A 25 year old asymptomatic man with a past history of
pulmonary tuberculosis
presented with a continuous murmur. Selective arteriography revealed a left internal mammary arteriovenous malformation in communication with vessels in the left upper pulmonary lobe. No significant hemodynamic abnormalities were detected. This is the 26th reported case of internal mammary arteriovenous fistula and the 6th with a pulmonary communication. Review of the data in previous cases suggests that surgical indications are limited to symptomatic relief,
heart failure
during infancy or the possible risk of endarteritis, proximal arterial degeneration or rupture.

An analysis of 500 consecutive
pulmonary tuberculosis
cases shows that lower lungfield tuberculosis occurs in 6.8 percent of the negroid population studied and therefore shows no racial predelection. The ratio of female to male involvement was 3:1. A clear association with young women and with pregnancy with or without other infections was demonstrated. Affected men were in the much older age group. Some association with diabetes and

were also observed. The initial diagnosis of most of these patients was basal pneumonia or lung abscess. Therefore, the most useful clinical pointers were productive cough with or without haemoptysis unresponsive to various conventional antibiotics. , previous peritoneal dialysis or abdominal surgery, chronic peritonitis, splenomegaly, splenic infarcts, and pulmonary or abdominal tuberculosis. Lung asbestos body concentrations were calculated in 570 subjects. Peritoneal plaques were recorded in 163 cases. In both univariate and multivariate analyses, they were associated with cirrhosis, pulmonary nonactive tuberculosis, pleural plaques, and asbestosis. Association with asbestos body counts was also found, with a significant trend. Peritoneal plaques were described long ago in different pathologic conditions, for some of which (cirrhosis and nonactive

pulmonary tuberculosis
complicated by chronic
heart failure
(CHF). Radioimmunoassay was used to determine plasma renin activity (PRA) and serum angiotensin I and aldesterone levels. There was higher RAAS activity, as shown by elevated PRA. RAAS activity decreased during CHF treatment with angiotension-converting enzyme inhibitors (captopril, ramipril, prestarium) and an angiotensin II-receptor blocker (cosaar), which is indicative of the efficiency of CHF treatment.
