Osteomata of the paranasal frontal and ethmoidal sinuses are benign, slow growing tumors generally asymptomatic. Complications due to orbital or intracranial development of the osteoma are rare and demand neurosurgical treatment. The authors report two cases. In the first a fronto-ethmoidal osteoma first caused exophtalmos and later ophtalmoplegia due to compression of the superior ophtalmic vein. In the second case the posterior development of an osteoma of the frontal sinus resulted in pneumocephalus with epileptic fits and headache and initially homolateral hemiparesis. In both cases CT Scan showed the extent of the osteoma and in the second case the gap in the wall of the sinus. Both osteomata were radically removed through frontal craniectomy. The literature is reviewed, 12 other cases of pneumocephalus due to posterior development of paranasal osteomata have been published during the last 50 years.

We present the case of a patient with old frontal headache, who was admitted with frontal acute sinusitis symptoms at the emergency service. The radiology study shows a compatible image with frontal osteoma, which was extirpated as being asymptomatic, by frontal osteoplastic technique. The case is described and it is made a frontal osteoma review and the surgical technique used to do it.

A group of 86 patients with osteomas on the bones of the skull and face, 21 suffered from osteomas of the paranasal sinuses. The mean age of the patients was 50 years, with a predominance of male subjects (2/1). The frontal sinus was the frequently involved (57%), followed by the maxillary, ethmoid and the sphenoid sinuses. Diagnosis was made accidentally in 45% of the cases. The most frequent presenting symptom was headache (57%). Simple excision of the osteoma paranasal sinus was possible in 18 cases. We realize a study of the facts, as well as a review of the literature to know the incidence, diagnosis, indications and surgery technics used in these type of osteomas.

We report a case of frontal sinus osteoma in a 66-year-old woman, with headache and deformation of the upper-inner orbit wall; it was satisfactorily solved by means of osteoplastic technique with the aid of surgical microscope. We emphasize the extrasinusal extension, and specially when the osteoma grows towards the orbit or the endocranium, in order to settle on the surgical technique.

Sixty-one patients with an osteoma of the frontal or ethmoid sinuses have been studied. The following indications for surgical removal of these osteomas are suggested: osteomas extending beyond the boundaries of the frontal sinus, if enlarging, if localized in the region adjacent to the nasofrontal duct, if signs of chronic sinusitis are present, osteomas of the ethmoid sinuses, irrespective of their size and if patients with osteomas complain of headache and other causes of headache have been excluded. The operation of choice is the osteoplastic flap operation.

A 51-year-old man complained of a headache of 2-months duration. Computerized tomography revealed pneumocephalus and an osteoma at the confluence of the frontal and ethmoid sinuses on the right. Craniotomy confirmed an osteoma with a spur as the cause of the pneumocephalus.

During a 3-year period, all inpatients in the psychiatry unit underwent routine screening computed tomography (CT) in an effort to detect clinically unsuspected intracranial abnormalities. Of 261 patients examined who had no focal neurologic deficits, 103 had schizophrenia, 71 had depression, 48 had bipolar disorders, and 39 had paranoid delusions. Findings on 230 (88.1%) of the CT scans were within normal limits, and 27 (10.4%) showed only cortical atrophy. The remaining four cases (1.5%) demonstrated basal ganglia calcification (n = 2), old lacunar infarction (n = 1), or osteoma arising from the inner table of the skull (n = 1), all of which were considered to be clinically unrelated to the patients' psychiatric conditions. In the absence of focal neurologic deficits or other findings suggesting an intracranial abnormality (eg, papilledema, seizures, persistent or increasing headaches), there is no justification for routine CT scanning in patients admitted to the hospital for psychiatric disorders.

Osteomas of the frontal sinus are usually symptomless. When they enlarge they may produce pressure symptoms such as headaches. Small osteomas developing near the fronto-ethmoidal duct may interfere with the patency of the duct and impair the aeration of the sinus. This is a report of a case in which a small osteoma was found to be the cause of severe chronic headaches.

Presenting complaints include headaches, cerebral symptoms, or visual disturbances, depending upon the location of the tumor. This paper describes an unusual case of ethmoid sinus osteoma, with epiphora as the primary symptom; it also caused the failure of a previous dacryocystorhinostomy. This case illustrates the need for thorough evaluation of epiphora, including sinus x-rays.
