The primary aim of the study was to evaluate practice differences in reported morbidity in the second and third national morbidity surveys (1970/71, 1981/82) and to discuss their cause. A secondary aim concerned the validation of trends identified from analysis of the data from the total populations in the practices. Altogether 19 practices participated in both surveys. Annual prevalences (that is, the number of patients attending the general practitioner with a condition per 1000 persons at risk) were examined for: all conditions; each of three categories of seriousness of disease; diseases aggregated by chapter of the International classification of diseases; and each of 130 rubrics of the disease classification. Annual prevalence for 'all conditions' was approximately the same for males in both surveys, whereas for females there was an increase. In both sexes, annual prevalence for 'serious conditions' increased slightly and for 'trivial conditions' increased substantially. For 'intermediate conditions', there was a modest decrease in males. In the analysis at ICD chapter level, substantial increases in prevalence occurred in infectious diseases, nervous system diseases, circulatory diseases, genitourinary diseases, musculoskeletal diseases, symptoms, signs and ill-defined conditions, injuries and poisonings. Decreases were found in blood diseases, mental disorders and digestive diseases. Among 130 individual conditions examined, increased annual prevalence was found for mumps, fungal infections, hypothyroidism, diabetes, gout, senile dementia, angina, left heart failure, catarrh, hay fever and asthma, orchitis, acne, osteoarthritis and for some symptoms. Decreases were found for iron deficiency anaemia, anxiety state, refractive errors, haemorrhoids, chronic bronchitis, functional disorders of the stomach, carbuncle and skin infections.(ABSTRACT TRUNCATED AT 250 WORDS)

Laboratory tests are sometimes combined into "panels," presumably to facilitate a swift and accurate diagnosis. "Rheumatic panels" were available from 16 of 17 members of the American Clinical Laboratory Association. Panels included an average of five tests (range three to 11). Panel prices ranged from $25 to $189. The three tests most common in the available panels were those for rheumatoid factor, antinuclear antibody, and uric acid level. A panel combining these three tests would have a positive predictive value of only 34.6% in identifying rheumatoid arthritis, systemic lupus erythematosus, or gout in a population with joint pain, in which the combined prevalence of these diseases is estimated to be 10%. Therefore, 65.4% of persons with a "positive" test would not have one of these three rheumatic diseases. Lack of independence between diseases and second tests (for example, positive antinuclear antibodies in rheumatoid arthritis) increases misclassification errors. A careful history and physical examination along with serial ordering of a few selected tests appear optimal to establish a clinical diagnosis of a rheumatic disease.

This study compared the effects of azapropazone and indomethacin plus allopurinol in the management of acute gout and hyperuricaemia. A group of 93 patients predominantly based in general practice were randomly allocated to the two treatment regimens (azapropazone (days 1-225) or indomethacin (1-28) followed by allopurinol (29-225)) on a double-blind double dummy basis. Azapropazone produced a substantial reduction in serum uric acid levels by day 4 compared with day 1 (P<0.002) and was superior to indomethacin with regard to recorded levels of serum uric acid at day 4 (P<0.01) and day 28 (P<0.05). From day 28 onwards allopurinol produced and azapropazone maintained similar reductions in serum uric acid. Both treatments rapidly controlled the initial acute attacks of gout and both produced side effects similar in frequency and nature. Fewer breakthrough attacks of gout occurred in the azapropazone group (12) than the indomethacin/allopurinol group (21).Although the results achieved in both treatment groups were similar it has been shown that azapropazone is effective monotherapy for controlling both acute attacks of gout and hyperuricaemia.

Hyperuricemia is the specific biochemical vulnerability of gout sufferers. However, hyperuricemia is much more frequent than gout. Gout also is more frequent in men; after menopause gout attacks become more frequent in women; psychosocial factors, like stressful life events, tension, and anxiety can trigger attacks of gout, which modern pharmacology can moderate and control.

Gout is the commonest cause of inflammatory arthritis among young men. A case of acute gout in a 39-year-old man is described. As part of his assessment, his cardiovascular risk factors were evaluated. He was found to have elevated body mass index, central obesity, hypertension, a family history of cardiovascular disease, and hypercholesterolemia. This case highlights the association between gout, hyperuricaemia and elevated cardiovascular risk. In young male patients, acute presentation with gout in primary care may provide the first opportunity for assessment of cardiovascular risk factors and primary prevention of ischaemic heart disease.
