagnosis. The propensity for embolic tumor fragments to grow and invade cerebral arterial walls is discussed along with its possible neurosurgical significance.
