Pulmonary congestion in chronic heart disease was evaluated by lung auscultation and roentgen examination of the chest and compared with pressure recording of the pulmonary circulation in 49 cases. Whereas auscultation appeared not to be correlated to the pressures, the radiologic findings proved to be a reliable measure of increased pressures. The radiologic assessment was based on isolated flow shift to the upper lobes of the lungs, indicating slight pulmonary venous hypertension, and flow shift in association with blurring of the lower lobe vessels, indicating pulmonary interstitial oedema. In distinction to the results from acute cardiac failure interlobular septa were often demonstrated in association with pulmonary interstitial oedema, and pulmonary alveolar oedema rarely demonstrated in spite of marked pulmonary venous hypertension.

We studied the effects of pulmonary congestion on the development of atopic asthma in 31 infants with congenital heart disease. Respiratory distress did not resolve after surgery in seven patients, six of whom had a family history of allergy. Pulmonary congestion in infancy may increase the risk of atopic asthma in genetically predisposed children.

Of 183 patients with terminal renal failure (TRF) on programmed hemodialysis 12(6.5%) were diagnosed to have acute pneumonia. As a rule, the disease develops in hyperhydration and hypertension of the lesser circulation, more rarely concomitant heart disease is diagnosed which runs with general circulation insufficiency. This indicates that pneumonia in dialysis patients is secondary. Pulmonary congestion in pneumonia debut may mask infiltrative alterations. Therefore, in obscure cases it is desirable to combine x-ray examination with polyposition tomoscintigraphy of the lungs. The agent was verified in 33.3% cases. The sputum contained for the most part gram-positive bacteria. Antibacterial therapy was combined with measures against hyperhydration and hypertension. Mean duration of the treatment was 21.25 +/- 13.6 days.

Chagas disease is a pleomorphic clinical entity that has several unique features. The aim of this study is to summarise some of the recent contributions from our research group to knowledge of the morbidity and prognostic factors in Chagas heart disease. A retrospective study suggested that ischaemic stroke associated with left ventricular (LV) apical thrombi is the first clinical manifestation of Chagas disease observed in a large proportion of patients. LV function and left atrial volume (LAV) are independent risk factors for ischaemic cerebrovascular events during follow-up of Chagas heart disease patients. Pulmonary congestion in Chagas-related dilated cardiomyopathy is common but usually mild. Although early right ventricular (RV) involvement has been described, we have shown by Doppler echocardiography that RV dysfunction is evident almost exclusively when it is associated with left ventricle dilatation and functional impairment. In addition, RV dysfunction is a powerful predictor of survival in patients with heart failure secondary to Chagas disease. We have also demonstrated that LAV provides incremental prognostic information independent of clinical data and conventional echocardiographic parameters that predict survival.
