We have treated with intravenous iloprost twelve patients suffering from
cardiac insufficiency
compensated under oral digoxin (NYHA class II) associated with severe limb ischaemia due to arterial insufficiency. Our aim was to study its possible interaction on digoxin levels and to evaluate the long-term efficacy of iloprost. Although iloprost slowed the digoxin absorption by approximately one hour, we found no clinically significant difference between the digoxin pharmacokinetic data before and during treatment by iloprost. Moreover, 11 out of the 12 patients had a good clinical fate after the treatment, which persisted at 6 months. The pain disappeared in 4 and diminished in 7; and all
skin ulcers
healed. This improvement has lasted up to two and a half years in two patients. The clinical tolerance of iloprost was acceptable despite frequent headache and flushing associated with hypotension and nausea. We conclude that iloprost seems to be a very promising treatment of severe limb ischaemia when no intervention on the proximal arteries is possible. The patients on digoxin can continue their treatment without dose alteration while starting on iloprost.

, Raynaud's phenomenon and arthritis, showing no improvement after intensive immunosuppressive therapy or high dose steroids, was treated with immunoadsorption onto protein A. With a combined therapy of low-dose cortisone and bosentan and 22 sessions of immunoadsorption, his condition improved significantly and he continues in clinical remission. At the time of writing no further immunosuppressive therapy or immunoadsorption had been necessary. The patient is now 15 years old and healthy with an age-based constitution comparable to the normal population.

Topical vitamin D3 agents have emerged as important options for the treatment of psoriasis. Calcitriol, a naturally occurring and biologically active form of vitamin D3, has been developed in an ointment formulation for topical psoriasis therapy in the United States. The product has been used outside of the United States for years. Several short-term (< 6 months) clinical trials have demonstrated that calcitriol ointment 3 microg/g improves symptoms of psoriasis in participants with mild to moderate plaque psoriasis without demonstrating clinical evidence of alterations in calcium homeostasis, but little information has been available about the safety and effectiveness of continuous long-term use of calcitriol ointment. In this open-label, multicenter study, 324 participants with primarily mild to moderate chronic plaque psoriasis were treated with calcitriol ointment 3 microg/g twice daily for up to 52 weeks. A total of 136 participants completed 52 weeks of treatment. Serious adverse events (AEs) (reported by 1 participant each unless otherwise noted) included a pretibial

, hospitalization due to arteriosclerosis, breast carcinoma, and an infection (due to a dog bite). Clinical improvement in psoriasis symptoms was assessed by an investigator-rated global severity score (GSS) and participant-rated global assessment of improvement in psoriasis symptoms from baseline. Improvements in GSS were seen over the course of treatment. Calcitriol ointment 3 microg/g is a safe, effective, and well-tolerated option for the long-term treatment of chronic plaque psoriasis. Clinical improvement was maintained for up to 52 weeks, with no clinical effect on calcium homeostasis or other relevant laboratory test parameters.

with Itraconazole use. A man in his early 30s was admitted with worsening leg swelling and dyspnoea on exertion. He had been on Itraconazole for blastomyces

. His ejection fraction (EF) was found to be 10%-15%. Another man in his 50s was admitted with similar symptoms; his EF was 40%-45%. He had been on Itraconazole for forearm cellulitis. No other aetiology was identified in both patients despite extensive work-up including cardiac catheterisation. Itraconazole was stopped in both the cases. Our first patient did not improve even months after cessation of therapy and was referred for heart transplant. Our second patient improved after a few weeks, and his ejection fraction had improved on repeat testing.
