80 strictly selected patients with chronic renal insufficiency with plasma creatinine values of 1.4--14.5 mg% were examined according to a fixed scheme to determine the presence of symptoms and signs of renal encephalopathy. The general cerebral symptoms complained of were headache in 33.4% of the patient material, dizziness in 30.3%, easy fatigability in 62.5%, giddiness in 18.8% and insomnia in 37.5%. The most prominent neurological findings were hyperactive deep reflexes in 30% and action tremor in 23.8%. The symptoms of organic brain syndrome were impairment of memory in 32.5%, weakness of concentration in 28.8% and lability of affect in 63.7%. Diffuse EEG abnormalities were found in 26.2%. While the clinical neuropsychiatric symptoms did not show any statistically significant correlation with the various internal medical data, a trend was observed in the greater number of pathological EEGs with an increase in the impairment of renal function. Furthermore, there was a statistically significant correlation, (alpha less than or equal to0.015) between the occurrence of pathological EEGs and the plasma creatinine and BUN values. It is remarkable that the patients with abnormal EEGs had a relatively low mean creatinine level of 5.89 mg%. The strict dietetic management of the patients is regarded as one of the deciding factors for the relatively low frequency of neuropsychiatric symptoms in the material studied.

A 35-year-old man ingested food contaminated with lindane, an insecticide containing almost pure gamma hexachlorocyclohexane. Grand mal seizures and severe acidemia developed rapidly. The seizures recurred for nearly 2 hours, then ceased. In addition, the patient had muscle weakness and pain, headaches, episodic hypertension, myoglobinuria, acute renal failure and anemia. Pancreatitis developed 13 days after the ingestion of lindane. A muscle biopsy on the 15th day of illness demonstrated widespread necrosis and regeneration of muscle fibres. The patient's condition improved and he was discharged 24 days after the onset of his illness. During the year following the poisoning the patient noted difficulty with recent memory, loss of libido and easy fatigability. One year after lindane ingestion the results of physical examination, including those for muscle power and bulk, were normal.

A 48-year-old woman underwent root canal treatment of the upper left lateral incisor and lower right second premolar. Close to the conclusion of the endodontic treatment she complained about headaches. Later on, because of aggravation of her condition, with headaches, fever, malaise, weakness, and numbness of the right limbs, she was admitted to the hospital. The disease progressed to an epileptic state, with appearance of a right hemiparesis. A brain scan and carotid arteriogram revealed the presence of a mass occupying the left parietal space. Craniotomy disclosed an abscess containing yellow pus from which Streptococcus viridans was cultured. After thorough surgical cleansing of the area, removal of the bone for decompression, and treatment with ampicillin the patient improved gradually and slowly regained the mobility of her right side.

A 54-year-old man experienced a right occipital headache accompanied by a roaring sound, nausea, vomiting, right facial weakness, and stiff neck. Vertebral angiography revealed an aneurysm of the right anterior inferior cerebellar artery (AICA) at the internal acoustic meatus which was later excised with favorable results. The literature is reviewed; operations have been reported in eight other cases. Inconstant waxing and waning cerebellopontine angle symptoms and signs can be found when a history of subarachnoid hemorrhage is lacking.

A case of primary interhemispheric subdural abscess was reported, which was not accompanied by abscess formation in other places such as subdural spaces of the convexity or in the brain. A 13-year-old school boy was admitted with 8 days' history of fever, headache and progressive weakness of the left lower extremity. Prior to admission he developed 2 spells of jacksonian seizure initiating from the left leg. Right carotid angiography was performed on admission and separation of the pericallosal and callosomarginal arteries with small avascular area was shown indicating possible abscess formation in the interhemispheric fissure. There was no evidence of purulent infection in the laboratory findings including the CSF and EEG showed diffuse slow wave discharge, more in the right frontocentral leads. He was placed on the vigorous treatment with antibiotics and steroids, and one month later the left lower weakness as well as the EEG abnormality showed some improvement. However, the separation of the anterior cerebral arterial branches became more prominent and formed a lens shaped avascular area, although the pericallosal artery was in the midline. A diagnosis of interhemispheric subdural abscess was made and was confirmed by surgery. Patient showed progressive recovery of the left lower paralysis, and the abscess was totally collapsed in the repeated angiographic follow-up study. Angiographic characteristics of the interhemispheric subdural abscess were described and particular emphasis was placed on the peculiar neurological symptoms referring to the previous reports in the literature.

The first symptoms of the disease encountered in 30 patients were paresthesia, constant dull headaches and a feeling of numbness of the face or its half. Eventually weakness, atrophia of the masticatory muscles, a drop or absence of the corneal reflexes and cerebral and focal symptoms super vened (the symptoms depended upon the direction of the neurinoma growth). Signs of hypertension were seen in 17 cases. There was also a protein-cell dissociation: 0.5--7% of protein in pleocytosis 6/3--68/3. The craniograms showed a destruction of the ground of the middle cranial cavity in 24 cases, while in 23 cases there was a destruction of the pyramidal apex. Antiographic studies demonstrated a displacement of the carotid siphon medially (18 cases), forward (8 cases) or behind (5 cases). The tumor vascular network was detected in 5 patients. The diagnosis of neurinomas of Gasser's node requires a comprehensive summarization of the clinical and x-ray findings.

This investigation was undertaken to define the "adequate" sodium concentration in the dialytic fluid allowing to maintain a stable plasma effective osmolality during dialysis. Isonatric dialysate is shown to miss this aim by inducing a predictable postdialytic hypernatremia. To avoid this effect a new approach was made. 17 clinically stabilized patients, previously dialyzed over a period of at least 2 years with a dialysate sodium concentration of 133 mEq/l, underwent dialysis with the "adequate" sodium concentration in the dialysate for over 3 years. During dialysis cramps, headache, hypotension, hypertensive crises and postdialytic weakness were reduced in frequency and nearly disappeared. No deterioration in blood pressure control occurred and improvement in some general parameters (hematocrit, glucose and insulin metabolism, well-being) was reported after prolonged treatment.

The case of a 47-year-old man who died one month after a history of paroxysmal occipital headaches, vertigo, vomiting, weakness, and sweating is presented. The death was due to a pontine softening caused by a subintimal dissecting aneurysm of the two vertebral, the basilar and the right posterior inferior cerebellar arteries. No etiological factor of the illness could be found. The clinical signs resembled those of a flap-valve tumor of the IIIrd ventricle.

A 11-year-old boy of short stature had recurrent right temporal pounding headaches of 7 months' duration, and progressive visual loss for 3 days. There was a left hemianopia, alexia without agraphia, and diffuse muscle weakness. Investigation established the presence of a mitochondrial myopathy with pyruvate and lactic acidemia and increased serum content of sarcoplasmic enzymes. On treatment with prednisone, the patient's strength and reading skill improved, symptoms resolved, and muscle enzymes returned to normal. Three attempts to reduce steroids resulted in accentuation of symptoms, seizures, weakness, regression of reading skills, and elevation of serum enzymes. The alexia was also reversible.

On Sept 21, 1973, during and following a football game at which they had participated, 57 members of an Alabama high school marching band (and one accompanying adult) experienced an illness characterized by headache, nausea, weakness, or dizziness. Six girls fainted. Thirty-six students were treated at a hospital emergency room. Those who had played wind instruments and had worn heavier uniforms including an impermeable plastic jacket overlay were affected earlier and more frequently than the others. Several organic causes were examined in an epidemiologic investigation and considered unlikely to explain the epidemic. Female preponderance, a bimodal epidemic curve, hyperventilation, relapses, and clinical features characterized by subjective complaints in the absence of physical findings suggested a syncopal reaction to heat exacerbated and propagated by mass hysteria.
