A case of corpus callosum lipoma with presumptive diagnosis is presented. Review of the literature disclosed 84 cases with such diagnoses. Lipoma of the corpus callosum is a rare intracranial lesion, perhaps congenital and often asymptomatic, but can present with seizure disorder, headache, mental changes, paresis or paralysis. Twenty-one patients had been operated upon. Surgical treatment seems to be of no value in this disease.

A case of lipoma of the corpus callosum associated with subcutaneous lipoma, whose symptoms were headache and trembling of the extremities, was reported. Roentgenograms showed characteristic findings and computerized tomography revealed a mass of very low density. The literature was reviewed and 15 cases not included in previous compilations were found. Clinical and radiological findings in 89 cases including the present case were analyzed.

Lipoma of the corpus callosum is a rare congenital condition, often asymptomatic, but which may present as epilepsy, hemiplegia, dementia, or headaches. This paper reviews the condition and reports the only two cases which are known to the Hospital for Sick Children, Great Ormond Street, London. The second case demonstrated the value of computerised axial tomography (EMI scan) in making the diagnosis and showing associated anomalies.

A patient with normal motor and intellectual development suffered headache; especially over the occipital area; and, blurring of vision for five months. Computerized tomography and magnetic resonance imaging of the brain showed a lipoma in the quadrigeminal/superior cerebellar cistern compressing of the aqueduct, inferior colliculus, and vermis. Using the modified sitting position, the tumor was almost totally removed via the infratentorial supracerebellar approach. Postoperative headache and blurring of vision had been improved, but ataxic gait and mild diplopia due to right superior oblique palsy was noted. Ataxic gait was disappeared in half a month and diplopia in two months respectively.

A 52-year-old female was admitted to our clinic with chief complaint of headache since 2 months ago. Plain computed tomography (CT) revealed low density mass in her right temporal lobe beneath the sylvian fissure with mean Hounsfield number of -52. On admission, no neurological deficit was found. This low density mass had no enhancement region. No abnormal finding was seen in plain skull roentgenogram and right carotid angiography. No epileptic pattern was found electroencephalographically. Right fronto-temporal craniotomy and total removal of tumor was performed to confirm the histological nature of this tumor. The tumor was elastic soft with yellowish color and totally removed. Histological examination revieled typical fatty cells and many capillaries. Her postoperative course was uneventful and was discharged with no neurological deficit. Intracranial lipoma is one of rare brain tumors with incidence of 0.03% or 0.046% in all brain tumor. These tumors are most frequently located in the midline structure, especially in the corpus callosum. After introduction of CT scan, however, lipomas of the extraaxial locations are also reported. From review of literature, location of the lipomas may be divided into 2 groups: the one is located in the midline or paramedian structures of the brain and the other is the extraaxial region such as the sylvian fissure or the cerebellopontine angle. The former group is often associated with anomalies of the central nervous system related to dysraphism. However, the latter group is seemed to have rare association with dysraphism.(ABSTRACT TRUNCATED AT 250 WORDS)

A 53-year-old woman suffered from sudden onset of severe headache on February 28 in 1982. She was admitted to our hospital soon after onset. On admission, she had a severe headache and nausea, and her consciousness was drowsy. CT scan showed a marked subarachnoid hemorrhage with intracerebral and intraventricular hematoma, a separation of the lateral ventricles with enlargement of posterior horns, and deformity of anterior horn. By these findings, the patient was diagnosed as an agenesis of the corpus callosum with subarachnoid hemorrhage. Left carotid arteriogram revealed an azygos anterior cerebral artery and an aneurysm at the terminal portion of this artery. Surgery was performed on the 24 th day after subarachnoid hemorrhage. Operative finding revealed a little finger's head-sized tumor was situated over the right frontobasal artery. An azygos anterior cerebral artery aneurysm was clipped and tumor was removed. Pathological diagnosis of the tumor was a lipoma. Namely, she had an agenesis of the corpus callosum, an azygos anterior cerebral artery, an aneurysm and a lipoma. There are some reports in which an agenesis of the corpus callosum is accompanied with a lipoma and an azygos anterior cerebral artery is accompanied with an aneurysm. But a case of agenesis of the corpus callosum with an azygos anterior cerebral artery was rarely reported. So this rare case with these anomalies was reported and pathogenesis about the development of 4 anomalies was also discussed.

Lipoma of corpus callosum and choroid plexus lipoma are rare developmental anomalies arising as a result of maldifferentiation of primitive meningeal tissue. Two cases are reported along with a brief review of literature. The first case presented with chronic headache, and a large lipoma of corpus callosum associated with choroid plexus lipoma was discovered. The second case had no symptoms before the discovery of corpus callosal lipoma on computed tomography, which was done for a head injury. None of the lipomas showed any calcification.

Intracranial lipoma is rare. The case reported here was a woman aged 60. She came to our hospital with complaint of headache. Neurological examination was negative. Plain craniogram showed abnormal calcification in the callosal region. A-P view of right carotid angiogram revealed winding sinous course of bilateral anterior cerebral arteries around the calcific deposit. Lateral view showed vertical stretching of ascending portion of the anterior cerebral artery. In the frontal projection of the pneumogram, the lateral ventricles were separated and concave medially with hornlike superior angles. The third ventricle extended upward between the lateral ventricles. The plain CT scan revealed an abnormal low density area (Hounsfield's number-90- -50) in the callosal region. The calcific deposit was seen within a low density area. At operation, the callosal tumor was partially removed. Histologically, lipomatous tissue was demonstrated. In reference to literature, we discussed the roentgenographic findings, especially computed tomography of callosal lipoma.

A case of large intracranial lipoma involving the suprasellar cistern is presented in a 50-yr-old man with a history of occipital headaches. Evaluations by routine computed tomography (CT) combined with metrizamide CT cisternography were extremely useful in the diagnosis of this rare condition. Intracranial lipomas are reviewed in the literature and the role of CT evaluations is discussed.
