In the cerebrospinal fluid of two groups of patients with subacute sclerosing panencephalitis (SSPE), the determinations of LTB4 and LTC4 leukotriene concentrations were performed by radioimmunoassay (RIA). In the first group of 10 patients with chronic SSPE (from 2 to 11 years), the LTB4 and LTC4 levels were 94.8 +/- 17.0 and 97.4 +/- 16.8 pg/ml respectively, and were significantly higher than those in six patients with discopathy, epilepsy and
headache
(p < 0.01). In the second group of 10 patients with beginning SSPE (disease duration from 1 to 11 months) LTB4 level was lower (was not different from that in the control group), while LTC4 concentration was 97.6 +/- 10.8 pg/ml and again was significantly raised. Higher LTB4 and LTC4 levels in the cerebrospinal fluid of patients with SSPE indicate that these compounds participate in chronic inflammatory reaction around cerebral veins.

A number of new clinical forms of

have been described. Therapeutic possibilities have also increased (beta-blockers, calcium blockers, serotonin receptor drugs, eg. sumatriptan, microsurgery of neuralgias etc). This opens a new period for satisfactory advances in that rather marginal, as yet, area.

The direct cytostatic and cytotoxic effect of recombinant human TNF alpha on human tumor xenografts encourages further clinical investigation. The most commonly observed toxicities (even after low doses), include constitutional symptoms, such as fever, chills,

, hypotension and local inflammatory reaction at the injection point. Pharmacokinetic study has shown a rapid clearance following iv. infusion (half time 10-30 min). The phase I study has not revealed any beneficial effect so far (2-4% response), which may be due to low doses. Now the phase II investigations has started incorporating the combined application of TNF with other cytokines and cytostatics as well. The preliminary results seem to be promising.

The occurring frequency of 14 most common chemotherapy and anti-nausea drug side-effects was examined. The studies were performed on 29 women with ovarian cancer treated by total number of 125 chemotherapy courses (schedule PAC and Acy) and additionally, in order to eliminate nausea caused by the chemotherapy, by anti-nausea drugs (Zofran, Solu-Medrol, Droperidol, Metoclopramide + Fenactil, Torecan). Zofran caused the fewest number of side-effects, solu-medrol inhibited nausea and vomiting significantly, however it caused many side-effects such as flush on a face, restlessness, incitement and
headaches
. Torecan did not prevent patients from vomiting. The greatest number of side-effects was observed after droperidol and metoclopramide + fenactil treatment.

Interferon-gamma (IFN-gamma) is produced by activated T cells and probably by NK cells. Its production can be induced by mitogens, antigens and other molecules. IFN-gamma interacts with cells by binding to specific membrane receptors. IFN-gamma--1b is an Escherichia coli--derived recombinant DNA product, which has biological activity identical to natural human IFN-gamma. This IFN type is a more potent immunomodulator than IFN-alpha and IFN-beta. Long term treatment with a therapeutic dosage of IFN-gamma--1b produces a significant reduction in the incidence of serious infections in patients with chronic granulomatous disease. This cytokine can be also useful in the treatment of patients with visceral leishmaniasis, Epstein-Barr virus infections, lepromatous leprosy and other infectious diseases. Phase I and II studies have demonstrated it to be capable of producing antitumor effects, especially in metastatic renal cell carcinoma and some hematologic malignancies. Clinical trials have suggested efficacy of IFN-gamma in the treatment of severe atopic dermatitis and rheumatoid arthritis. The most common adverse reactions are fever,

and erythema at the injection site.

The immune abnormalities included an increase of CD8+ lymphocyte percentage resulting in decrease od CD4/CD8 ratio. HIV antigenemia was found 4 weeks after the presumed exposure whereas anti-HIV became detectable 2 weeks later.
