The risks and benefits of the newer oral contraceptives are evaluated, considering cancer, teratogenicity, drug interactions, cardiovascular risks, and carbohydrate metabolism. Oral contraceptives confer the lowest mortality risk of all contraceptives, except sexual abstinence, in all women under 30 and in nonsmokers through age 40 in developed countries. In less developed countries where maternal mortality can be as high as 5-10%, the risks of even nonmedically supervised oral contraceptives are dwarfed. The pill protects against ovarian cancer even after the pill is discontinued because it suppresses ovulation, and endometrial cancer because it blocks estrogen receptors. The relationship of oral contraception to breast cancer is still in dispute, but no good evidence exists for increased risk, especially with new low- dose pills. There may be a slightly increased risk of cervical cancer, although it is difficult to separate out other risk factors co-existing in pill users, such as earlier sexarche, more partners and more frequent screening. The incidence of pelvic inflammatory disease, functional ovarian cysts and ectopic pregnancy is reduced by pills. There is only 1 report of increased incidence of congenital heart disease in infants whose mothers took pills during pregnancy. Drug interactions are common, and must be managed by the physician. Among currently popular pills, only the norgestrel and levonorgestrel-containing multiphasic pills are said to decrease HDL2 and impair glucose tolerance, because they are androgenic enough to overcome the low dose of estrogen.

The Indian government's plan to introduce the new long-acting contraceptive Norplant in the National Family Planning Program under pressure from the US government is opposed because Norplant has not been adequately tested. The government has reduced the funding for the national program for eradication of malaria and tuberculosis, but it is proposing to finance a Norplant based population project for the State of Uttar Pradesh. The powers that can turn a deaf ear to the possible hazards of Norplant. Implanted in the arm of a woman, the chemical is released into the bloodstream providing contraception for 5 years. Severe adverse reactions include depression, heart disease thromboembolism, high blood pressure, and ovarian cysts. Many such long-acting contraceptives are being developed including injectables, vaccines, nasal sprays, and vaginal rings with potential permanent impairment to fertility. One of the major objectives of the Family Planning Program is the improvement of the health status of women, but the introduction of Norplant would harm healthy young women. Therefore, the group Saheli and others in the campaign demand: 1) that plans for introduction of Norplant in the Family Planning Program be halted immediately; 2) that the introduction of any other long acting invasive contraceptive such as Net-En, vaginal ring, nasal spray, and anti-fertility vaccine be banned, both on the grounds of inadequacy of the health services and loss of user controls; 3) that information on the safety aspects of Norplant and the basis on which the Drugs Controller has granted his approval be made public; 4) that each and every one of the hundreds of women who still have the implant should be located, and the implant removed; and 5) that all hormonal contraceptive preparations be banned in the social marketing program as their use involves extensive monitoring.

270 women underwent laparoscopic tubal sterilizations with Yoon silicone rings at the Obstetrics and Gynecology clinic of the Dakar teaching hospital in Senegal between 1979-84. Written consent of the husband was required for all married women. 235 women underwent sterilization because of grand multiparity and 34 for medical reasons. The medical indications included 1 case of cardiopathy, 12 of severe psychosis, and 3 of serious vasculorenal syndrome. 70 operations were done under local and 200 under general anesthesia. 1 patient was 20 years old and the rest were 33-44. The average age was 32 and the average parity was 8. Observations during the laparoscopies included 40 cases of pelvic adhesions, 10 of uterine retroversion, 30 of congestive tubes, and 13 of ovarian cysts. Placement of the rings was usually accomplished easily 2-3 cm from the cornu. In 6 cases, rings were lost in the peritoneal cavity and in 14 cases a tube was ruptured. In 1 case the extent of a patient's obesity was underestimated and an IUD was inserted instead of a ring. 1 case of allergy to pethidine and 1 case of subcutaneous emphysema were observed. 174 follow-up hysterosalpingographies were done. 1 pregnancy occurred 4 months after the sterilization operation. Postpartum hysterosalpingography revealed a broken ring. The period of hospitalization varied from 48-72 hours and averaged 60 hours.
