The studied material comprised 15 men with 12 of them having the episodic from of Horton's
headaches
and 3 with chronic form. In the episodic form the age of the patients was from 32 to 48 years, mean age 38. The history of the disease was from 6 to 12 years, the duration of a spell of
headaches
was 2-3 months and the patients had then 2-5 attacks of pain daily. Before the onset of treatment the patients were not given for 1 week any drugs or placebo. After determination of the index of attacks treatment was begun. For 7-10 days the patients received daily intramuscular testosterone propionate 25 mg, and then 10 mg for a similar time period. Before the treatment the index of attacks was 3.66 (with the total number of attacks 308). In the first week of treatment it decreased to 1.11 (94 attacks), in the second week it was 0.16 and in the third week 0.05. In 10 patients the attacks disappeared completely (83%), in the remaining 2 ones they regressed in a later time. Three patients with the chronic form received this treatment without effect.

In view of the still unexplained pathomechanism of Horton's cluster headaches 9 patients were given a challenge test with nitroglycerin while at the same time the attacks were blocked with indomethacin. The attacks were provoked with one dose (1 mg) of nitroglycerin. Then during 3 days the patients took 75-100 mg indomethacin which dose was quite sufficient for inhibition of the activity of arachidonic acid cyclooxygenase. On the fourth day another nitroglycerine provocation was done. No inhibitory effect of indomethacin on the attack was observed. It seems doubtful that products of arachidonic acid cyclooxygenation play any role in the pathomechanism of this
headache
.

The authors describe 3 children aged 12-15 years with severe basilar artery migraine. The follow-up was 2-3 years. In all cases attacks of throbbing
headache
occurred which showed varying intensity and lateralization in 2 cases. Only one child had always very strong right-sided

. The accompanying symptoms and signs were mostly from the area of vascularization of the basilar artery. In one child attacks of unconsciousness occurred additionally. The authors suggest that the symptoms and signs from brain-stem structures associated with attacks of
headache
may be due not only to construction of the basilar artery but may be due to a steal effect from certain regions of the brain stem in the phase of vasodilation, although the primary haemodynamic disturbance may not necessarily develop within the basilar artery system.

The authors present the results of own investigations on the occurrence of side effects following myelography with the contrast medium Amipaque administered usually by the lumbar route. Sixty patients aged 21-65 years with various diseases of the spinal cord and cauda equina were studied. Clinical and myelographic investigations were performed before and after myelography. In 6 out of 60 cases (10%) transient neurological disturbances developed including epileptic seizures, speech disturbances of the type of aphasia and dysartria, visual disturbances and twitching of lower extremities. Other symptoms and signs included:

, vomiting, collapse. One patient with cardiorespiratory failure died hours after myelography with evidence of increased symptoms of cardiorespiratory failure. EEG changes appeared after myelography in 2/3 of cases and persisted for up to 12 days. The authors call attention to the high proportion of neurological complications and EEG changes which must be taken into account when indications to myelography are considered. Particular caution is necessary in cases with coexistent cardiorespiratory failure.

203 women 19-40 years of age were administered the gestagen preparations Volidan, Cyclofarlutal, Ovulen, Ciba AC-101, and Stediril as a contraceptive. 49 other women were administered these preparations as therapeutic and/or diagnostic measures in different gynecological complications. Side effects such as spotting, acylic bleeding, nausea,
headaches
and breast swelling were more frequent with the high-dose preparations such as Volidan and Cyclofarlutal. Intolerance to the preparation and subsequent discontinuation occurred only in a small percentage of the users. All of the preparations achieved 100% effectiveness as contraceptives. It is noted that these preparations were used with some success in treating dysmenorrhea, menometrorrhagia because of hyperplasia of the endometrium, and in severe climacteric syndromes. It has also been used as a pregnancy test. Constant medical control of the administration of these preparations is necessary both when they are used as contraceptives and as therapeutic measures, particularly in the case of young women.

The authors made an attempt at treating chronic cluster
headache
with ketotifen (Zaditen, Sandoz). The drug is the first known agent stabilizing the cell membrane of mast cells which may be taken orally. The treatment was administered to 15 patients giving the drug in doses of 1 mg thrice daily for 8 weeks. A very good therapeutic effect was achieved in 8 cases. Good effect was noticed in 5 cases. In 2 cases only slight improvement was obtained. Reduction in the number of
headache
attacks occured already in the first week of treatment and progressed gradually during 8 weeks of this treatment.

Sulocton effect was evaluated in 33 patients with symptoms of brain ischaemia during atherosclerosis. Thirty patients received the drug orally for 2 months in doses of 100 mg thrice daily. Three patients discontinued the treatment earlier (two of them discontinued it because of side effects). After two months of treatment a significant improvement was observed in such disturbances important for wellbeing and social contacts as: anxiety and fear, mood depression, disequilibrium of emotion, motivation and initiative. In many patients

Forty-five cases of pituitary adenomas with clinical characteristics of acromegaly treated surgically from an approach through the nose and sphenoid sinus are presented. After the operation the somatic features of acromegaly decreased in 66% of cases,

regressed in 88%, visual field defects disappeared in 20% of cases. Selective removal of pituitary microadenoma caused return of regular menstrual cycles in 12 women and in 3 of them successful pregnancy and labour resulted. In cases of extrasellar growth of the tumour no satisfactory clinical remission was obtained.
