This paper reports on the findings of a cross-sectional study of 79 data entry clerks employed by an Ontario-based organization in five separate offices. The study was conducted to identify factors associated with physical complaints experienced by these office workers. A large percentage of the data entry clerks reported chronic physical complaints (
eyestrain
, musculo-skeletal pains,
headache
) as well as emotional or mental stress which they felt were associated with their occupation. Poorly designed overhead lighting systems in all offices contributed to both
eyestrain
and
headache
. Duration of employment as a data entry operator within the organization was not associated with stress or physical health complaints. The presence of an adjustable workstation did not in itself guarantee any protection from these conditions. Highly significant associations between occupational stress levels and some physical complaints were observed. The presence of these associations indicated that the management of both physical and stress complaints within these office environments should be tackled simultaneously.

Pain around the eye can be caused by local ophthalmic disorders or by disease of other structures sharing trigeminal nerve sensory innervation. In general, most ocular causes for pain also cause the eye to be red, thus alerting the examiner to the focality of the problem. However, conditions like
eyestrain
, intermittent angleclosure glaucoma or neovascular glaucoma, and low-grade intraocular inflammation can be painful and not be associated with obvious redness. Ocular signs and symptoms also occur with numerous other causes of

. Double vision in association with periocular pain can result from orbital lesions, isolated cranial neuropathies, and cavernous sinus lesions. Pupillary abnormalities like Horner's syndrome may result from a variety of painful conditions, including cluster

, parasellar neoplasms or aneurysms, internal carotid dissection or occlusion, and Tolosa-Hunt syndrome. Pain with a dilated and unreactive pupil may reflect a benign condition like Adie's syndrome or ophthalmoplegic migraine, or it may herald the presence of a life-threatening posterior communicating artery aneurysm.
Headache
and transient visual loss can be manifestations of classic migraine, or be symptoms of ocular hypoperfusion from ipsilateral internal carotid occlusion or increased intracranial pressure from pseudotumor cerebri. In a young patient,
head pain
with a fixed visual deficit may result from optic neuritis, in an older adult, temporal arteritis may be the culprit. Ophthalmologic aspects of

thus encompass problems that range from simple and benign to complex and formidable.

An epidemiological survey on 30,000VDT operators has been carried out to evaluate the relationship between
asthenopia
and monitor characteristics. A VDU operator has been classified as asthenopeic if he complained about at least two of the following ten symptoms:
headache
, tearing, eye smarting, blurred vision, double vision, ocular itching, photophobia, blinking, nausea, eye heaviness. Visual discomfort has been related to 1) the presence of flicker; the possibility to regulate, 2) brightness, 3) height; and 4) inclination of monitor.
Asthenopia
has resulted statistically correlated to the presence of flicker and to the impossibility of regulating height and inclination of monitor for both sexes. The possibility to regulate monitor brightness has not determined a reduction of visual discomfort either in men or in women.

Eye strain

are common ocular symptoms, and the main cause in persons aged 15 to 40 is convergence insufficiency. It is more common in females than in males. Many patients are supplied with glasses for eye strain without the causative convergence insufficiency being treated. The treatment in most cases is orthoptic exercises.

A retrospective review of the records of 114 subjects with accommodative dysfunction has been completed. Most subjects (N = 96) were found to have accommodative insufficiency. Lesser numbers of subjects were categorized in the class of infacility of accommodation (N = 14), spasm of accommodation (N = 3) and fatigue of accommodation (N = 1). A majority of the subjects presented with complaints of blur,
headaches
and/or
asthenopia
while attempting nearwork. Most subjects presented with reduced abilities in one or more of the following areas: accommodative amplitude and facility, fusional vergences, near point of convergence and stereo acuities. The clinical characteristics of the group as a whole and the major subgroups have been examined both before and after treatment of the condition with orthoptic exercises and/or plus lenses at the nearpoint. The result of the treatment indicates that although most subjects (96%) experienced some relief with treatment only about half (53%) had their problems totally solved. The importance of these findings is briefly discussed.

Seventy-three patients with
asthenopia
and 43 control subjects were examined with the Pola-Test. The two populations showed a significantly different frequency of pathologic results on the four test patterns. The so-called "stereo test" seems to be the most important test pattern. Patients with a pathologic stereo pattern reported asthenopic symptoms significantly more frequently than the control subjects who were questioned for similar symptoms. However, since 30% of the asthenopic patients showed normal results on the four Pola-Test patterns, the indication for treatment can be based only on a combined evaluation of binocular tests and asthenopic symptoms. visual and conjunctival symptoms are more prominent than

. At times they may be masked by psychogenic factors. In a double-blind study on 12 asthenopic patients symptoms improved significantly during treatment with prisms.

A study was carried out by seven ophthalmologists practising in the Berne region on 473 patients complaining of eye strain. Medical histories were taken by questionnaire, refraction was done and phoria measured, followed by therapy in accordance with the physician's opinion. In 380 cases, glasses were prescribed, including prismatic glasses for 14 patients. In 123 cases, local medical therapy was instituted. The statistica evaluation of pre-treatment findings and results of therapy, was carried out using a computer. The principial complaints before treatment were

and visual disturbances, followed by conjunctival irritation and

. Women complained more about

, men more about visual disturbances. A majority of the patients with visual disturbances needed minus lenses, while for the treatment of
headaches
mostly plus lenses were needed. In patients with conjunctival irritation local medical treatment resulted in additional relief of symptoms. Cylindric correction had only a minor influence on the result of therapy. Heterophoric patients did not have more complaints than orthophoric ones, and results with spectacles without prisms were the same as in orthophoric patients.

Virtual reality (VR) has become increasingly well-known over the last few years. However, little is known about the side-effects of prolonged immersion in VR. This study set out to investigate the frequency of occurrence and severity of side-effects of using an immersion VR system. Out of 146 subjects, 61% reported symptoms of malaise at some point during a 20-min immersion and 10-min post-immersion period. These ranged from symptoms such as dizziness, stomach awareness,
headaches
,
eyestrain
and lightheadedness to severe nausea. These symptoms caused 5% of the subjects to withdraw from the experiment before completing their 20-min immersion period. Further research needs to be conducted that attempts to identify those factors that play a causative role in the side-effects of the VR system, and that looks for methods of reducing these side-effects.

The authors report the frequency of side effects of light therapy in 105 patients with seasonal affective disorder treated with three intensities of light. Common symptoms to emerge during treatment were
headache
(19%),
eyestrain
(17%), and feeling "wired" (14%). There was no relationship between side effects and intensity of light used.
