Acute decompensated heart failure (ADHF) is common in the elderly, who often have various heart diseases and co-morbidities. Another term, acute heart failure syndrome (AHFS) means that it is an association of heart conditions and that it must be treated immediately. An overview of ADHF treatment begins with common hyperacute-phase treatments including advanced cardiac life support, alleviation of dyspnea, and alleviation of congestion and/or hypo-perfusion. Non-invasive positive-pressure ventilation (NPPV) is often effective for dyspnea. Congestion and hypo-perfusion are assessed with Nohria-Stevenson classification, and should be properly adjusted. Then, the patients are treated differently, dependent on the clinical scenario, which helps to discriminate cardiac failure and vascular failure. After echocardiography, specific treatment for specific heart disease is performed.

According to the recent definition proposed by the Consensus conference on Acute Dialysis Quality Initiative Group, the term cardio-renal syndrome (CRS) has been used to define different clinical conditions in which heart and kidney dysfunction overlap. Type 1 CRS (acute cardio- renal syndrome) is characterized by acute worsening of cardiac function leading to AKI (5, 6) in the setting of active cardiac disease such as ADHF, while type - 2 CRS occurs in a setting of chronic heart disease. Type 3 CRS is closely link to acute kidney injury (AKI), while type 4 represent cardiovascular involvement in chronic kidney disese (CKD) patients. Type 5 CRS represent cardiac and renal involvement in several diseases such as sepsis, hepato - renal syndrome and immune - mediated diseases.

Heart failure (HF) is a leading cause of hospitalisation and healthcare costs worldwide. Acute decompensated heart failure accounts for more than 1 million hospitalisations in the US. Despite advances in the quality of acute and chronic HF disease management, gaps in knowledge about effective interventions to support the transition of care for patients with HF remain. Despite multiple trials of promising therapies, standard care consists of decongestion with IV diuretics and haemodynamic support with vasodilators and inotropes and this has remained largely unchanged during the past 45 years. Newer advances in medical innovations and structural heart disease interventions have now given promise to improved survival, outcomes and quality of life for patients with advanced HF of multiple aetiologies. In this article, we focus on structural interventions in the treatment of patients with HF.
