Eighteen of the 71 cases of
plague
reported in New Mexico from 1980 to 1984 were septicemic. We reviewed these cases to better describe the clinical presentation of this disorder and to identify risk factors for developing septicemic

, and gastrointestinal symptoms) and signs (tachycardia, tachypnea, and hypotension) of septicemic

are similar to those of other forms of gram-negative septicemia. Abdominal pain was reported in nearly half of the cases, and differential white blood cell counts revealed a marked shift to the left. The risk of developing septicemic
plague
was higher for persons greater than 40 years of age. Because of empirical antibiotic treatment of older persons, deaths from septicemic
plague
occurred primarily among persons less than 30 years old. Deaths from septicemic
plague
could be reduced by aggressive antibiotic therapy for patients with a clinical presentation suggesting gram-negative septicemia, especially patients less than 30 years old.

The pharmacotherapy of migraine has advanced in parallel with our understanding of the pathophysiology of the disease. It is likely that it is the gap in our knowledge of the pathogenesis as opposed to the pathophysiology of migraine which hinders development of preventative agents. This century has seen the shift from the vascular theory of migraine pathophysiology (i.e. that cranial vessels were the prime movers in the disorder and thus vasoconstriction would be the prime treatment) to a more integrated neurovascular theory. The neurovascular theory takes the view that vascular change is secondary to neural activation, so changes such as release of trigeminal neuropeptides is predicted, has been demonstrated and suggests a possible new treatment. Similarly, it may be possible to block trigeminal nociceptive transmission in the trigeminal nucleus which might avoid the vascular adverse effects that, while small,

In February 2003, the highly pathogenic avian influenza-A virus, subtype H7N7, was the causative agent of a large outbreak of fowl

in the Netherlands. Two days after visiting a poultry farm that was infected by fowl
plague
, a 57-year-old male veterinarian developed malaise,

and fever. After 8 days he was admitted to hospital with signs of pneumonia. Five days later, his condition deteriorated alarmingly. Despite extensive pharmacotherapy he died 4 days later of acute pneumonia. Influenza-A virus, subtype H7N7, was identified by means of reverse transcriptase/PCR in broncho-alveolar washings that had been obtained earlier; routine virus culture yielded the isolate A/Nederland/219/03, which differs by 14 amino-acid substitutions from the first isolate in a chicken (A/kip/Nederland/1/03). Partly as a result of this case, the preventive measures were then adjusted; people who came into contact with infected poultry were given increased possibilities for vaccination and the administration of oseltamivir.

In 1529, Sudor anglicus, the 'English Sweating Sickness', spread from England to Germany reaching the city of Augsburg. Its exact nature is unclear: the symptoms were profuse sweating, uncontrollable thirst, and
headaches
, with death occurring within hours of infection. Those who survived the first twenty-four hours returned to health. According to one source the fever arrived in Autumn 1529 and in September there were 800 deaths; another source gives November as the onset with 600 deaths. While these death rates were in fact relatively low compared with the
plague
, for instance, people were particularly frightened by the sudden appearance of an unknown fever and the speed of death. Augsburg was aware that the 'English Sweating Sickness' was spreading in Germany. What is remarkable was the quick reaction of the printing trade. Two related types of handbooks so on appeared; whichwill serve as the subject of this paper. Firstly, handbooks dealing with the fever as a medical issue, and secondly, those dealing with the fever as an issue of theology. An illustrative example of each handbook is discussed here. Authored at speed and quickly published, they reflect the urgent response to the outbreak. What is demonstrated is the need to attend both to body and soul, that the 'English Sweating Sickness' was a challenge not just to physicians but also to theologians. The printing trade seized the opportunity to meet both needs.

A severe epidemic of bubonic plague devastated Brescia in 1478. We became aware of the tragic events via a manuscript preserved in the town's Queriniana Library after it had been rediscovered and brought to light by the well-known local historian, Paolo Guerrini, who also edited the complete transcription. An important symptom of the disease, as reported from the beginning of a notary's dramatic description, was a splitting

, described by people as mal del zuchot or del mazzucco. The disease appeared slowly at the beginning of March and spread like wildfire in July; the epidemic continued in its serious form for several months until July the following year. It is worth noting that at that time in Brescia popular devotion started to be paid to Saint Rocco, the French pilgrim invoked as patron saint against the

. From the town, devotion to this Saint spread throughout the Province. In the space of a few years there was no parish nor village where Saint Rocco was not accepted among its patron saints; an oratory or altar was erected in his honour.

The aim of the meta-analysis was to provide more solid evidence for the reliability of the new classification. A systematic literature search was performed using PubMed, Armed Forces
Pest
Management Board Literature Retrieval System, and Google Scholar up to August 2012. A pooled odds ratio (OR) was calculated using either a random-effect or a fixed-effect model. A total of 16 papers were identified. Among the 11 factors studied, five symptoms demonstrated an increased risk for SDD, including bleeding [OR: 13.617; 95% confidence interval (CI): 3.281, 56.508], vomiting/nausea (OR: 1.692; 95% CI: 1.256, 2.280), abdominal pain (OR: 2.278; 95% CI: 1.631, 3.182), skin rashes (OR: 2.031; 95% CI: 1.269, 3.250), and hepatomegaly (OR: 4.751; 95% CI: 1.769, 12.570). Among the four bleeding-related symptoms including hematemesis, melena, gum bleeding, and epistaxis, only hematemesis (OR: 6.174; 95% CI: 2.66, 14.334; P < 0.001) and melena (OR: 10.351; 95% CI: 3.065, 34.956; P < 0.001) were significantly associated with SDD. No significant associations with SDD were found for gender, lethargy, retroorbital pain, diarrhea, or tourniquet test, whereas

appeared protective (OR: 0.555; 95% CI: 0.455, 0.676). The meta-analysis suggests that bleeding (hematemesis/melena), vomiting/nausea, abdominal pain, skin rashes, and hepatomegaly may predict the development of SDD in patients with DF, while

On March 22, 2015, the Agency for Toxic Substances and Disease Registry (ATSDR) was notified by the U.S. Environmental Protection Agency (EPA) of four cases of suspected acute methyl bromide toxicity among family members vacationing at a condominium resort in the U.S. Virgin Islands. Methyl bromide is a pesticide that has been banned in the United States for use in homes and other residential settings. An investigation conducted by the U.S. Virgin Islands Department of Health (VIDOH), the U.S. Virgin Islands Department of Planning and Natural Resources (DPNR), and EPA confirmed that methyl bromide had been used as a fumigant on March 18 in the building where the family had been residing, 2 days before they were transported to the hospital; three family members had life-threatening illness. On March 25, 2015, a stop-use order for methyl bromide was issued by DPNR to the pest control company that had performed the fumigation. Subsequent investigation revealed that previous fumigation with methyl bromide had occurred on October 20, 2014, at the same condominium resort. In addition to the four ill family members, 37 persons who might have been exposed to methyl bromide as a result of the October 2014 or March 2015 fumigations were identified by VIDOH and ATSDR. Standardized health questionnaires were administered to 16 of the 20 persons for whom contact information was available; six of 16 had symptoms consistent with methyl bromide exposure, including

Pest
control companies should be aware that use of methyl bromide is banned in homes and other residential settings, and clinicians should be aware of the toxicologic syndrome that exposure to methyl bromide can cause.

Super Bowl XLIX took place on February 1, 2015, in Glendale, Arizona. In preparation for this event and associated activities, the Maricopa County Department of Public Health (MCDPH) developed methods for enhanced surveillance, situational awareness, and early detection of public health emergencies. Surveillance strategies implemented from January 22 to February 6, 2015, included enhanced surveillance alerts; animal disease surveillance; review of NFL clinic visits; syndromic surveillance for emergency room visits, urgent care facilities, and hotels; real-time onsite syndromic surveillance; all-hazards mortality surveillance; emergency medical services surveillance, review of poison control center reports; media surveillance; and aberration detection algorithms for notifiable diseases. Surveillance results included increased influenzalike illness activity reported from urgent care centers and a few influenza cases reported in the NFL clinic. A cyanide single event exposure was investigated and determined not to be a public health threat. Real-time field syndromic surveillance documented minor injuries at all events and sporadic cases of gastrointestinal and neurological (mostly
headaches
) disease. Animal surveillance reports included a cat suspected of carrying
plague
and tularemia and an investigation of highly pathogenic avian influenza in a backyard chicken flock. Laboratory results in both instances were negative. Aberration detection and syndromic surveillance detected an increase in measles reports associated with a Disneyland exposure, and syndromic surveillance was used successfully during this investigation. Coordinated enhanced epidemiologic surveillance during Super Bowl XLIX increased the response capacity and preparedness of MCDPH to make informed decisions and take public health actions in a timely manner during these mass gathering events.
