Five hundred and eighty-eight patients with mitral valve disease were studied. The incidence of systemic emboli was found to be higher in patients with pure mitral stenosis (16.6%) and mixed mitral stenosis and incompetence (19.4%) than in patients with mitral incompetence (3.1%). The patients with mitral stenosis who had episodes of systemic emboli tended to be older than 40 years, with moderate or severe valve lesions, atrial fibrillation and moderate or gross enlargement of the left atrial appendage or left atrium. , may dominate the clinical picture. The known causes are varied, and include drugs, toxins, and some infectious agents, however, in most cases a cause cannot yet be identified. Circumstantial evidence suggests that picornaviruses may initiate some cases in humans, and a very similar disease in mice caused by a picornavirus is actively under study. Studies of autoantibodies and cellular immune function support a central role for disordered immunity in the pathogenesis. The myositis-specific autoantibodies, especially those directed at certain enzymes important in protein synthesis (the aminoacyl-transfer RNA synthetases), are found in a clinically distinct subset of patients. Although most patients respond initially to corticosteroids, cytotoxic drugs are sometimes added when steroid toxicity or refractoriness develops. We describe several newer therapies under study for such cases and outline future directions in research.

, and left ventricular (LV) aneurysm is reported. A diagnosis of SLE was suspected owing to the presence of active lymphocytic myocarditis and fibrinous endocarditis at LV endomyocardial biopsy and was confirmed by identification of 4 of the 11 criteria proposed by the American
Rheumatism
Association for the definition of SLE. A 2-month period of steroid therapy was followed by a remarkable recovery of LV function and progression of endomyocarditis to a healed phase at control LV biopsy. The LV aneurysm disappeared, likely because thrombosis occurred as a result of the hypercoagulable state accompanying the presence of anticardiolipin antibodies. This is the first reported case of LV aneurysm induced by SLE and is a rare clinicohistologic documentation of the effectiveness of steroid treatment on lupus endomyocarditis.

There was no correlation between increasing microalbuminuria and patients age and duration of disease. There was also no correlation between microalbuminuria and erythrocyte sedimentation rate or immunological activity parameters (i.e. antinuclear antibodies, anti dsDNA antibodies, levels of C3 and C4 components of complement).

Correction of mitral and aortic valve diseases included double valve replacement--in 37 (94.9%) patients, mitral valve replacement and plication of aortic cusp by Trusler's method--in 2 (5.1%). From 39 consecutive operated patients at the hospital period 15 died. The main reason of death was acute

. The late results were followed-up in 21 patients. The good result were marked in 6 (28.6%) patients, satisfactory one--in 6 (28.6%) and unsatisfactory--in 1 (4.8%). 8 (38.1%) patients were died during 1 till 8 years. Aconitum kusnezoffii Reichb., one of the earliest recorded toxic species of genus Aconitum, has been used as traditional Chinese medicine and medicinal diet over the last 2500 years to treat
heart failure
congestion, neuralgia,

and gout, etc. In the present paper, four water-soluble polysaccharide fractions isolated from the tubers of A. kusnezoffii Reichb. were studied the antioxidant and immunological activities for the first time. In vitro antioxidant assays indicated that fraction WKCP-A had noticeable scavenging activities on DPPH radical, hydroxyl radical, superoxide anion, H(2)O(2) and self-oxidation of 1,2,3-phentriol, ferrous ion-chelating ability and reducing power. Moreover, the in vivo immunological assay exhibited that fractions WKCP-A and WKHP could more significantly enhance splenic lymphocyte proliferation and macrophage phagocytosis than other fractions. Therefore, the water-soluble polysaccharides from A. kusnezoffii Reichb., especially WKCP-A, have the potential to be explored as novel natural antioxidants and immunostimulating agents for using in functional foods or medicine.
