Aneurysm of the sinus of Valsalva is a rare congenital
heart disease
. It constitutes about 0.14%-1.5% of congenital heart surgery cases. Aneurysm of the sinus of Valsalva can be asymptomatic for many years. Only rupture of the sinus of Valsalva aneurysm leads to hemodynamic disturbances which in turn causes rapid deterioration of patient clinical condition. Some of the congenital heart diseases can be complicated by of infective endocarditis. This case report concerns the rupture of the sinus of Valsalva aneurysm in a 45 year old woman, which caused infective endocarditis on the tricuspid valve, which has been initially diagnosed as a complication of congenital
heart disease
in the form of the intraventricular defect.

Cardiac involvement in sarcoidosis may be incidentally discovered without any symptoms of the disease. When undiagnosed and untreated it is potentially fatal. Although there is no recommended strategy for the diagnosis, the introduction of newer technology is promising and may be useful for early diagnosis of sarcoid

and for the evaluation of response to therapy. We present a case of 53-years old woman, with asymptomatic cardiac arrhythmias and x-ray chest picture changes without any respiratory symptoms. Further assessment and the use of cardiac magnetic resonance imaging allowed a diagnosis of cardiac sarcoidosis to be made.

Every patient after heart valve replacement belongs to the risk group of infective endocarditis (IE). Some of the laryngological procedures are related to the transient bacteriaemia and consequently IE. Patients after heart valve replacement need a proper care before some laryngological procedures. The proper cooperation of cardiologist, cardiac surgeon, and laryngologist can decrease the risk of IE. In the paper the laryngological procedures for which prophylaxis against infective endocarditis is recommended have been identified. The standarts therapy for prophylaxis have been presented. The specificity of prophylaxis in the case of some other laryngological procedures have been shown. Over the last 40 years, we have witnessed an impressive revolution in the management of congenital
heart disease
(CHD). The major factors contributing to this advance include: enhanced diagnostic abilities, improved surgical procedures, sophisticated intensive care and finally better understanding of natural history of CHD. Recent epidemiological studies have documented that although CHD still remains the principal cause o death among infants, a significant increase in the number of patients with heart defects who survive into the adulthood have been observed. Nowadays, for the first time, the number of adults with CHD equals the numbe of children with this defect. Tetralogy of Fallot (TOF) is one of the most frequent congenital cyanotic heart diseases treated with either corrective or palliative surgical techniques. Successful operation followed by specialized care enable patients to have almost normal life expectancy. In the current paper, we have provided the update on the management, diagnostic approach and treatment of adult patients after the TOF correction. Figures present typical problems related to the echocardiographic evaluation.

Thyroid hormones are essential to maintain normal function of many systems including the cardiovascular system. Their excess or deficiency may upset human body homeostasis. Hyperthyroidism leads to cardiovascular system's hyperdynamic status which is characterized by tachycardia, increased difference between systolic and diastolic arterial pressure, significant increase of the stroke volume and improvement of the left ventricular diastolic function. Long-lasting thyrotoxicosis in patient with
heart disease
may result in atrial fibrillation, deterioration of angina pectoris or congestive heart failure. Hypothyroidism leads to hemodynamic disturbances which are quite different than those observed in hyperthyroidism, but cardiac symptoms are scant in clinical practice. Hypothyroidism's clinical significance is limited to atherosclerosis progression and intensification of ischaemic heart disease symptoms. Both leads to symptomatic cardiovascular system failure or its deterioration. We should emphasize that cardiovascular system dysfunction associated with thyrometabolic disturbances subsides when euthyreosis is restored. It sounds promising that there are reports suggesting a potential advantage of thyroxin treatment in patients with acute or chronic cardiovascular system diseases. These hypotheses result from the observations that heart dysfunction in hypothyroidism is similar to that observed in heart failure.

Echocardiography is a valuable tool for the evaluation of systolic and diastolic cardiac function. A high correlation between measurements of diastolic mitral inflow parameters analyzed with Doppler echocardiography and invasive methods makes the former valuable. The aim of this study was to ascertain if significant differences occur in diastolic myocardial parameters between dogs with no
heart disease
and dogs with subclinical or clinical dilated cardiomyopathy. Furthermore the aim of the study was to determine whether heart failure in dilated cardiomypathy is a result of systolic dysfunction alone or both systolic and diastolic dysfunction. Eleven parameters were analyzed: E wave, E-AT, E-DT, E time, A wave, A-AT, A-DT, A time, E+A time, E/A ratio, and IVRT. The study confirmed the value of noninvasive echocardiographic assessment of diastolic function in dogs with dilated cardiomyopathy. Significant differences were found in E wave, E-AT, E time, E/A ratio and IVRT between healthy dogs and dogs with dilated cardiomyopathy. All are characterized by a significant decrease compared to healthy dogs after taking into account age and body weight except for the E/A ratio, which significantly increased in value. There were no significant changes in any of the Doppler parameters for diastolic evaluation in subclinical cases of DCM. Advanced heart failure in dilated cardiomyopathy entails systolic and diastolic dysfunction.

was first time diagnosed. Differential diagnosis between tetralogy of Fallot and double-chambered right ventricle is discussed. At surgical exploration tetralogy of Fallot was confirmed. The patient underwent successful total surgical correction.

Double tachycardia (DT) is a coincidence of two arrhythmias. We described the coincidence of frequent and symptomatic premature ventricular contractions (PVCs) from the right ventricular outflow tract (RVOT) and atrioventricular nodal reentrant tachycardia (AVNRT). 54-years-old woman had no organic
heart disease
and 24 hours ECG Holter monitoring showed paroxysm of tachycardia and more than five thousands of PVCs. The patient was selected to invasive electrophysiology study (EPS) and radiofrequency (RF) ablation. The EPS confirmed induction of AVNRT and the presence of PVCs originating from RVOT The modification of slow pathway of AVNRT using RF ablation was performed. The ablation of PVCs was unsuccessful. After the ablation, ventricular arrhythmia remained symptomatic and patient underwent second session of RF ablation. With applications of RF current in RVOT only the reduction of PVCs was obtained but after 24 hours PVCs disappeared. During 12-month follow-up the patient remained free of any arrhythmias and palpitations.
