Typical cases of pollen allergen (hayfever, allergic asthma), together with isolated non-respiratory "equivalent" manifestations (urticaria, eye conditions,
headache
, etc.), are easy to detect on the basis of skin tests and the clinical history. Such manifestations may also occur in "false pollen allergy", related in most instances by atmospheric moulds (Dematiaceae), sometimes by house dust or dermatophytes (Candida Albicans, Trichophyton sp), by food or by a bacterial infection or allergy. A combination of pollen allergy and false pollen allergy is common. In cases of false pollen allergy the proportion of negative skin reactions would appear to worsen with the repeated use of prolonged action corticosteroid injections, given on a preventive basis. Similarly, these disorders, initially seasonal, change to more chronic manifestations throughout the year. Desensitization with aqueous extracts of allergens ensured the most complete protection against the causes of pollen allergy and false pollen allergy. Allergen extracts percipitated with alun (semi-retard extracts), more effective than
tyrosine
adsorbates (Pollinex) have the advantage of offering more rapid treatment without the risk of dangerous reactions. The best therapeutic results have obtained over the course of the last ten years, by the authors, combining on each occasion a semi-retard allergen with an aqueous allergen, thereby acquiring the benefit of the adjuvant effect of the first, in a course of ten to fifteen injections per year. Non specific therapy (antihistamines, cromoglycate, theophylline, etc.) retains all of its symptomatic indications. Oral corticosteroid therapy is better metabolized in the organism and has less of a disturbing effect on the circadian rhythm of cortisol, and is hence to be preferred to injections of delyaed action corticosteroid suspensions.

Varying reactions of the vegetative nerve system to various point combinations (for example: vomiting, dizziness, diarrhea, urge to urinate, fatigue or drowsiness,

), especially to the needling of Tai Chong (Li 3), induced us to perform biochemical studies before and after acupuncture treatment. A group of children and a group of adults were studied. The material studied was urine and blood; from the children, urine only. The following were determined in the urine: indolacetic acid, 5-hydroxy-indol-3-acetic acid, homovanillic acid, and vanillic-mandelic acid; in the blood,

and tryptophan (free and bound). Individual points with wide influence (He Gu = LI 4; Zu San Li = St 36; Tai Chong = Li 3) and their combination with generally effective points were tested. The needling of Tai Chong especially showed a clear increase in indolamine metabolism. Isolated increases in metabolites of catecholamine metabolism could be correlated with the patient's increased physical activity after acupuncture. Noteworthy is the observation that no significant chemical reactions were evident if local reactions to the needling no longer appeared at the end of a series of acupuncture treatments.

Amino acid levels in plasma were measured by amino acid autoanalyser in 130 convulsive children. The levels of taurine, serine and tryptophan were significantly lower in convulsive children as compared to normal control; in contrast, isoleucine, homocystine, GABA, histidine, arginine and ammonia were higher. The children with paroxysmal disorders (
headache
, dizziness and abdominal epilepsy) had the highest levels of isoleucine, histidine and arginine and the lowest levels of glutamate and cystein. Clinical seizure activity within 6 months prior to the test seemed to have no obvious effect on the plasma amino acid pattern, except for the levels of glycine and arginine tended to return to normal, and the level of GABA was significantly increased in patients with the seizure being controlled. The patients treated with carbamazepin as a single anticonvulsant had the highest GABA level compared to those with other anticonvulsants. Hyperglycinemia and hyperammonaemia were also noted in patients who took valproic acid. The levels of serine, isoleucine and phenylalanine in the CSF within 6 hours after convulsion were significantly lower than the normal control; while asparagine,

, lysine and arginine were significantly higher. The concentration of ammonia in the CSF was also elevated after convulsion as compared to the normal control. Amino acids play an important role in the generation of epilepsy and recently there has been an increasing number of studies to help determine their effects during an epileptic attack. Tyramine was measured after column separation and purification, according to the spectrofluorometric technique with 1-nitroso-2-ortophtalate aldehyde, according to Carou with couple of modifications by authors. Histamine levels ranged from 0 to 157 mg/kg and tyramine levels ranged from 3.8 to 575 mg/kg. The very high levels of histamine and tyramine in many samples of cheeses support the opinion, that sometimes the storage temperature has not been sufficient to stop bacterial multiplication and in consequence enzymatic activity of decarboxylases of native amino acids--precursors of biogenic amines in ripening cheeses.

We studied biochemical parameters related with central dopaminergic neurotransmission in migraine patients during crisis. We determined

and 3,4-dihydroxyphenylacetic acid (DOPAC) levels in cerebrospinal fluid (CSF) of 47 patients, 29 suffering migraine without aura and 18 suffering migraine with aura, comparing them with 27 control subjects.

levels did not differ significantly between patients and controls. The CSF concentration of DOPAC was 0.73 +/- 0.55 ng/ml in the control population, 3.84 +/- 2.08 ng/ml in patients with migraine without aura and 3.30 +/- 1.49 ng/ml in patients suffering migraine with aura. The concentration of DOPAC correlated positively with the intensity of

. These results suggest that patients with migraine have a central dopaminergic hyperfunction, probably related to a coexisting central dysfunction of noradrenergic neurotransmission.

The vasomotor effects of calcitonin gene-related peptide (CGRP) analogues have been studied in circular segments of fresh human cerebral arteries obtained at neurosurgical operations using a sensitive in vitro system. Human alpha-CGRP, human beta-CGRP, rat alpha-CGRP and rat beta-CGRP induced strong and potent relaxation of precontracted circular vessel segments. The Imax (maximum relaxant effect) to human calcitonin was low and the pD2 (concentration for half maximum effect) 7.7 was much lower than that of CGRP. The CGRP-1, antagonist human alpha-CGRP8-37 blocked the response to human alpha-CGRP but not to human beta-CGRP, while the putative antagonist [
Tyr
]CGRP28-37 did not. Capsaicin (10(-15)-10(-8)M) caused relaxation of the cerebral arteries by 22% of precontraction. Pre-treatment with 10(-6)M human alpha-CGRP8-37 inhibited this relaxation. Human alpha-CGRP increased the cyclic AMP content of human cerebral arteries in a concentration-dependent manner. This increase in adenylyl cyclase activity was blocked by human alpha-CGRP8-37. The results suggest that CGRP-1 receptors coupled to adenylyl cyclase are present in human cerebral arteries.

A 44-year-old woman complained of

and palpitation. Magnetic resonance imaging showed bilateral adrenal tumors 10 x 9 cm in size on the left side and 8 x 4 cm in size on the right side. CT scan revealed a 0.7 x 0.7 cm mass in the thyroid. Hormonal examinations showed high values of urinary cathecholamines and serum calcitonin. DNA sequence analysis of peripheral white blood cells revealed that codon 634 in exon 11 of the RET gene was mutated from TGC (Cys) to TAC (

). From these findings, a diagnosis was made of MEN2A with bilateral adrenal pheochromocytomas and medullary thyroid carcinoma. Bilateral adrenalectomy and thyroidectomy were performed. The same mutation of the RET gene was detected in all her 3 children, in two of whom, early stage medullary thyroid carcinoma was detected and thyroidectomy was performed. DNA analysis of the RET gene was useful for the diagnosis of carriers of MEN2A and the early detection of medullary thyroid carcinoma.

A 58-year-old African-American woman presented with a 6-month history of

. A magnetic resonance imaging scan of the head revealed a 5-cm, enhancing dura-based mass in the left parietal region. The variably cellular tumor was composed of uniform spindle cells associated with intercellular collagen and numerous radially arranged "petal-shaped" clusters of eosinophilic crystals. The tumor was diagnosed by light microscopy as a fibrous meningioma. Ultrastructural examination disclosed cells with complex interdigitating processes connected by desmosome-like cell junctions, abundant intercellular collagen fibers, and prominent, densely osmiophilic crystals featuring radiating teardrop shaped petals emanating from a central core. A positive Millon reaction showed these crystals to consist at least in part of

. By morphology, histochemistry, and ultrastructure, the crystals resembled
