The article reports data from the WHO cross-national study on health behaviour in schoolchildren. A sample of 11,774 children aged 11, 13 and 15 years in Denmark, Finland, Norway and Sweden answered a questionnaire on social situation, health, health behaviour and lifestyle. Self assessed health is reported excellent among approximately two thirds of the Swedish children, half the Danish children, one third of the Finnish children and one fifth of the Norwegian children. In Denmark, Norway and Sweden self-assessed health is better among boys than girls. The most frequent symptom reported by the children is feeling low. Approximately half the children have experienced this condition at least once during the last week. Difficulties in getting to sleep, head ache, and feeling nervous are symptoms experienced by approximately one third of the children at least once a week. Smaller proportions of the children have experienced stomach-ache,
back pain
, bad temper and feeling dizzy. There are minor variations between countries and between age groups, but in almost all age groups in all countries more girls than boys report these symptoms. Approximately every fourth pupil have used medication against

, stomach-ache and coughs during the last month. More girls than boys have used these medications, less pupils in Denmark than in the other countries. Very small proportions have used medication against nervousness and difficulties to getting to sleep.

We reviewed 63 cases of cytologically confirmed leptomeningeal metastases (LM). 31 (49%) had solid tumors 17 (27%) had leukemia and 15 (24%) had lymphoma. The most common presenting symptom was pain (76%) with radicular discomfort (58%),
headache
(32%), neck or
back pain
(17%). The predominant neurological signs were mental status abnormalities (49%), weakness (47%), seizures (14%). The mode of presentation varied with tumor type. Patients with leukemia (18%) and lymphoma (13%) tended to present frequently with LM without systemic involvement, or during periods of apparent remission (leukemia 35%, lymphoma 27%), while patients with solid tumors had established systemic metastases (90%) at time of presentation. Laboratory studies did not vary among the groups. 71% had positive cytology on the first lumbar puncture (LP) and only 8% required more than 2 LPs. The cell count was a poor predictor of positive cytology as 29% of LP's with positive cytology and 36% of all LP's had less than 4 cells/mm. We conclude that 1) LM presents with pain and seizures more frequently than has been previously recognized; 2) LM is frequently the mode of presentation in patients with leukemia and lymphoma and; 3) cytology is positive frequently in CSF specimens with normal cell counts and chemistries.

The knowledge and opinions of health professionals of Botucatu about the frequency and severity of thirteen symptoms and signs of diseases were studied with a view to comparing then with the opinions of Botucatu's urban population. Four hundred and thirty-five active health professionals (physicians, nurses, nurse aides, health workers (orderlies) and others were interviewed. Most of them were women, with ages ranging from 25 to 44. The health workers (orderlies) were the most numerous category. In general, the last five symptoms included in the form: bloody sputum, vaginal bleeding, breast lump, seizures and bloody urine, were considered less frequent and more serious as compared with the first eight symptoms: shortness of breath, fever, weakness,

The clinicians gave more value to these two factors than the surgeons for almost all symptoms. High thoracic epidural anesthesia was administered by anesthetists in 20 patients undergoing submuscular breast augmentation. An average of 12 ml of 2% lidocaine was instilled after sedation with midazolam, 2-6 mg. The augmentation procedure averaged 90 minutes. In 3 patients, the block developed more rapidly on one side than the other, but soon became symmetrical in all; additional subcutaneous infiltration of lidocaine was necessary in 1 patient because of infraclavicular pain; ephedrine, 10 mg was needed in 2 patients to treat hypotension (greater than 20% decrease in blood pressure). Three patients felt infraclavicular pressure; 1 had a brief sensation of breathlessness; 3 had nasal stuffiness from Horner's syndrome associated with the block; none developed

, or paresthesias; and 3 had postoperative nausea. The average time from the end of the procedure to patient discharge was 96 minutes. In this limited series, high thoracic epidural anesthesia for submuscular breast augmentation was extremely satisfactory.

One hundred patients aged 18-49 yr, undergoing elective arthroscopy of the knee joint, were allocated randomly to either spinal anaesthesia using a 29-gauge spinal needle or general anaesthesia. Dural puncture was considered difficult in 18% of the patients receiving spinal anaesthesia. In three patients (6%) it was necessary to supplement the spinal anaesthetic with general anaesthesia. Spinal and general anaesthesia were otherwise uneventful in all patients. The incidence of postoperative

in the 0.33 mm needle group, while in the 0.52 mm needle group an incidence of 25% was found. The incidence of

was the same in both groups. The technique of performing spinal anaesthesia was evaluated and concluded to be slightly more difficult with a 0.33 mm needle, as estimated by the number of redirections of the needle needed to obtain cerebrospinal fluid. There were no differences between the two needles with respect to obtaining adequate spinal anaesthesia and spread of blockade.

Elevated (greater than 3.0 nmol/l) cerebrospinal fluid neopterin concentrations were observed in 20 of 21 patients with Lyme neuroborreliosis compared with three of 11 control patients with

or psychoneurotic disorders. Neopterin concentrations were correlated to mononuclear cell counts and protein concentrations in the cerebrospinal fluid (CSF). Following antibiotic treatment, CSF neopterin levels decreased. Serum neopterin levels were not significantly raised in patients with neuroborreliosis when compared to control subjects. Neopterin levels as well as cell count and protein concentration in the CSF are valuable inflammation markers of disease activity in Lyme neuroborreliosis.

The efficacy of indomethacin suppositories (100 mg 1-3 times a day) in the treatment of primary dysmenorrhoea was investigated in a double-blind, crossover study involving 40 patients, in comparison to placebo. The patients were treated for four menstrual periods-two periods with placebo and two periods with indomethacin suppositories. A dysmenorrhoeic score based on subjective estimations of nine symptoms was used, the symptoms including pelvic pain,

As compared to placebo, indomethacin suppositories led to a insignificant decrease in the frequency and severity of the associated symptoms, as evaluated by subjective rating (P less than 0.05). Indomethacin suppositories were well tolerated and there was no drop-out. No side effects were reported except for a mild burning sensation in the rectal region experienced by 3 patients on indomethacin suppositories.
