Some new predictors of postoperative psychosis in open-heart surgery have been identified in a multifactorial study of somatic and psychological factors: Somatic predictors are lower body weight, higher blood levels of urea nitrogen and creatinine, preoperative use of
tranquilizers
, especially in patients with double valve replacement or congenital
heart disease
and preoperative cerebral embolism in mitral valve disease. Bypass-time has not proved to be a valid predictor. Psychological predictors are distress provoked by severity and duration of illness, repeated delay of surgery as well as the manner of psychological coping with the illness.

In an attempt to commit suicide, a 32-year-old women swallowed a vast amount of psychiatric drugs, i.e.
tranquilizers
, amphetamines, hynotic and antidepressant agents. By intensive care, using high doses of catecholamines and appropriate antidota, satisfactory circulation and oxygenation could be maintained. 3 days after admission a peritonitis became apparent. A 50 cm long section of the distal ileum was found to be completely necrotic and had to be resected. However, circulation of the correspondent mesenterium was not disturbed at all. A drug-induced non-occlusive intestinal ischemia was postulated to be the pathophysiological mechanism of intestinal necrosis. Non-occlusive intestinal ischemia is rare; it has been reported in young adults intoxicated by cocaine or phenobarbital, in children with high overdosage of iron compounds, in elderly individuals suffering from low-output congestive
heart disease
and in patients treated with digitalis drugs, with or with or without overdosage.

From among the pregnant women in the Collaborative Perinatal Project of the National Institute of Neurological and Communicative Disorders and Stroke, we identified 508 who had migraine, and 3192 who had no history of migraine, of taking headache medication during the previous 12 months, and of headaches during the pregnancy. Migraineurs smoked more heavily and had a longer smoking history than their headache-free peers. Among migraineurs, smokers were not more likely to consume analgesics than nonsmokers. Regardless of smoking classification, more migraineurs consumed
tranquilizers
, amphetamines, and sleeping pills than headache-free women. Among smokers only, migraine was associated with
heart disease
, thrombosis/phlebitis, asthma, peptic ulcer, and pneumonia. In nonsmokers, migraine was associated with drug sensitivity and other allergies.

The T wave in leads II, V4, and V6 in the electrocardiograms of 1,401 consecutively entering 18-year-old male conscripts at a military processing center in 1975 were classified, and the relationship between the frequency of the T-wave changes relative to the psychosocial and medical data of the subjects was examined. One percent of the conscripts had a negative T in one or more of these leads, and twelve percent showed some deviation from the "normal T," usually a T wave of less than ten percent of the R-wave amplitude. A common finding was the presence of a "notch" in the T wave of lad V4 (a negative deflection in the middle of a positive T wave). Those with T-wave changes showed the same incidence of signs and history of
heart disease
as those with a normal T. However, those with an abnormal T showed higher heart rate and systolic blood pressure at rest. Further, they had more nervous symptoms and a higher consumption of alcohol and
tranquilizers
. These findings were as common in the larger group of flat T waves as in the smaller group with T-wave changes of the type normally attributed to organic
heart disease
(i.e., having notched, or more or less inverted T waves without ST depression). The maximal work capacity and vital capacity were slightly lower in subjects with abnormal T waves. However, this was fully explained by lesser height. It is suggested that in the majority of these cases, T-wave changes in young adults are due to increased sympathetic tone rather than to organic
heart disease
.

In 2793 respondents aged 65 and over in a survey of a geographically defined community, who were asked about falls in the preceding twelve months, the estimated annual prevalence rate (persons) of falls was 28.0%. Standardized for age, the rate was twice as high in women as in men. Rates increased with age but more steeply in men than in women. Compared with non-fallers, fallers had been in more recent contact with their general practitioner, showed higher prevalence of problems with mobility and daily living, a more frequent history of stroke and
heart disease
, and had more episodes of non-rotatory vertigo, double vision, faints and blackouts and episodes of weakness or numbness. Fallers also showed more frequent evidence suggestive of cognitive impairment. More fallers than non-fallers were taking diuretics and
tranquilizers
and these associations merit further study.

Since the rediscovery of lithium salts as a prophylactic against depression and the introduction of long-term neuroleptic drugs, Europe has experienced a stagnation in the development of new psychotropic drugs of new structure and influence radius. In the past two years, only one new antidepressant, three neuroleptic drugs and three
tranquilizers
with a particular activity spectrum and range of indication have been developed. The antidepressant maprotilin (Ludiomil) of the substance group of dibenzo-bicyclo-octadiene shows-contrary to the previously used antidepressants-an incentive-increasing and mood-elevating but also an anxiolytic effect, nearly no influence on the circulation, insignificant side effects and a prompt onset of effect after administration. Therefore, it may also be administered for people suffering from
heart disease
and old age. Its main ranges of indication are for anxious and retarded endogenous as well as involutional depressive states. In the field of neuroleptics, the trend goes to development of long-term neuroleptics and also to neuroleptic drugs with prompt efficacy. Concerning
tranquilizers
, one is searching for anxiolytic drugs with slight sedative effects. The chemical structure related to influence radii, the ranges of indication and the side effects of these recently developed drugs are presented.
