Eosinophilic meningitis, also named human nervous angiostrongyliasis, results from the infestation of the Nervous System by larvae of Angyostrongylus Cantonensis realizing there a parasitic deadlock. The Authors studied 54 patients whose main complaints were severe and lasting

, and paresthesias. A meningitic syndrome is not always encountered and a facial paralysis of the lower motor neuron type has been noted in a few patients. Study of the Cerebrospinal fluid shows pleiocytosis with a variable percentage of eosinophiles. Complete recovery is usual after a couple of weeks and the prognosis can be given as excellent despite of the fact that no specific therapy is known at the present time.

The outbreak of meningitis during the period from the end of August to the beginning of October 1974 in Kiel affected 45 children, particularly those in the age group between 4 and 9 years. An potential causative agents ECHO viri were identified in 15/45 cases and Coxsackie B viri in 2/45 cases. The patients presented the typical meningitic symptoms (fever,
headache
, stiffness of the neck). In all cases the disease ran a benign course. - Epidemiologically, it was possible to trace 32/45 cases back to three locally demarcated groups with demonstrable possibilities of contact for the diseased children (family, playground, kindergarden and school). The remaining 13/45 cases occurred isolated over the urban area without any visible connection. - This outbreak of meningitis must be regarded as an accumulation of illnesses resulting from contact infections by enteroviri and lacking or deficient disinfection. Besides the rather limited measures adopted to prevent the spread of such an infection, the correct psychological management of the population especially by the health authorities is of great importance in such epidemics.

Hypertensive encephalopathy is a dramatic syndrome characterized by severe elevation of blood pressure,
headache
, visual disturbances, altered mental status, and convulsions. Although the syndrome is uncommon, to recognize and treat it promptly is important or the condition may prove to be fatal. Hypertensive encephalopathy should be distinguished from other cerebral complications of severe hypertension by obtaining careful history and performing thorough physical examination. The only definitive criterion for the diagnosis of this syndrome is its prompt response to therapy. If the patient's condition does not improve with hypotensive therapy, the physician should immediately search for alternate diagnoses. Potent drugs are available for prompt reduction of blood pressure. There are few medical emergencies in which the objective response to therapy is so strikingly apparent as in hypertensive encephalopathy.

Twenty-two patients with cutaneous metastases of malignant melanoma were treated with intralesional injections of the methanol extraction residue of bacillus Calmette-Guerin (MER). The local reaction consisted of erythema and pustule formation followed by ulceration and tumor necrosis. Side effects included fever, chills,
headache
and malaise in the majority of patients; nausea, vomiting, cyanosis and hypotension occurred infrequently. Hypersensitivity reactions were not observed. Temporary abnormalities in liver function were seen in 11 of 19 patients tested. Reversible lymphopenia and thrombocytopenia developed in 7 of 17 and 7 of 18 patients, respectively. Immune function, as measured by skin tests for delayed hypersensitivity and the in vitro response of isolated lymphocytes to mitogens and microbial antigens, was not influenced by treatment with MER. Transient increases were observed in total hemolytic complement, complement components and the reduction of nitroblue-tetrazolium by neutrophils. Eight of eighteen evaluable patients showed a complete disappearance of all injected lesions. We conclude that intratumoral injection of MER is effective treatment for cutaneous metastases of malignant melanoma, with a complete response rate comparable to that observed after intralesional injection of BCG.

Aneurysmal bone cyst of the skull is rarely seen. There are 29 reported cases in the literatures as far as we could collect. We reported an additional case of aneurysmal bone cyst of the skull which originated from the right parietal bone of 4-year-old boy. We have summarized these 29 cases. The age incidence in this series is from 14 month-old to 31 year-old. There are 11 cases under the age of 10. In most cases clinical symptoms are palpable mass or
headache
and exophthalmos. Eyeball displacement and proptosis are also the symptoms when this disease occurs at the orbital roof. According to the characteristic radiographic appearance, it is "blown-out pattern with a shell of periosteal new bone over the mass or soap-bubble appearance". However in our case the radiological finding was osteolytic. It is possible to remove totally when this lesion occurs in the cranial vault, but only curettage may be performed when the skull base is involved. Total removal is the best treatment. Radiation therapy is usually done in the recurrent cases. It is necessary to follow up for at least 4 years because of the rarity of recurrence beyond 4 years after the initial treatment. We also described the other differential diseases and pathogenesis of this disease.

The efficiency of a standardised inhalation test procedure was studied by examining the reproducibility of responses to histamine and methacholine. In addition, the responses to the two agents were compared. Each set of duplicate tests was carried out on a separate day within one week, and all factors known or presumed to influence responses were carefully controlled. The results were expressed as the provocative concentration of the agent causing a 20% fall in forced expired volume in one second (PC20). Responses to histamine and methacholine were highly reproducible (coefficients of determination [r2] = 0.994 and 0.990 respectively). Responsiveness to histamine correlated closely with responsiveness to methacholine (r2 = 0.85). There was a small but significant cumulative dose effect with methacholine (P less than 0.01) but not with histamine. Side effects of throat irritation, flushing, and
headache
were more frequent with histamine than methacholine, and were dose-related. The high level of reproducibility indicates the efficiency of the test procedure. The similar severity of effects by agents with different mechanisms of action suggests that the primary cause of non-specific bronchial hyperreactivity lies at the level of bronchial smooth muscle.
