Degenerative changes of the cervical spine often cause the pains and vertigo as well as pathological results of otoneurological tests. The aim of the study was to estimate the frequency of the symptoms occurrence in the group of 146 patients, 63 male and 83 female, with degenerative changes of the cervical spine. Permanent or periodical
headaches
of various intensity occurred in 60% of the patients. Balance disturbances was observed in 74% of the patients, specially in women. In the cervical spine X-ray there were observed most frequently marginal degenerations with osteophytosis (16%), intervertebral disk narrowing (10%), and shallowing of the physiological lordosis (11%). In ENG examination, positional nystagmus (42-38%), gaze nystagmus (14-23%) were mainly recorded, with the eyes opened and closed. The eye-tracking pattern test was pathological in 57%. Optokinetic test results showed pathological records in 32% of the patients, unilateral weakness was observed in 22% and bilateral weakness in 17% of the patients. Observations mentioned above confirm frequent pathology and the necessity of early diagnosis and treatment of patients with degenerative changes of the cervical spine.

PMID:[The examination of vestibular system in patients with degenerative changes of the cervical spine]. 853 53
The authors review data on sumatriptan obtained in the results of their own studies performed on 78 patients in 10 Polish neurological centres. In all 154 migraine attacks were treated with 6 mg of subcutaneous sumatriptan. Significant decrease of pain intensity was achieved in 30% of cases after 15 min and 60%--after 30 min. According to the patient's own opinion, 62% of them evaluated the drug as very efficient, 23% of patients--as efficient. Adverse effects were reported by 11 patients (15%), but they were mild and transient.

The published results of transcranial Doppler (TCD) studies of main cerebral arteries, performed in the patients with idiopathic

, especially with migraine, are controversial. Some authors could not find any changes, others observed increased blood flow. These results were, however, based on single examination, and the comparison on mean velocities calculated for the whole groups. In the present study the authors decided to perform a series of TCD studies in each patient and to analyse the individual cases. It was found that blood flow velocity in the main cerebral arteries in chronic tension-type
headache
(8 cases) and cluster
headache
(7 cases) showed normal values, established for 35 healthy subjects. In migraine without aura (20 cases) and with aura (11 cases) a considerable increase (above 30%) in cerebral blood flow was found in some studies. In around 50% of the cases increased blood flow in anterior and middle cerebral arteries was found unilaterally. The authors believe that the above findings might be specific for migraine and reflect transient vasoconstriction of the vessels that, on the other head, may be the premonitory laboratory sign of the attack.

The endonasal microscopic sinus surgery (EMSS) in polyposis nasi is a grateful and in most cases successful procedure with a relative low risk of complications. In 1988 and 1989 we operated on 302 patients with polyposis nasi following the "Heermann Concept of EMSS". In each case the total ethmoidectomy with maxillary, sphenoid and frontal sinus surgery was performed. After an average of 5.2 years we started a follow-up in 185 patients including a questionnaire and clinical examination with endoscopic control and olfactometry. We found recurrent polyposis in 17.3% and a reoperation was necessary only in 6.5%. The complaints in polyposis as nasal obstruction, frontal
cephalgia
and hyposmia could be improved in more than 80%. About 26.5% of our patients suffered from bronchial asthma. Every second of them (51%) saw an improvement of asthma complaints after the operation. It is remarkable, that the clinical and endoscopic findings do not correlate in all cases with the subjective judgement of the patients. We saw patients with manifest recurrent polyposis, who did not deplore any complaints and vice versa. At last 85.4% said, that they would undergo the operation again.

The medical-environmental questionnaire, physical examination and pre-shift and post-shift spirometry have been performed in 48 furniture factory workers. The workers showed the work-related symptoms: cough, shortness of breath, chest pain,

, general malaise, skin symptoms, eye symptoms, rhinitis. No relationship was found between the spirometry values and the frequency of the symptoms. The exposed workers showed a significant post-shift reduction of the FVC, FEV1, FEV1%VC and PEF (p < 0.001). The higher drops of the spirometric parameters occurred in younger workers. The presented data show that processing of wood may be associated with the work-related respiratory symptoms and diseases in exposed workers.

Leukotrienes B4 and C4 have been assayed in CSF of 24 patients with the attacks or slowing-progressing course of multiple sclerosis, and in 23 patients with other noninflammatory diseases. Leukotrienes concentrations have been assayed with RIA technique with the use of commercially available kits manufactured by Amersham. Leukotrienes B4 and C4 levels in CSF of patients with multiple sclerosis have been 91.8 +/- 5.6, and 88.6 +/- 7.5 pg, and have been significantly higher than those in other neurological disorders (p < 0.01). Mean LTB4 and LTC4 levels have been significantly lower in patients with atherosclerotic dementia (69, 12.2 and 63, 02.9 pg/ml) or in patients with
headache
(72.7 +/- 2.8 and 64.5 +/- 8.2 pg/ml). Higher LTB4 and LTC4 levels in patients with multiple sclerosis is probably due to both increased penetration through blood-brain barrier and their synthesis in blood-brain barrier, and cerebral nervous tissue. Further investigations are necessary to show whether LTB4 and LTC4 levels may indicate a stage of inflammatory process activity and enable to draw any conclusions on efficacy of anti-inflammatory therapy.
