One hundred and one adults (19 to 74 years of age) with cerebral palsy were interviewed and examined. There were 52 subjects with dyskinesia, 28 with spastic quadriparesis, 11 with spastic diplegia, and 10 with spastic hemiplegia. Neuromuscular dysfunction was mild in two cases, moderate in 72 and severe in 27. 76 per cent of the subjects had multiple musculoskeletal problems. In 63 per cent, these occurred under 50 years of age, suggesting that abnormal biomechanical forces and immobility had led to excessive physical stress and strain, overuse syndromes, and possibly early joint degeneration. A number of the patients had urinary complaints due to difficulties with toilet accessibility and possible neurogenic bladder. General health care seemed satisfactory for acute illnesses, but preventive health care was almost totally lacking. Treatment for the musuculoskeletal system and availability of adaptive devices were less adequate than for children with cerebral palsy.

Intermittent self-catheterization is common for patients who have neurogenic bladder associated with traumatic brain injury (TBI). Intermittent self-catheterization is considered the gold standard for bladder drainage because of the significantly decreased incidence of urinary tract infection in comparison with other catheterization methods. Occupational therapists educate patients in how to complete intermittent self-catheterization and assist them with adaptive equipment so that they may catheterize themselves independently. This case study describes adaptive equipment fabricated to allow a male patient with TBI who was independent with intermittent self-catheterization to continue independent intermittent self-catheterization after a second injury resulting in hemiplegia. Without the use of his left hand, the patient could not complete self-catheterization; therefore, a trough-shaped prop was fabricated to maintain the penis in an upright, lengthened position.

Spontaneous spinal epidural hematoma (SSEH) is a rare cause of spinal cord compression. Symptoms may include sudden-onset axial pain followed by neurologic involvement including weakness, numbness and incontinence. Here we report the case of a patient followed prospectively after surgical intervention following SSEH and recovery following inpatient rehabilitation. This patient presented with right hemiplegia, neurogenic bladder and bowel with autonomic dysfunction. the patient with significant gains in Functional Independence Measure scale that improved from 15 on admission to 35 1 month following surgery. This case suggests that treating this type of patients requires hospitals specialized in spinal cord injury.
